CONTEXT: Although the benefits of antenatalcorticosteroids have been widely demonstrated in other countries, there are few studies among Brazilian newborn infants. OBJECTIVE:To evaluate the effectiveness of antenatal corticosteroids on the incidence of respiratory distress syndrome and intra-hospital mortality among neonates with a gestational age of less than 34 weeks. TYPE OF STUDY:Cross-sectional. SETTING:A tertiary-care hospital. PARTICIPANTS:Neonates exposed to any dose of antenatal corticosteroids for fetal maturation up to 7 days before delivery, and newborns paired by sex, birth weight, gestational age and time of birth that were not exposed to antenatal corticosteroids. The sample obtained consisted of 205 exposed newborns, 205 non-exposed and 39 newborns exposed to antenatal corticosteroids for whom it was not possible to find an unexposed pair. PROCEDURES:Analysis of maternal and newborn records. MAIN MEASUREMENTS:The primary clinical outcomes for the two groups were compared: the incidence of respiratory distress syndrome and intra-hospital mortality; as well as secondary outcomes related to neonatal morbidity. RESULTS:Antenatal corticosteroids reduced the occurrence of respiratory distress syndrome (OR: 0.33; 95% CI: 0.21-0.51) and the protective effect persisted when adjusted for weight, gestational age and the presence of asphyxia (adjusted OR: 0.27; 95% CI: 0.17-0.43). The protective effect could also be detected through the reduction in the need for and number of doses of exogenous surfactant utilized and the number of days of mechanical ventilation needed for the newborns exposed to antenatal corticosteroids. Their use also reduced the occurrence of intra-hospital deaths (OR: 0.51: 95% CI: 0.38-0.82). However, when adjusted for weight, gestational age, presence of prenatal asphyxia, respiratory distress syndrome, necrotizing enterocolitis and use of mechanical ventilation, the antenatal corticosteroids did not maintain the protective effect in relation to death. With regard to other outcomes, antenatal corticosteroids reduced the incidence of intraventricular hemorrhage grades III and IV (OR: 0.28; 95% CI: 0.10-0.77). CONCLUSIONS:Antenatal corticosteroids were effective in the reduction of morbidity and mortality among premature newborns in the population studied, and therefore their use should be stimulated within our environment.
Objetivo: avaliar se ciclos múltiplos de corticosteróide antenatal (CEA) são mais efetivos, em relação ao ciclo único, na redução da morbimortalidade de prematuros. Métodos: estudo retrospectivo envolvendo 184 neonatos com idade gestacional inferior a 34 semanas, nascidos em hospital de nível terciário de São Paulo no período de janeiro de 1988 a dezembro de 1998. Os pacientes foram divididos conforme o tratamento em: ciclo único (n=135): se a gestante recebeu um ciclo completo do CEA (2 doses de betametasona ou 4 doses de dexametasona, entre 24 horas e 7 dias do parto) e ciclos múltiplos (n=49): se a gestante foi tratada com dois ou mais ciclos completos. Foram comparados entre os dois grupos do estudo: síndrome do desconforto respiratório (SDR), mortalidade intra-hospitalar e morbidade neonatal combinada (ocorrência de um ou mais: SDR, hemorragia periintraventricular, displasia broncopulmonar, enterocolite necrosante, sepse ou óbito intrahospitalar). Os dados maternos e neonatais foram comparados pelo teste t de Student ou Mann-Whitney para as variáveis numéricas e pelo χ 2 ou exato de Fisher, com cálculo das "odds ratio" e seu intervalo de confiança, para as variáveis categóricas. Resultados: não foi demonstrada diferença significante na evolução dos neonatos do grupo ciclo único em relação ao múltiplo quanto à ocorrência de SDR (ciclo único: 22% e ciclo múltiplo: 18%), mortalidade intra-hospitalar (único 18% e múltiplo 12%) e morbidade neonatal combinada (único 62% e múltiplo 63%). Conclusões: o uso de ciclos múltiplos de CEA não diminuiu a morbimortalidade de prematuros. Esse estudo reforça a recomendação atual da utilização de ciclo único de corticosteróide para a maturação fetal em gestantes em trabalho de parto prematuro.

Consideremos, brevemente, algo relacionado à Saúde e Proteção à Maternidade.É inquestionável o Direito de cada cidadão, e de cada mulher grávida, a níveis aceitáveis de saúde, física e mental. Como ambicionar este Direito, universal, com tantas barreiras econômicas, sociais e culturais?No nosso entender o cerne do problema, de modo especial no que se refere à maternidade em adolescentes, reside no abandono educacional e cultural destas jovens mães. O ensino, a educação fundamental, obrigatórios, rigorosamente controlados, anulariam ou reduziriam a cifras irrisórias o percentual de adolescentes grávidas, com pais desconhecidos, perambulando por nossas megalópoles.Qual o porvir destas gestações seja para a mãe seja para a prole? A maternidade prematura, indesejada e acidental, tolherá as oportunidades de crescimento na atividade laborativa ou no crescimento intelectual destas jovens. Por outro lado, a prole será precocemente abandonada elevando o contingente de crianças e adolescentes relegados em nossas grandes cidades, candidatos férteis para a criminalidade.É tema para profunda, séria e premente reflexão. A saúde física e mental das mães, e da prole, está indissociada da educação básica com os conhecimentos fundamentais de higiene pessoal e direcionamento da sexualidade para o nobre fim da reprodução, na época oportuna, com todas as garantias de Maternidade Segura e Planejada.
Objectives: Rotavirus (RV) is the most frequent cause of severe gastroenteritis frequently requiring hospitalization. RV is responsible for > 1/2 of all hospital stays for acute gastroenteritis. The objective was to estimate the burden of community acquired rotavirus gastro-enteritis requiring hospitalization (CRVGE) in children ≤ 5 years old in Czech Republic (CR) and Slovakia (SK). MethOds: Multi-center, retrospective patient chart review was conducted in both pediatric and infection disease settings in CR (n= 109) and SK (n= 115). Resource use analysis including length of hospital stay and tests performed were evaluated. Patients requiring rehydration, complications and comorbidities were considered. Direct cost from payer's perspective were retrieved from official DRG lists (CR) and fixed hospitalization cost rates per case (SK). Micro-costing was done in parallel based on the resource use data. Results: Mean length of hospital stay in CR and SK was 3.9 (SD 1.9) and 4.1 days (SD 1.7) respectively. Prevalent diagnostic tests used were latex agglutination 44.0% (CR) and immunochromatography 92% (SK). Rehydration was required in 84.4% (CR) and 97% (SK) of cases. Comorbidities were reported in 24.8% (CR) and 27% (SK); complications in 10.1% (CR) and 7.8% (SK). The national list-based reimbursement per hospitalized CRVGE is € 370-645 (CR) and € 561 (SK). The calculated average total costs, including treatment prior to, and after admission, were € 462 (CR) and € 583 (SK). The major cost item was the hospital stay with € 391 (CR) and € 540 (SK). Costs for tests and drugs during hospitalization were € 30 (CR) and € 25 (SK). The costs of pre and post-hospitalization care were € 20 (CR) and € 13 (SK). cOnclusiOns: Although the length of hospitalization in both countries is similar costs seem to be substantially lower in CR, possibly as a result of recently launched DRG system. Common complications and comorbidities account for 30% of average hospital costs.
