Psammomatoid juvenile ossifying fibroma (PJOF) is a rare benign tumor that usually affects the paranasal sinuses, orbit, and skull. In most cases, extensive incisions are necessary for full access to the tumor site. The aim of this paper is to report a case of extensive PJOF in which an intraoral surgical approach was performed with complete excision of the tumor. A female patient, 18-year old had a deforming volume increase in the region of the left facial middle third with an approximate evolution time of 2 years. She complained of headache, epiphora in the left eye, and total obstruction of the left nostril. Extraoral examination showed facial asymmetry with dystopia, ocular proptosis, and considerable sclera exposition of the left eye. Tomography examination showed a mixed aspect lesion on the left side of the face, well delimited. The intraoral surgical approach was chosen for the excision of the lesion. Under general anesthesia and nasotracheal intubation, total resection was performed, followed by exodontia of the directly involved teeth. In the definitive histopathological examination, the diagnosis was confirmed. The patient is currently with 2 years and 6 months of postoperative follow-up and has good healing of intraoral surgical wounds and stable occlusion. Extraoral examination showed harmonic projection of the facial middle third, but still with excessive exposure of the sclera of the left eye due to the defect in the orbital floor. The surgical treatment of PJOF was possible by intraoral approach, even taking into account the limitations of the access and the complexity of the anatomy of the facial bones involved.

Osteomas são neoplasias benignas que se caracterizam pela proliferação de osso maduro compacto ou esponjoso. Os locais de ocorrência mais frequentes são a mandíbula e os seios paranasais. O objetivo desse artigo é relatar diagnóstico e o tratamento cirúrgico de um osteoma periférico mandibular em paciente não sindrômico. O caso em estudo é o de um Paciente do gênero masculino, 40 anos, ASA I, procurou o serviço de Cirurgia Bucomaxilofacial do Hospital Universitário Oswaldo Cruz – PE com queixa de aumento de volume intraoral em região anterior de mandíbula, firme a palpação, bem delimitada e com cerca de 2cm. Após exame físico a hipótese diagnóstica levantada foi osteoma e torus mandibular. A proposta cirúrgica planejada foi acesso intra-oral, exérese da lesão, osteoplastia mandibular e encaminhamento do material colhido para análise histopatológica para confirmação do diagnóstico. A análise histopatológica confirmou a hipótese diagnóstica de osteoma périférico. O paciente foi acompanhado no pós-operatório e após 6 meses do procedimento cirúrgico não foi observado sinais de recidiva.

Introdução: A sialolitíase é a desordem mais comum das glândulas salivares acometendo principalmente a glândula submandibular e o seu ducto. Caracteriza-se pela formação de cálculos ou sialólitos no interior do ducto ou do próprio parênquima glandular. A etiologia pode estar associada ao pH alcalino e aumento da concentração de cálcio na saliva, além de fatores anatómicos como estenose ou diverticulação do ducto salivar. Objetivo: relatar um caso clínico de sialólito localizado no ducto da glândula submandibular, bem como discutir a literatura pertinente sobre o tema. Métodos: Paciente do gênero masculino, compareceu ao Hospital Geral de Areias com queixa de aumento em região de soalho bucal direito. Foram solicitadas radiografias panorâmica e oclusal e observou-se massa radiopaca de formato cilíndrico e alongado na região submandibular. A excisão cirúrgica foi realizada sob anestesia local. Paciente não apresentou complicações e retomou-se o reestabelecimento da patência do ducto e normalidade do fluxo salivar. Conclusões: A conduta mostrou-se eficaz no manejo da lesão através de remoção cirúrgica por acesso intrabucal. Os defeitos mandibulares devido à ressecção de lesão óssea interferem a harmonia e estética facial comprometendo a qualidade de vida dos pacientes. Grandes defeitos exigem planejamento minucioso, principalmente quando lançamos mão de enxertos e placas de reconstrução, evitando assim resultados insatisfatórios ou até mesmo sequelas. Apesar de algumas classificações dos defeitos mandibulares vêm sendo discutidas ao longo dos anos, ainda não há um protocolo definido para reconstrução mandibular. O uso de prototipagem na Cirurgia Bucomaxilofacial tem se tornado cada vez mais frequente; a precisão da reconstrução, diminuição do tempo de cirurgia reflete em recuperação mais rápida do paciente. Hemimandibular hyperplasia was first described in 1836 by Adams as a disorder that causes condylar hyperplasia, deforming facial asymmetry and has an unknown etiology. The objective of this study was to report a patient with surgical correction through orthognathic surgery and high condilectomy for the treatment of hemimandibular hyperplasia. The patient complained of pain and cracking in the right temporomandibular joint region and was dissatisfied with the aesthetic appearance of the face. Virtual planning was done for bimaxillary orthognathic surgery and preparation of prototyped surgical guides. Stereolithographic models were used for the preparation of the acrylic guides of the osteotomies for the high condilectomy and the contour of the mandibular base. Orthognathic surgery resulted in the maxillary repositioning with correction of the inclination of the occlusal plane, reduction of the height of the ramus and right mandibular body and class I dental attachment. Mandibular contour osteotomy was performed with acrylic guide in the basilar. The condilectomy was performed by endaural access. In 2-year follow-up, there are no signs of recurrence.

Hemangiomas são lesões benignas de origem vascular comuns na região da cabeça e pescoço, mas dificilmente surgem em cavidade bucal. O manejo e o conhecimento das técnicas diagnósticas são fundamentais para o diagnóstico diferencial. O oleato de monoetanolamina, como agente esclerosante, promove uma reação inflamatória levando a fibrose endotelial, sendo eficaz no tratamento de lesões vasculares benignas da cavidade bucal; além das reações adversas serem de baixa intensidade e pouco relatadas na literatura; mostrando-se uma alternativa ao tratamento cirúrgico. Este trabalho teve por proposito relatar uma série de casos de pacientes submetidos à escleroterapia e discutir os resultados do tratamento de lesões vasculares bucais benignas com oleato de monoetanolamina. Quatro pacientes compareceram no serviço de Cirurgia e Traumatologia Buco-Maxilo-Facial do Hospital Universitário Oswaldo Cruz (CTBMF/HUOC/UPE) – Recife/PE com queixa de lesão oral. Após exame clínico detalhado e a utilização da vitropressão foram obtidos os diagnósticos de hemangiomas optando-se pelo tratamento à base de escleroterapia com oleato de monoetanolamina apresentando satisfatória resolução clínica. Nos casos aqui relatados, o agente esclerosante mostrou-se eficaz para o tratamento de hemangiomas em cavidade bucal.

Objective: To compare the agreement of clinical and radiographic diagnosis with the histopathological diagnosis in fibro-osseous lesions of the jaws. Material and Methods: An analytical and exploratory study was made based on systematic collected data, carried out in the laboratory of surgical pathology of a public Dental School. There were evaluated cases of fibrous dysplasia (FD), cemento-osseous dysplasia (COD) and ossifyng fibroma (OF), diagnosed by clinical, radiographic (panoramic and periapical radiography), and histopathological analysis, in a period of 12 years (from March 2001 to June 2013). Descriptive and inferential statistics (Fisher's exact test) were obtained. Results: Ninety-six cases of FOLs were evaluated. The radiographic aspects of the FOLs studied did not differ significantly (p=0.09). Radiolucent lesions were the least frequent, corresponding to approximately 13.5% of radiographic findings. Mixed lesions and radiopaques were more present, how they were COD and FD, respectively. The more aggressive variation of OF (Juvenile Ossifying Fibroma -JOF) was less frequent among the pathologies evaluated. In approximately 61.46% of the cases clinical and radiographic diagnosis were confirmed by histopathological diagnosis of FOLs. The highest agreement and the highest disagreement were observed in COD cases (40.7% and 62.2%, respectively). Conclusion: FOLs of the maxillaries represent a group of lesions in which the establishment of the clinical and radiographic diagnosis supported by the histopathological confirmation is critical and challenging.
