La colestasis intrahepática del embarazo es la patología más frecuente de las enfermedades hepáticas ocurridas durante la gestación y es caracterizada por la aparición de prurito y elevación de los niveles de ácidos biliares, principalmente en el segundo o tercer trimestre del embarazo. La incidencia de esta enfermedad a tratar varía según la localización geográfica y población, así como la genética y factores ambientales asociados. En condiciones normales, los ácidos biliares se transportan del feto a la madre, mientras que en un embarazo complicado por colestasis intrahepática, el transporte transplacentario ocurre en la dirección opuesta. Por tanto, los niveles de ácidos biliares aumentan tanto en la madre como en el feto, y esta elevación induce a estrés oxidativo, apoptosis y un riesgo elevado de complicaciones para el feto tales como, parto pretérmino, presencia de líquido amniótico meconial, hipoxia fetal y muerte fetal intrauterina. Con base en recomendaciones y guías clínicas, se valorará el óptimo y adecuado manejo de la presente patología a desarrollar.
La meralgia parestésica o neuralgia del nervio femorocutáneo es una enfermedad que se caracteriza por parestesias, adormecimiento, debilidad y dolor tipo quemante en la parte externa del muslo. La principal causa es la compresión del nervio cutáneo femoral lateral el cual es un nervio sensitivo puro, que se forma por la unión de las raíces nerviosa de L2-L3 yes el principal encargado de la sensibilidad de la región anterior y la mitad superior de la cara lateral del muslo. Es una enfermedad que afecta predominantemente a pacientes entre los 30 y 40 años con mayor incidencia en el género masculino.En la mayoría de los casos, las causas más frecuentes se deben a obesidad, embarazo, ropa ajustada, así como también, a un traumatismo local o a una enfermedad como la diabetes mellitus. En la mayoría de los casos, el manejo consiste en medidas moderadas, sin embargo, en casos graves el tratamiento puede constar de medicamentos analgésicos y en raras ocasiones, tratamiento quirúrgico.
La lumbalgia es un problema de salud pública que implica grandes consecuencias para el sistema de salud y para quienes sufren algún episodio de esta patología, ya que en ocasiones se torna incapacitante para quien lo sufre. Es importante conocer la clínica de las diversas presentaciones de la lumbalgia y reconocer los factores de riesgo o banderas rojas, para brindarle el mejor abordaje terapéutico. De este modo, se deben establecer estudios de imágenes para determinar con mayor claridad la sintomatología y buscar el mejor escenario para el paciente. Se debe tomar en cuenta que el reposo absoluto por más de 48 horas puede ser perjudicial en la lumbalgia, y que se debe incluir como parte fundamental en su tratamiento las actividades diarias que no requieran esfuerzo físico; asimismo, la primera línea de tratamiento es el paracetamol, debido a los mínimos efectos adversos que ocasiona, comparado con los antiinflamatorios no esteroideos. La combinación de analgésicos con complejos de vitamina B aumenta la eficacia del tratamiento y brindan un mayor alivio de la sintomatología. Los opioides de baja intensidad pueden ser de utilidad en los casos refractarios a AINES, y los esteroides son de beneficio en las lumbalgias con componentes inflamatorios.
La neumatosis intestinal es definida como la presencia de aire dentro de la pared intestinal. Este es un signo radiológico en vez de un cuadro clínico. La neumatosis tiene un largo historial médico, su detención ha sido favorecida ampliamente por el uso de la tomografía computarizada a lo largo de las últimas décadas. Se le halla con mayor frecuencia en el intestino delgado, y raramente es de localización extra intestinal. Se le estima una prevalencia de 0.03%, y es más comúnmente visto entre la edad de los 40 y 50 años. La neumatosis intestinal puede ser clasificada como primaria y secundaria. La forma primaria consiste en una infiltración de gas idiopática en la pared intestinal que asemeja un racimo de uvas, mientras que la forma secundaria es más frecuente y suele estar asociada a un amplio rango de condiciones patológicas como la enfermedad intestinal inflamatoria, malignidades, quimioterapia, estados de inmunodeficiencia o iatrogenia médica. Pero principalmente es un signo que se ha relacionado a isquemia y necrosis intestinal, y que ha sido considerada una emergencia médica, especialmente sí se relaciona con portograma aéreo. En la forma secundaria de neumatosis intestinal, el patrón radiológico es más variable. Esta es mejor detectada y evaluada con la tomografía computarizada, la cual es la prueba de elección para detectar neumatosis intestinal y debe ser utilizada en adultos en cualquier caso en el cual la radiografía convencional o ecografía aumenten su sospecha diagnóstica. El objetivo principal de este artículo es determinar la utilidad de la neumatosis intestinal como signo radiológico, con particular atención a su fisiopatología, clasificación y diagnóstico radiológico.
La aparición de la pandemia por el virus SARS-CoV-2 ha causado una afectación en múltiples sectores, en especial en el sistema de salud. Las generalidades desde su surgimiento, fisiopatología y cuadro clínico, así como los hallazgos en los métodos de imagen que orientan hacia ciertos patrones característicos de la enfermedad, deben difundirse por todo el personal de salud involucrado en el diagnóstico y el tratamiento de pacientes. Aunque el diagnóstico es microbiológico, las técnicas de imagen tienen un papel importante para apoyar la gravedad de la enfermedad debido a que la afectación es principalmente pulmonar, por lo cual el primer método que suele emplearse es la radiografía de tórax, permitiendo una identificación de los pacientes en quienes se evidencia un empeoramiento del estado respiratorio. El objetivo del siguiente artículo es describir los hallazgos principales de la enfermedad y clasificar su gravedad en la radiografía de tórax por SARS-CoV-2, y hacer una breve mención de la fisiopatología y cuadro clínico.
