PurposeMost patients suffering from advanced lung cancer die within a few months. To exploit new therapy regimens we need better methods for the assessment of a therapy response.Material and MethodsIn a pilot study we prospectively enrolled 36 patients with advanced NSCLC and SCLC (34 stage IV, 2 stage IIIB) of whom 34 received standard platinum-based chemo/radiotherapy and two were treated with a tyrosine kinase inhibitor. We measured the levels of extracellular methylated SHOX2 DNA (mSHOX2) in plasma before and during therapy until re-staging. The mSHOX2 analysis was blinded with respect to the clinical data making it an observational study.ResultsAccording to the re-staging of 31 first-line patients, 19 patients were classified as non-responders while 12 patients were in the responder group. We observed a tight correlation between radiological data and the change of plasma mSHOX2 level as the equivalent for a therapy response. A ROC analysis showed a high discriminatory power for both patient groups already one week after therapy start (AUC 0.844). Additionally, a Kaplan-Meier and Cox Proportional Hazards analyses revealed a strong relationship between survival and plasma mSHOX2 value p≤0.001 (hazard ratio 11.08) providing some evidence for mSHOX2 also being a predictive marker.ConclusionThe longitudinal measurement of extracellular plasma mSHOX2 DNA yields information about the response to cytotoxic treatment and allows an early assessment of treatment response for lung cancer patients. If confirmed in a larger study this would be a valuable tool for selecting and guiding a cytotoxic treatment.
Zusammenfassung Dieses gemeinsame Positionspapier der 3 Fachgesellschaften Bundesarbeitsgemeinschaft der Akutkrankenh?user mit Abteilungen f?r fach?bergreifende Fr?hrehabilitation (BAG Fr?hrehabilitation), Berufsverband der Rehabilitations?rzte Deutschlands (BVPRM) und Deutsche Gesellschaft f?r Physikalische Medizin und Rehabilitation (DGPMR) stellt den aktuellen Stand, die Perspektiven und notwendige Ma?nahmen zur Weiterentwicklung der fach?bergreifenden Fr?hrehabilitation dar. Fr?hrehabilitation ist die fr?hestm?glich einsetzende Therapie in Kombination aus akutmedizinischer und rehabilitationsmedizinischer Behandlung. Fr?hrehabilitation ist dabei als erstes Glied einer nahtlosen Rehabilitationskette zu verstehen, gleichzeitig integraler und gesetzlich geforderter Bestandteil der Krankenhausbehandlung. Die Notwendigkeit und die Bedeutung der Fr?hrehabilitation steigen durch die demografische und die medizinisch-technische Entwicklung. Internationale Richtlinien (WHO, EU) und die nationale Gesetzgebung begr?nden ein Recht auf Fr?hrehabilitation. Das Positionspapier grenzt die fach?bergreifende Fr?hrehabilitation von der Fr?hmobilisation und der postakuten Rehabilitation ab. Ferner erfolgt die Einordnung in den Kontext weiterer Formen der Fr?hrehabilitation. Die verschiedenen Arten der Leistungserbringung, im Besonderen die bettenf?hrenden Fr?hrehabilitationsstationen (Acute Rehabilitation Units) und die Behandlung durch ein dezentrales Fr?hrehabilitationsteam (Acute Rehabilitation Team), werden dargestellt. Ziele und Aufgaben sowie strukturelle und prozessuale Voraussetzungen werden definiert. Typische Indikationen werden synoptisch dargestellt und sowohl Indikationskriterien als auch Ausschlusskriterien f?r die Indikationsstellung zur Fr?hrehabilitation daraus abgeleitet. Die Versorgungsstruktur in Deutschland und regionale Unterschiede werden analysiert und Bedarfssch?tzungen gegen?bergestellt. Aus ?konomischer Sicht wird das Finanzierungssystem der Fr?hrehabilitation im DRG-System diskutiert, das den Ressourcenaufwand derzeit nur inad?quat abbildet. Auf der Grundlage dieser Analysen werden grunds?tzliche Forderungen abgeleitet und ein Katalog konkreter Ma?nahmen zur Weiterentwicklung der Fr?hrehabilitation vorgelegt. Bereits vorhandene Ans?tze der Fr?hrehabilitation im Akutkrankenhaus sollen gest?rkt werden. Gesetzliche Vorgaben sollen umgesetzt und eine fach?bergreifende, bedarfsgerechte Versorgung mit Leistungen der Fr?hrehabilitation sichergestellt werden.
An early, intensive rehabilitative therapy accelerates the recovery of the functions of patients. It contributes to a reduction in the complication rate as well as an improvement in physical and social functioning/participation in the long-term follow-up. Early rehabilitation must be strengthened on the basis of the existing structures: the creation and maintenance of adequately qualified early-stage rehabilitation facilities, at least in hospitals with priority and maximum supply contracts. Patients with long-term intensive care and polytrauma must be rehabilitated as soon as possible (intensive medical rehabilitation).Specialists in physical and rehabilitative medicine, rehabilitative geriatrists, neurologists, orthopaedists and accident surgeons and other regional physicians must cooperate in a targeted manner. Exclusion criteria using corresponding OPS codes must be canceled. Additional specialist physician groups (anesthetists and intensive care physicians, general practitioners, accident and thoracic surgeons, internists) must be sensitized to the importance of early rehabilitation.In the case of more than 500,000 hospital beds, 25,000 beds should be identified as age- and diagnosis-independent early-care beds in the country-specific bed-care plans. A cost-covering financing of the different, personal and cost-intensive early rehabilitation must be ensured. A phase model similar to the BAR guidelines for neurological-neurosurgical early rehabilitation is to be considered for other disease entities.In order to make the rehabilitation process as successful as possible, medical (acute) treatment, medical rehabilitation, occupational integration and social integration have to be understood as a holistic event and are effectively interrelated, as a continuous process which accompanies the entire disease phase-wise. For this purpose, a continuous case management or a rehabilitation guidance has to be established.
Zusammenfassung Einleitung Die Untersuchung diente der Erhebung der Versorgungssituation in der neurologisch-neurochirurgischen Frührehabilitation (NNFR) in den Bundesländern Niedersachen (NDS) und Bremen (HB). Methodik Zur Untersuchung wurden alle Anmeldungen aus Akutkliniken zur NNFR in NDS / HB innerhalb einer Periode von zwei Wochen jeweils im November 2015 und 2016 erfasst und über einen weiteren Beobachtungszeitraum von sechs Wochen der Anteil der davon aufgenommenen Patienten ermittelt. Ebenso wurde ermittelt, welche Faktoren eine Aufnahme zur NNFR in NDS oder HB begünstigen bzw. erschweren. Ergebnisse Nur 45 % der angemeldeten Patienten konnten in einer Frührehabilitationseinrichtung in NDS / HB aufgenommen werden. Eine Aufnahme wurde deutlich durch das Vorhandensein isolierungspflichtiger Keime erschwert. Insgesamt nur 21 % der mit Problemkeimen besiedelten Patienten konnten aufgenommen werden, bei MRSA (Methicillin-resistenter Staphylococcus aureus) waren es sogar nur 13 %. Nachteilig wirkten sich ebenfalls eine Dialysepflicht (20 %), die Hauptdiagnosen Polyneuropathie / Guillain-Barré-Syndrom (GBS) (33 %) und hypoxischer Hirnschaden (37 %) aus, ebenso das Vorhandensein einer Beatmungspflicht (37 %). Eine leicht überdurchschnittliche Aufnahmewahrscheinlichkeit hatten Patienten mit den Hauptdiagnosen Subarachnoidalblutung (52 %) oder Hirninfarkt (51 %). Kaum Einfluss auf die Aufnahmewahrscheinlichkeit hatten die Faktoren Alter, Wert des Frührehabilitations-Barthelindex (FRB), Monitorpflicht, Vorhandensein einer Trachealkanüle, Dysphagie, Orientierungs- oder Verhaltensstörung, alle anderen Hauptdiagnosen und Anzahl der Anmeldungen in einer oder mehreren Frührehabilitationskliniken. Schlussfolgerung Die wohnortnahe NNFR für Patienten aus NDS / HB war in mehr als der Hälfte der Fälle nicht möglich. Die Studie sollte Anlass für eine differenzierte Betrachtung der aktuellen Versorgungsstrukturen im Bereich der NNFR in NDS und HB sein.
Zusammenfassung Zusammenfassung Hintergrund Fachübergreifende Frührehabilitation (FR) ist die Anwendung einer intensiven rehabilitativen Intervention noch während der akutmedizinischen Behandlung im Krankenhaus. Nach der Einführung des Komplexkodes im Jahr 2005, dessen Mindestmerkmale im Operationen- und Prozedurenschlüssel detailliert festgelegt sind (OPS 8–559.-), stehen im DRG-System siebzehn separate Fallpauschalen zur Verfügung. Im DRG-Grouping zeigt sich jedoch eine sehr unterschiedliche Auswirkung im Kontext der auslösenden Behandlungsaspekte. Ziel war die Darstellung der DRG-Systematik zur fachübergreifenden Frührehabilitation und einer Umfrage zu Strukturmerkmalen unter bundesdeutschen Kliniken mit einer Abteilung für eine fachübergreifende Frührehabilitation. Methoden Die DRG-Systematik bei Anwendung der fachübergreifenden Frührehabilitation wurde exemplarisch dargestellt. Eine anonymisierte Umfrage zu Strukturmerkmalen und Kennzahlen zur DRG-Systematik wurde unter Kliniken mit entsprechender Fachabteilung durchgeführt. Ergebnisse Die DRG-Systematik unterscheidet in 2018 einerseits „klassische“ Fallpauschalen mit einem identischen Casemix zwischen unterer und oberer Grenzverweildauer und andererseits krankenhausindividuelle Entgelte für DRGs, die nicht mit einem Casemix in einer Fallpauschale vergütet werden. Unter den Fallpauschalen nehmen die Prä-MDC eine Sonderstellung ein. Die Klinik- Befragung ergab, dass die DRGs „Frührehabilitation bei Krankheiten und Störungen an Muskel-Skelett-System und Bindegewebe“ ohne (I40Z) und mit (I96Z) operativen Prozeduren die in 2018 am häufigsten abgerechneten DRGs in diesen Kliniken sind. Eine Mehrheit (7 von 10) der befragten Kliniken bietet zusätzlich noch eine neurologisch-neurochirurgische oder geriatrische Frührehabilitation an. Diskussion Patient:innen mit entsprechendem Bedarf haben einen im Sozialgesetz verankerten Anspruch auf Frührehabilitation noch im Akutkrankenhaus. Warum wird dieser Anspruch und auch der Bedarf so wenig konsequent umgesetzt? Sicher, die DRG-Systematik der fachübergreifenden Frührehabilitation ist komplex, gestaltet sich jedoch nach der hier dargestellten Systematik gut kalkulierbar.
