Resumo Atualmente, os ecossistemas urbanos tornaram-se cada vez mais propícios aos pombos, e a população dessas aves aumentou significativamente em várias partes do mundo, incluindo o Brasil. Esse aumento se tornou um problema ambiental e de saúde pública. O objetivo desta pesquisa foi descrever a ocorrência de Cryptococcus neoformans nos excrementos de pombos em locais públicos do município de Redenção, Pará, Brasil. Trata-se de um estudo transversal, quantitativo e descritivo. Foram selecionados dez bairros, onde foram coletadas 5 amostras de cada um, totalizando 50 unidades de material. O fungo foi identificado através de excrementos suspensos com solução estéril de cloreto de sódio a 0,9%, seguida de semeadura em ágar Sabouraud Dextrose mais cloranfenicol. Um exame direto das colônias com tinta chinesa foi realizado em triplicata para observar a sugestiva micromorfologia de C. neoformans. Um teste de identificação confirmatória foi realizado pelo teste de produção de urease. Das 50 amostras examinadas, 16% (n = 8) foram positivas para C. neoformans e nos 10 bairros estudados, 60% (n = 6) foram positivos para o fungo. Os resultados mostraram que metade das amostras positivas foram encontradas em apenas dois bairros, sugerindo que a presença de C. neoformans em local público na cidade estudada pode ser considerada uma fonte potencial de infecção para humanos e animais.
Introduction: Hemodialysis is the main treatment for patients with Chronic Renal Insufficiency and one of the main complications resulting from it is the occurrence of infections caused by opportunistic microorganisms belonging to the “ESKAPE” group. Aim: to determine the frequency of isolation of pathogens from the ESKAPE group and evaluate their respective antimicrobial resistance profiles in hemodialysis patients in a medium and high complexity hospital located in the Amazon. Outlining: Retrospective study carried out with collection of data available in the medical records of 167 patients diagnosed with Chronic Renal Insufficiency, undergoing hemodialysis or peritoneal dialysis from 2015 to 2019 in a regional hospital in the Southeast of Pará. Results: Of the 174 bacteriological cultures analyzed, 70.1% were positive and, of these, 57.4% were positive for bacteria belonging to the ESKAPE group. The most frequent microorganism belonging to the ESKAPE group was Staphylococcus aureus (47.1%); as to the resistance levels, 50% of Acinetobacter baumannii were resistant to carbapenems. Implications: The phenotypic profile of multidrug resistance to antimicrobials in strains of Acinetobacter baumannii was identified, highlighting the importance of knowledge about the profile of sensitivity to antimicrobials for a more assertive treatment and better prognosis.
RESUMO: Objetivos: Determinar o fenótipo de susceptibilidade frente aos antimicrobianos de bactérias isoladas antes e depois da desinfecção de leitos em dois hospitais no Sudeste do Pará. Métodos: Estudo do tipo descritivo, realizado em dois hospitais no Sudeste do Estado do Pará, um de baixa e média complexidade- hospital A (HA) e outro média e alta complexidade- hospital B (HB). A identificação quanto ao gênero e espécie seguiram as recomendações da ANVISA e para determinação do perfil de susceptibilidade frente aos antimicrobianos as recomendações do BrCAST. Resultados: Das superfícies avaliadas 62,5% (50/80) apresentaram positividade. Dessas verificou-se maior frequência no hospital A 67,5% (27/40). Quanto ao perfil de susceptibilidade das bactérias gram-negativas, observou-se que os isolados nos leitos do hospital A e B apresentaram elevada resistência às cefalosporinas e penicilinas, podendo chegar a 100%. Observou-se também que 54,0% (27/50) das cepas isoladas nos dois hospitais multirresistentes. Considerações finais: A resistência bacteriana é um problema emergente de saúde pública que exige esforço para a sua mitigação e foi detectada de forma significativa nos dois hospitais. Portanto, é imprescindível a triagem de leitos para contaminação por cepas MDRs, além da adoção das boas práticas para controle, como a compilação de higiene ambiental, das mãos e culturas de vigilância.
