La pancreatitis aguda es una condición inflamatoria con una mortalidad y morbilidad significativa en la actualidad. Las dos causas más comunes a nivel mundial son por litos biliares y por abuso de alcohol. Cuenta con tres criterios diagnósticos, de los cuales con la presencia de 2 de los 3 ya se puede realizar el diagnóstico. Los dos criterios más importantes son la presentación clínica del dolor abdominal localizado en epigastrio de inicio súbito con irradiación hacia espalda y la elevación de 3 veces el valor límite superior de la amilasa y lipasa. Los estudios de imágenes usualmente son utilizados para buscar complicaciones de la enfermedad. La clave del manejo es la fluidoterapia temprana, manejo del dolor y la nutrición de los pacientes. Estos pasos garantizan la resolución de la mayoría de los cuadros sin llegar a generar complicaciones locales y sistémicas.
La colestasis intrahepática del embarazo es la patología más frecuente de las enfermedades hepáticas ocurridas durante la gestación y es caracterizada por la aparición de prurito y elevación de los niveles de ácidos biliares, principalmente en el segundo o tercer trimestre del embarazo. La incidencia de esta enfermedad a tratar varía según la localización geográfica y población, así como la genética y factores ambientales asociados. En condiciones normales, los ácidos biliares se transportan del feto a la madre, mientras que en un embarazo complicado por colestasis intrahepática, el transporte transplacentario ocurre en la dirección opuesta. Por tanto, los niveles de ácidos biliares aumentan tanto en la madre como en el feto, y esta elevación induce a estrés oxidativo, apoptosis y un riesgo elevado de complicaciones para el feto tales como, parto pretérmino, presencia de líquido amniótico meconial, hipoxia fetal y muerte fetal intrauterina. Con base en recomendaciones y guías clínicas, se valorará el óptimo y adecuado manejo de la presente patología a desarrollar.
La meralgia parestésica o neuralgia del nervio femorocutáneo es una enfermedad que se caracteriza por parestesias, adormecimiento, debilidad y dolor tipo quemante en la parte externa del muslo. La principal causa es la compresión del nervio cutáneo femoral lateral el cual es un nervio sensitivo puro, que se forma por la unión de las raíces nerviosa de L2-L3 yes el principal encargado de la sensibilidad de la región anterior y la mitad superior de la cara lateral del muslo. Es una enfermedad que afecta predominantemente a pacientes entre los 30 y 40 años con mayor incidencia en el género masculino.En la mayoría de los casos, las causas más frecuentes se deben a obesidad, embarazo, ropa ajustada, así como también, a un traumatismo local o a una enfermedad como la diabetes mellitus. En la mayoría de los casos, el manejo consiste en medidas moderadas, sin embargo, en casos graves el tratamiento puede constar de medicamentos analgésicos y en raras ocasiones, tratamiento quirúrgico.
