Purpose: To compare the effectiveness of limbal relaxing incisions (LRI) with that of toric intraocular lens (IOL) implantation for the treatment of astigmatism during phacoemulsification using nonvectorial (predictability, safety, and efficacy) and vectorial analyses. Methods: This longitudinal observational case series assessed 62 eyes of 31 consecutive cataract patients with preoperative corneal astigmatism of 0.75-2.50 diopters (D) in both eyes. Patients were randomly assigned to 2 groups: a toric IOL group, which received toric IOLs in both eyes, and an LRI group, which received spherical IOLs associated with LRI placement in both eyes. All patients were evaluated at 1, 3, and 6 months after surgery, when refractive astigmatism analysis was performed using both nonvectorial and Alpins vectorial methods. Outcomes were assessed within each group and compared between groups. Results: The proportion of eyes within ±0.50 D of the intended correction at 3 and 6 months after surgery was 75% and 71.88%, respectively, in the LRI group and 40% and 66.67%, respectively, in the toric IOL group. In the remaining period, the proportion was greater in the toric IOL group. The safety index showed no difference at any time point. The efficacy index at 1 and 3 months after surgery was significantly higher in the toric IOL group (0.43 and 0.44, respectively) than in the LRI group (0.31 and 0.36, respectively). At 6 months after surgery, the status of eyes in the LRI group was as follows: 53.13% were undercorrected, 43.74% achieved the intended correction, and 3.13% were overcorrected. In the toric IOL group, 16.76% eyes were undercorrected, 76.67% achieved the intended correction, and 6.67% were overcorrected. The success rates for astigmatic surgery, astigmatism reduction at the intended axis, and astigmatism corrected were 43%, 62%, and 64%, respectively, in the LRI group and 57%, 81%, and 94%, respectively, in the toric IOL group. Conclusions: Our results suggest that the use of toric IOLs may be advantageous over the use of LRIs for the treatment of astigmatism during phacoemulsification. Although such advantages seem subtle in nonvectorial analyses, they are highlighted in vectorial analyses.

Artigo Original | Original article INTRODUÇÃOA ceratoplastia lamelar anterior profunda (DALK -deep anterior lamelar keratoplasty) tem sido proposta como alternativa à tradicional ceratoplastia penetrante (CP) para tratamento de várias doenças corneanas que não acometem o endotélio, como é o caso do ceratocone. O procedimento propõe a retirada das camadas corneanas até o mais próximo da membrana de Descemet, sem penetrar no globo ocular, seguido do transplante corneano. Os benefícios teóricos da RESUMO Objetivos: Avaliar através de um estudo retrospectivo pacientes com ceratocone, a partir do 12º mês após a ceratectomia lamelar anterior profunda usando a técnica "big-bubble". Métodos: Estudo retrospectivo de consecutivos 79 olhos de 71 pacientes com ceratocone moderado a grave, com baixa acuidade visual após as correções ópticas e intolerantes à adaptação de lente de contato. Todos os transplantes foram realizados pelo mesmo experiente cirurgião, no período de janeiro de 2007 a fevereiro de 2009, utilizando a técnica de ceratectomia lamelar anterior profunda pelo "big-bubble". Os dados analisados foram sexo, idade, paquimetria corneana, densidade endotelial, astigmatismo dinâmico e melhor acuidade visual corrigida (BCVA) após a ceratectomia. Resultados: A idade média verificada foi de 30,4 (± 10,2) anos, com 39 (54,9%) do sexo feminino e 32 (45,1%) do masculino. A paquimetria ultrassônica média foi de 518,6 (± 29,5) mm. Setenta e dois (91,1%) dos transplantes realizados obtiveram acuidade visual dinâmica corrigida melhor ou igual a 20/50. A média do astigmatismo dinâmico foi de -3,5 (± 1,5) dioptrias cilíndricas. A média da densidade endotelial encontrada conforme a faixa etária teve a distribuição com os pacientes de 11 a 20 anos com 3.053 (± 532) cél/mm 2 ; 21 a 30 anos com 2.674 (± 725) cél/mm 2 ; 31 a 40 anos com 2.737 (± 575) cél/mm 2 ; 41 a 50 anos com 2.585 (± 476) cél/mm 2 e 51 a 60 anos com 2.398 (± 667) cél/mm 2 . Microperfuração no per-operatório teve uma incidência de 8,8%. Conclusão: A ceratectomia lamelar anterior profunda pela técnica "big-bubble" é um valioso tratamento para pacientes com ceratocone, alcançando uma melhor acuidade visual corrigida final semelhante ao transplante penetrante. A contagem média de células endoteliais apresenta-se dentro da normalidade para a faixa etária após o procedimento. Não foram encontradas córneas rejeitadas ou com edema após a retirada de todos os pontos. O astigmatismo dinâmico na ceratectomia lamelar anterior profunda é similar aos encontrados nos relatos de ceratoplastias penetrantes. A ceratectomia lamelar anterior profunda pela técnica do "big-bubble" deve ser estudada como primeira opção para o tratamento do ceratocone. DALK incluem a manutenção do endotélio receptor, eliminação da rejeição endotelial, retirada de pontos mais precoce, além de reduzir complicações importantes da CP como descolamento de coroide, glaucoma agudo pós-operatório precoce, sinéquias, catarata e extrusão de conteúdos intraoculares (1) .
