Splenic cysts are rarely detected, and clinically diagnosed commonly as spleenomegaly . Thenature of the cyst could not be diagnosed pre-operatively because of its’ anatomical location andonly be confirmed by the histopathological assessments. These cysts are found as a sequel ofsplenic infarct followed by the abscess formation. This is a very rare condition of a minor boy ofonly 10 years’ old who presented with a hugely enlarged spleen. The common investigation(s)did not prove that it was in the haematological conditions. Ultrasonogram (USG) investigationshows spleenomegaly due to the cystic lesion. CT scan of the abdomen showed large andwell defined cystic mass which was 13.4cmX7.9cmX15.3cm in size, multi-loculated, septated,sharply demarcated as such well delineated cyst. After necessary preparation like vaccination,blood grouping, cross matching and other fitness, laparotomy and total spleenectomy was doneas the cyst occupied most of the Splenic tissue. The post operative recovery was uneventful andthe final diagnosis was confirmed by histopatholgical examinations as epidermoid cyst of thespleen. Intussusception, invagination of a segment of the gut into an adjacent one, is a relatively common cause of the intestinal obstruction in children but a very rare and uncommon clinical entity in the adults accounting to 1% and near about 65% of these are caused by malignant lead point. The Lipomatous polyp accounts for 4% of all benign tumors of the gut. In most of the cases, these are seen particularly in large intestine, usually sub mucosal and around ileocecal valve. These are mostly and very often asymptomatic. Mrs. Rahima Begum aged 65 years, previously was a healthy Bangladeshi housewife, with the history of recurrent abdominal colic of moderate intensity for the last 6 months, associated with the appearance of mass in the right lower quadrant with features of sub-acute intestinal obstruction got admitted into the Ibn Sina Medical College Hospital, Kallyanpur, Dhaka, Bangladesh. The required and most relevant investigations revealed the Ileocolic intussusception and underwent the right hemicolectomy. Postoperative recovery of the patient was uneventful excepting the hypertension only. Histopathology investigations revealed lipomatous polyp arising from the Ileocecal junction. This study reports intussuception in the adult due to lipomatous polyp, a very rare case presenting with sub acute intestinal obstruction. Background: Pilonidal sinus is the relatively common condition affecting men almost twice as often as the women we experienced. The estimated incidence is 0.26 per thousand populations in general. The management of pilonidal sinus disease remains controversial, and gold standard treatment modality has yet to be established. Limberg procedure is a safe and reliable technique in the treatment of the sacrococcygeal pilonidal sinus disease, with a low complication and recurrence rates if performed according to the appropriate surgical principles. Methods: This is a Prospective study on 24 patients between the period from July 2014 to Dec 2018 in the Department of Surgery in Bangabandhu Sheikh Mujib Medical University (BSMMU) as well as other different hospitals in Dhaka, Bangladesh. The patients having primary or recurrent pilonidal sinus disease underwent the foresaid surgical treatment. Results: Twenty four patients underwent this operation. Among them, the picture was that 20(83.3%) were males and 4(16.7%) were female. The mean age was 31, (Range: 17-45 years). 6(25%) patients were presented with recurrent sinus and 5 of them had one or more occasion previous surgery. Twenty one patients (87.5%) had full primary healing without any complication. But 1(4.2%) patient had minimal sarcoma, 1(4.2%) patient had superficial infection and the remaining other 1(4.2%) had partial flap necrosis. However, all three healed completely with conservative treatment. The mean length of hospital stay was 2.45 (Range: 1-5) days and the most patients returned to their work within 3 weeks. Conclusion: Limberg flap is very effective for sacrococcygeal pilonidal sinus disease with low complication rates, shorter hospital stay, low recurrence rates, earlier healing and reduced off-work period. The surgery can be easily mastered. We recommend Limberg flap as preferred surgery in the cases of Sacrococcygeal Pilonidal sinus. Bangladesh Journal of Medical Science Vol.19(1) 2020 p.105-109
Objective: To study the clinical, radiological and histopathological characteristics along with the management outcome of salivary gland neoplasm. Materials and Methods: This study included thirty two patients (16 male and 16 female) with salivary gland tumours presented at Combined Military Hospital Dhaka, Apollo Hospital Dhaka and Ibn Sina Medical College and Hospital Dhaka from January 2006 to December 2009. The data of each patient included age, sex, presenting symptoms and signs, provisional diagnosis, preoperative investigations, operation notes, histopathological examination and state at follow up. Results: The mean age was 42.5 years. 14 (43.75%) patients presented at 5 th decade. Main presenting symptom was swelling (100%) followed by pain (18.75%). All patients underwent fine needle aspiration cytology which was conclusive in 26 patients (81.25%). Thirty one patients underwent surgical excision. 24 cases were parotid neoplasm and 8 cases were submandibular neoplasm. 20 patients (16 parotid and 4 submandibular) were pleomorphic adenoma. Conclusion: There is no substitute for detailed clinical history and examination in the assessment of salivary gland neoplasm. Imaging studies are probably academic but determine the extent of the disease. Early diagnosis, adequate and proper treatment improves the prognosis.
