Defek septum ventrikel (VSD) adalah kelainan jantung kongenital yang ditandai dengan adanya hubungan abnormal antara ventrikel kiri dan kanan jantung, sehingga menimbulkan gangguan hemodinamik. VSD merupakan kelainan jantung bawaan yang paling sering ditemui pada anak-anak, serta merupakan kelainan kedua paling sering ditemui pada orang dewasa setelah katup aorta bikuspid. Sebagian besar VSD menutup spontan, namun VSD yang gagal menutup dapat menimbulkan komplikasi seperti hipertensi arteri pulmoner, disfungsi ventrikel, dan risiko aritmia. Diagnosis VSD komprehensif melalui anamnesis, pemeriksaan fisik, dan pemeriksaan penunjang untuk menentukan tata laksana dan prognosisnya. Ventricular septal defect is a congenital heart disorder characterized by an abnormal connection between the left and the right ventricle of the heart, causing hemodynamic disturbances. VSD is the most common congenital heart defect in children, and is the second most common abnormality in adults after the bicuspid aortic valve. Most VSDs close spontaneously, but failure to close can lead to complications such as pulmonary arterial hypertension, ventricular dysfunction, and the risk of arrhythmias. Diagnosis needs to be comprehensive through history, physical examination, and other supporting examinations for treatment planning and prognosis.
Kanker hati atau karsinoma hepatoseluler menempati urutan ke-5 kanker terbanyak di Indonesia. Data Riskesdas 2018 menunjukkan prevalensi perokok di Indonesia sebesar 33,8% dan merokok banyak dikaitkan dengan berbagai komplikasi dan peningkatan angka kematian berbagai penyakit. Tinjauan ini merupakan laporan kasus berbasis bukti untuk melihat mortalitas pasien karsinoma hepatoseluler yang memiliki riwayat perokok. Pencarian artikel dari 4 database, yakni Pubmed, Cochrane, ScienceDirect, dan Scopus dengan kata kunci “((Hepatocellular carcinoma) AND (Smoking) AND (Mortality))” dilakukan dengan kriteria inklusi dan eksklusi; telaah kritis berdasarkan panduan untuk studi prognosis. Didapatkan 1 studi prognosis mortalitas pasien karsinoma hepatoseluler dengan riwayat merokok yang telah menjalani telaah kritis dan dapat dinyatakan sebagai studi yang valid, penting, dan aplikatif. Studi tersebut merupakan systematic review dan meta-analisis dengan OR 1,20 (95% CI 1,03-1,40). Riwayat merokok dengan frekuensi minimal 20 batang/hari selama 1 tahun akan meningkatkan risiko mortalitas pasien karsinoma hepatoseluler sebesar 1,2 kali dibandingkan pasien karsinoma hepatoseluler tanpa riwayat merokok. This review was an evidence-based case report on the mortality of hepatocellular carcinoma patients with smoking history. An article search was conducted based on 4 databases: Pubmed, Cochrane, ScienceDirect, and Scopus using the keywords “((Hepatocellular carcinoma) AND (Smoking) AND (Mortality))” according to inclusion and exclusion criteria and based on guidelines for prognosis studies. History of smoking with a frequency of at least 20 cigarettes/ day for 1 year will increase the risk of mortality by 1.2 times in hepatocellular carcinoma patients compared to hepatocellular carcinoma patients without a history of smoking.
Osteomielitis merupakan peradangan tulang dan sumsum tulang yang biasanya disebabkan oleh infeksi bakteri. Osteomielitis dapat menimbulkan gangguan fungsional irreversibel dan disabilitas. Namun, hingga saat ini, belum ada guideline khusus diagnosis dan manajemen tata laksana osteomielitis. Diagnosis dan tata laksana osteomielitis harus komprehensif agar memberikan hasil baik. Osteomyelitis is an inflammation of bone and bone marrow which is usually caused by bacterial infection. Osteomyelitis can cause irreversible functional problems and disabilities. There is still no specific guidelines for diagnosis and management of osteomyelitis. Diagnosis and management must be conducted comprehensively to result in good recovery.
Masih meningkatnya kasus positif dan kematian COVID-19 di Indonesia menuntut fasyankes agar bisa mempersingkat durasi hospitalisasi/ rawat inap untuk efisiensi sumber daya kesehatan dan peningkatan kualitas pelayanan. Terapi adjuvan pasien rawat inap antara lain terapi plasma konvalesen (TPK). Review ini bertujuan untuk mengetahui pengaruh pemberian TPK dan terapi standar dibandingkan terapi standar saja terhadap durasi rawat inap pasien COVID-19. Penelusuran artikel secara manual pada empat database, yaitu PubMed, Cochrane, ScienceDirect, dan Scopus. Kata kunci yang digunakan adalah convalescent plasma, COVID-19 therapy, COVID-19 serotherapy, length of stay. Metode seleksi berdasarkan kriteria eligibilitas mendapatkan 1 artikel yang kemudian ditelaah kritis dari segi kesahihan, kepentingan, dan kemamputerapan. Satu artikel tinjauan sistematis merupakan artikel yang sahih dan penting, tetapi kurang memiliki kemamputerapan karena demografi dan kondisi klinis yang tidak sesuai. Review ini menyimpulkan bahwa pemberian TPK tidak memengaruhi durasi rawat inap pasien COVID-19. Diperlukan studi lebih lanjut dengan demografi dan kondisi klinis yang sesuai untuk mendapat keyakinan tertinggi. Indonesia is still in an ongoing trend of positive cases and deaths caused by COVID-19. This condition urges health care facilities to reduce length of stay for more efficient health resources management and higher quality of care. Adjuvant therapy to hospitalized patients includes convalesent plasma therapy (CPT). This review aims to determine the effect of CPT on length of stay of COVID-19 patients in comparison to standard therapy. Literature search is through hand searching and through four databases: PubMed, Cochrane, ScienceDirect, and Scopus. Keywords used are: convalescent plasma, COVID-19 therapy, COVID-19 serotherapy, length of stay. Selection through eligibility criteria resulted in one systematic review article, then critically appraised by its’ validity, importance, and applicability.The article selected is valid and important, yet lack of applicability due to discrepancy in demographics and clinical conditions. The review concluded that CPT does not affect the length of stay of COVID-19 patients.More research with similiar demographics and clinical conditions are needed to achieve highest level of certainty.
Pendahuluan: Salah satu permasalahan kesehatan utama di Indonesia merupakan penyakit infeksi saluran pernapasan. Penyakit saluran pernapasan dapat diakibatkan oleh jamur, sehingga disebut mikosis paru. Jamur-jamur penyebab infeksi saluran pernapasan pada manusia terdiri atas banyak spesies mulai dari spesies-spesies Candida sp. hingga spesies jamur penyebab mikosis yang lebih patogen, yaitu Aspergillus fumigatus. Metode biakan spesimen respirasi berupa sputum dapat digunakan untuk mengidentifikasi spesies jamur penyebab mikosis paru. Tujuan penelitian ini dilakukan untuk mengetahui perbandingan hasil kultur yang dapat tumbuh pada media agar saboraud dekstrosa dengan metode biakan konvensional menggunakan agar miring dan metode biakan High Volume Culture pada cawan petri (plate).Metode: Penelitian ini menggunakan desain potong lintang di mana hasil kultur diidentifikasi dari sputum yang dibiakan pada metode yang berbeda.Hasil: Metode High Volume Culture dapat menumbuhkan lebih banyak koloni jamur termasuk Aspergillus fumigatus dibandingkan dengan metode konvensional yang tidak menumbuhkan Aspergillus fumigatus. Data hasil kultur pada dua metode berbeda tersebut kemudian dianalisis korelasinya dengan uji McNemar dan didapatkan nilai kemaknaan p=0.000.Kesimpulan: Hubungan antara metode biakan dengan hasil kultur mempunyai korelasi yang signifikan (p<0.05). Metode High Volume Culture merupakan metode yang lebih baik untuk menumbuhkan lebih banyak koloni jamur termasuk Aspergillus fumigatus dibandingkan dengan metode kultur konvensional.
