AIMS: To observe the acquisition of skills in videolaparoscopic surgery in a group of residents of general surgery, after training in a high fidelity simulator.METHODS: Case series with a qualitative, descriptive and applied approach. The participants were residents of general surgery who had already completed the surgical techniques module and accomplished the training in a high-fidelity simulator of videolaparoscopy. Data were collected through a semi-structured questionnaire applied to the residents after the training and interviews with the preceptors surgeons, recorded between one year and a year and a half after the training, when the residents were already performing the procedure in real patients.RESULTS: According to the inclusion criteria, the responses of seven general surgery residents were analyzed. After simulator training, they reported to have acquired skills in videolaparoscopic procedures. Two among the interviewed residents mentioned that they have had difficulties in performing the tasks due to the high sensitivity required by the simulator in some procedures. The "Six-Steps" methodology used during simulator training was well accepted by the residents. The importance of the presence of a tutor during the use of the simulator was pointed out. The videolaparoscopic surgery preceptors reported that skills acquired by the residents after training in the high-fidelity simulator were transferred to the same procedure in real patients.CONCLUSIONS: The present study allows to conclude that, according to the technical perception and criticals of residents and preceptors of the medical residency program in general surgery, the standardized training using the high-fidelity videolaparoscopic surgery simulator has proved to be effective for skills acquisition by this group of residents, including future surgical procedures in real patients.
Itágores Hoffman I Lopes S. Coutinho 3 INTRODUÇÃOA apresentação clínica de pacientes com carcinoma hepatocelular depende principalmente da extensão da doença. Particular interesse tem sido aplicado à possibilidade de identificar a doença assintomática em pacientes de alto risco, em especial aqueles portadores de cirrose ou hepatite crônica por vírus B, devido às vantagens da intervenção precoce antes do aparecimento de metástase 1 .O pulmão é o local mais comum de metástase do carcinoma hepatocelular. O envolvimento ósseo por metásta-se pode ocorrer em dois a 16% dos pacientes. O diagnósti-co precoce da doença primária pode oferecer a única real possibilidade para se estabelecer o tratamento efetivo e aumentar a sobrevida a longo prazo 1 . Tem sido incomum o primeiro sinal clínico do paciente ser devido à metástase óssea. 2 Apresentamos o caso de um paciente com carcinoma hepatocelular cuja manifestação clínica inicial foi relacionada com a metástase óssea. RELATO DO CASOPaciente do sexo masculino, 39 anos, foi atendido na Disciplina de Clínica Cirúrgica III (UFMA) apresentando dor no hipocôndrio direito há duas semanas, associada a perda de peso e aumento de volume abdominal. Neste perío-do apresentou alguns episódios de febre (38ºC). Os antecedentes pessoais revelou uma história de dor lombar há três meses, diagnosticada como hérnia de disco e em tratamento com antiinflamatório não hormonal. O paciente apresentava história de etilismo social.Ao exame físico, encontrava-se com sinais vitais normais, deambulação prejudicada, emagrecido e hipocorado (++/4). O abdome estava flácido e o fígado palpável a 10cm do rebordo costal direito. A coluna lombar estava dolorosa à palpação.Realizou exames laboratoriais que demonstraram: hematócrito 32%; hemoglobina 9g/dl; bilirrubina total 3,2mg/dl (direta 2,3mg/dl); fosfatase alcalina 450 U/l; AST 1.275 U/l; ALT 1.351 U/l. A tomografia computadorizada do abdome evidenciou fígado aumentado de volume com múl-tiplas massas heterogêneas, com componentes císticos e sólidos de dimensões variadas distribuídas difusamente no parênquima hepático com focos de calcificação em algumas lesões (Figura 1) e lesões osteolíticas com componentes de partes moles em L5 (Figura 2).Na internação, o paciente evoluiu com dor abdominal e sinais de choque hipovolêmico. Foi submetido a tratamento cirúrgico de urgência e se observaram hemoperitônio e rotura de lesão em lobo esquerdo do fígado. A hemostasia foi realizada por rafia da lesão e o material enviado para estudo anatomopatológico. À exploração da cavidade abdominal, não foram observadas outras alterações.
