Patients on haemodialysis are becoming increasingly older and this means there is an increase in the number of vascular access complications. Infections of tunnelled catheters cause a high morbidity/mortality.
Research in nephrology seeks to fi nd increasingly effi cient extrarenal purifi cation techniques. The dialysis technique of potassium-profi led acetatefree biofi ltration offers better tolerance and reduces the risk of arrhythmias. Our aim was to compare this technique with high-fl ow haemodialysis and with haemodiafi ltration in order to ascertain which dialysis treatment is the most suitable for each patient.
IntroducciónEn España, en la década de los 60, eran muy pocos los hospitales que contaban con la terapia de diálisis, por la falta de espacio, recursos humanos y económicos. Esta escasez fue la que hizo que se desarrollasen programas de Hemodiálisis Domiciliaria (HDD). Hacia mediados de los años 70 se empezaron a crear más unidades de hemodiálisis y se desarrolla la diálisis peritoneal por lo que desapareció casi totalmente la HDD.
den en el momento actual 322 pacientes. De esta población, 23 pacientes se encuentran incluidos en programa de diálisis peritoneal, y 54 pertenecen al programa de hemodiálisis hospitalaria. Los demás de pacientes se hallan incluidos en el programa de hemodiálisis de los 5 centros concertados existentes en la provincia.Nos planteamos el conocer las características de las de urgencias de los pacientes en diálisis atendidos en nuestro hospital. Entendiendo por urgencias los actos fuera del trabajo programado y fuera de nuestro horario laboral, así como las causas, el tiempo en que se producen y la supervivencia de estos pacientes. En la unidad de diálisis de nuestro hospital, el horario laboral programado es de ocho a veintidós horas. Desde las veintidós a las ocho y las veinticuatro horas del domingo, permanecen localizados un nefrólogo y una enfermera/o.
MATERIAL Y MÉTODOSSe han recogido todas las urgencias, que han precisado actuaciones del personal de diálisis, fuera del horario laboral programado, durante los años 1995, 1996, 1997, 1998 y 1999. Se han recogido las urgencias según los trimestres del año, así como la franja horaria (8-22; 22-3; 3-8) en que se producen. Para la recogida de datos se han consultado las Memorias de Gestión del período 1995-1999 y el Libro de Urgencias de la Unidad de diálisis del mismo período.Las causas de las urgencias se han catalogado como: edema agudo de pulmón, hiperpotasemia, intercambios y otros (intoxicaciones, acidosis, etc). Se han considerado tres tipos de procedimientos: hemodiálisis, ultrafiltración e intercambio peritoneal.Se analiza la media y los porcentajes. La supervivencia de los pacientes se analizó mediante el método de Kaplan y Meiers.
RESULTADOSSe han realizado un total de ochenta actuaciones en los cinco años revisados. De ellos destaca el año 1996 con 22 actuaciones.En todos los años, excepto en el 1997, la principal causa de las urgencias fue el edema agudo de pulmón, siendo en el 1997 la hiperpotasemia (figura 1).No hay diferencias en el número de actuaciones, cuando se analizan por trimestre. No obstante destacan las franjas horarias de 22-3 y de 8-22 (domingo), con 32 (40%) actuaciones cada franja.Urgencias en diálisis en toda una provincia. Resultados preliminares
