O objetivo deste estudo foi avaliar a sobrecarga dos cuidadores de idosos com acidente vascular cerebral (AVC), assim como correlacioná-la com as horas de cuidado, a idade e a independência funcional dos idosos. Trata-se de estudo transversal feito com 62 idosos com AVC e seus cuidadores. O instrumento continha variáveis sociodemográficas e econômicas, Mini-Exame do Estado Mental, Medida da Independência Funcional (MIF) e a Escala de Zarit. A possível correlação entre os escores da escala de Zarit e as outras variáveis foi avaliada por meio do Coeficiente de Correlação de Pearson. A maioria dos cuidadores era adultos, filhos, casados e do sexo feminino. A média do escore de Zarit foi 34,92 (15,8). A MIF apresentou correlação negativa com a sobrecarga do cuidador, porém, não houve correlação com a idade e as horas de cuidado. A sobrecarga da maioria dos cuidadores variou de moderada a severa e parece estar relacionada ao nível de independência funcional dos idosos.
OBJECTIVE To determine the demographic and health factors related to the frailty syndrome in older adults.METHODS This is a longitudinal quantitative study carried out with 262 older adults aged 65 years and older, of both sexes, living at home. Data collection was carried out in Period 1 between October 2007 and February 2008, and in Period 2 between July and December 2013. For data collection, we used the sociodemographic profile instrument, the Edmonton Frail Scale, the Mini-Mental State Examination, the number of falls in the last 12 months, the number of self-reported diseases and used drugs, the Functional Independence Measure, and the Lawton and Brody Scale. We used descriptive statistics for data analysis, in the comparison of the means between periods, the nonparametric Wilcoxon test, and the method of Generalized Estimating Equations, which is considered an extension of the Generalized Linear Models with p ≤ 0.05.RESULTS Of the 515 participants, 262 completed the follow-up, with a predominance of females, older individuals, and those who had no partner; there was an increase in frail older adults. In the Generalized Estimating Equations analysis, frailty score was related to sociodemographic (increase in age, no partner, and low education level) and health variables (more diseases, drugs, falls, and decrease in functional capacity). There was an association between the variables of age (older), marital status (no partner), and loss of functional capacity.CONCLUSIONS Frailty syndrome was associated with increasing age, having no partner, and decreased functional capacity over time, and investments are required to prevent this syndrome and promote quality in aging.
OBJECTIVE:To measure the prevalence in frail elderly people, their consequences and associated demographic factors. METHODS:This was an epidemiological and cross-sectional study with a probabilistic sample composed of 240 elderly people (≥ 60 years) living in Ribeirão Preto, Sao Paulo state. Data were collected between November 2010 and February 2011, through a questionnaire that included socio-demographic data, fall assessment and the Edmonton Frailty Scale. Uni-variate and bivariate analyses were carried out. RESULTS:The mean age was 73.5 (± 8.4), with higher ages among women; 25% of the interviewees were aged 80 or older; 11.3% presented moderate frailty and 9.6% severe frailty. The prevalence of falls in frail elderly participants corresponded to 38.6%; higher levels were found among women and younger subjects (60 to 79 years old); 26.8% were victims of 1 to 2 falls, 27.1% of which occurred in the bedroom, 84.7% fell from their own height, 55.9% lost their balance, 54.2% suffered scratches and 78% were afraid of suffering a new fall. Higher fall prevalence levels were found in frail elderly 1,973 (1,094-3,556) compared to non-frail. CONCLUSIONS:We highlight the importance of addressing the health of frail elderly people, especially regarding the risk of falls, as well as of increasing investment in prevention strategies of these syndromes and in the formation of train like a virgin ed human resources to better care for this population.
Objective:To analyze the prevalence of falls and frailty syndrome and the association between these two syndromes in the elderly population. Method: Systematic review, without restriction of dates, in English, Portuguese and Spanish languages, in the databases PubMed, CINAHL, LILACS and in the SciElo virtual library. The association between both variables was extracted from the studies (Odds Ratio and 95% Confidence Intervals). Results: The review included 19 studies published between 2001 and 2015. The prevalence of falls in the frail elderly population was between 6.7% and 44%; in the pre-frail, between 10.0% and 52.0%, and in the non-frail, between 7.6% and 90.4%. The association between both variables presented a value of OR 1.80 (95% CI 1.51-2.13). Conclusion: There is evidence that falls are associated to the frailty in the elderly. Other factors may influence this association, such as age, sex, data collection instrument of the studies, place where they live and the process of senescence. Objectives: To characterize the sociodemographic profile of the elderly, verify the level of fragility according to gender, functional independence and instrumental activities of daily living, and to correlate the dimensions of the Functional Independence Measure and Instrumental Activities of Daily Living with age, education, frailty and morbidities. Methods: A cross-sectional and observational study using a sample of 240 elderly living in Ribeirão Preto, São Paulo (Brazil). The collection period was November/2010 and February/2011. The questionnaires used were: sociodemographic, Edmonton Frail Scale, Functional Independence Measure and the Instrumental Activities of Daily Living Scale of Lawton and Brody. For analysis, we used descriptive statistics and the Student's t-test and Pearson's correlation. Results: Mean age was 73.5 years (± 8.4), 57.5% were married, 39.1% had some level of fraility. Among the frail elderly, 29.8% had minimal dependence/supervision, and 81.9% had partial dependence for instrumental activities of daily living. Conclusion:There was a greater dependence in activities for the frail elderly, and females had a higher prevalence of frailty. The aim of this study was to analyse the changes in psychological stress and identify its basal predictors among elderly stroke survivors after 6 months following discharge from hospital to home directly, rather than to a rehabilitation facility. The sample comprised 50 elderly stroke survivors. Data were collected at 2 weeks (T1), at 3 months (T2), and at 6 months (T3) after hospital discharge. The following instruments were applied: Perceived Stress Scale-10 items (PSS-10), National Institute of Health Stroke Scale, Functional Independence Measure, and Geriatric Depression Scale-15 items. Study records indicated that the age of the study participants ranged from 60 to 87 years old (mean = 70.3; standard deviation = 7.6). The number of male and female participants was similar. The PSS-10 score decreased almost 6 points between T1 (mean = 15.1) and T3 (mean = 9.7; p < .001). Both Functional Independence Measure (p = .025) and Geriatric Depression Scale-15 (p = .017) scores at T1 predicted the PSS-10 score at T3. The study thus demonstrated that elderly stroke survivors experienced significant stress after hospital discharge, which tended to improve over the next 6 months. Depression and lower functional independence 2 weeks after discharge were predictors of a greater level of psychological stress at 6 months following discharge.
Objective: to examine the transition of care in families caring for elderly persons who suffered the first episode of a cerebrovascular accident. Methodology: an instrumental ethnographic case study was used. The sample comprised 20 subjects: 10 caregivers and 10 elderly persons aged 65 or over, of both sexes, with diagnoses of first episode of cerebrovascular accident, capable of communicating, and requiring care from a main carer in their family. The data was collected through interviews, observation, existing documentation and field notes. Qualitative analysis techniques were used to codify and classify the data and to formulate significant categories, which generated typologies of care. Results and Discussion: The central idea was the Transition of Care and showed the context in three typologies: The care process for the dependent elderly person, Strategies for the care process and Impact and acceptance of the limitations. Conclusion: The data indicates that caring for an elderly person after a cerebrovascular accident is a challenge for the family. The data permitted it possible to elaborate a proposal for a model for the organization of the work, with a view to holistic care delivery in the health services, forming a care network, which constitutes an advance for the area of nursing.Descriptors: Aged; Stroke; Family; Caregivers; Geriatric Nursing. Objectives: To assess the burden of caregivers of frail elders living at home; to characterize elderly that are considered frail and their caregivers; to assess the degree of dependency of the elderly according to the level of frailty and to correlate it with the burden of work of their caregivers. Methods: A transversal study with a sample of 60 caregivers and frail elderly who lived at home. Data collection was conducted in the home with frail elders and their caregivers. We used the sociodemographic profile of the elderly and the caregiver instruments, the Edmonton Frail Scale, the Functional Independence Measure for the elderly, and the Zarit Burden Interview Scale for the caregivers. Descriptive statistics and the Pearson test were employed for data analysis. Results: The majority of caregivers were female (75%), married (58.3%) and were children of the elderly (45%). Regarding the burden, 31.7% said they rarely felt burdened. However, a correlation was found between frailty and burden: the higher the level of frailty, the greater the caregiver burden. Conclusion: It was found that the majority of caregivers were female and the greater the degree of functional dependence, the greater the degree of frailty, which elevated the level of caregiver burden. Quanto à sobrecarga, 31,7% responderam que raramente se sentiam sobrecarregados. Mas, houve correlação entre a fragilidade e a sobrecarga, ou seja, quanto maior o nível de fragilidade, maior a sobrecarga do cuidador. Conclusão: Evidenciou-se a maioria dos cuidadores do sexo feminino e quanto maior o grau de dependência funcional, maior o grau de fragilidade o que eleva o nível de sobrecarga do cuidador.
