A AB BS ST TR RA AC CT T O Ob bj je ec ct ti iv ve e: : This study was conducted to evaluate the efficacy of pre-emptive pregabalin compared with plasebo and diclofenac combination for attenuating postoperative pain, analgesic consumption and cognitive function after laparoscopic cholecystectomy. M Ma at te er ri ia al l a an nd d M Me et th ho od ds s: : Sixty adults with ASA physical status I-III of either sex undergoing elective laparoscopic cholecystectomy were included in this prospective, randomized placebo controlled, single-blind study. Subjects were divided into two groups being 30 pat.ents in each to receive either a matching placebo or pregabalin 300 mg, administered orally 1 h before surgery. Intramuscular 75 mg diclofenac sodium was given to placebo patients 15-20 minutes before the estimated finishing time of surgery for pain relief after surgery. In the first 15 minutes after extubation, Aldrete Score and Ramsay Sedation Scale were evaluated. Mini mental test (MMT) was performed 1 hour and 6 hours after extubation. Postoperative pain was assessed by visual analogue scale (VAS). Time to first analgesic requirement, total analgesic dose and side effects were recorded. R Re es su ul lt ts s: : The difference between preoperative 1 st h and 6 th h MMT values was not statistically significant between the two groups. VAS scores were significantly higher in Group C compared to Group P (p <0.01). In Group P, the time to first analgesic rescue dose was significantly longer than in Group C (p<0.01). Total analgesic doses were significantly lower than in Group C patients (p<0.01). Sedation was significantly higher in Group P (p<0.05). Objective To determine the possible risk factors associated with hepatic dysfunction during open-heart surgeries. Methods After excluding 71 patients, 307 patients with possible low and moderate cardiac risk who underwent either coronary artery bypass graft surgery (CABG) (n=176) or valve repair surgery (mitral valve, mitral and aortic valves and/or tricuspid valve) (n=131) were investigated prospectively during a 6-month period. Hyperbilirubinemia is defined as an occurrence of a plasma total bilirubin concentration >34 µmol/L (2 mg/dL) in any measurement during the postoperative period; the patients were divided into groups with or without postoperative hyperbilirubinemia. The collected parameters were: alanine transaminase (ALT), aspartate transaminase (AST), alkaline phosphatase (ALP), total bilirubin (TBil), gamma-glutamyl transpeptidase (GGT) and albumin. The parameters were collected preoperatively and postoperatively on days 1, 3 and 7. Preoperative, intraoperative, and postoperative risk factors were investigated. Logistic regression analysis was performed to identify the risk factors for postoperative hyperbilirubinemia. Results Postoperative hyperbilirubinemia was observed in 7 of 176 patients (4%) who underwent CABG, and in 11 of 131 patients (8.4%) who underwent valve replacement surgeries. Independent risk factors for early postoperative hyperbilirubinemia were found as: ejection fraction (EF), aortic cross-clamp (ACC) time, intensive care unit stay and extubation time ( P <0.001). In comparison to CABG procedures, postoperative hyperbilirubinemia was observed more frequently in patients undergoing valve surgeries ( P =0.027). Conclusion Low EF and prolonged ACC time are significant independent risk factors for early postoperative hyperbilirubinemia during open-heart surgeries with cardiopulmonary bypass. Valve surgeries show a higher incidence of hyperbilirubinemia in comparison to CABG.
Deliryum kardiyak cerrahi sonrası sık görülen geçici nöropsikiyatrik bir sendromdur. Yaşlılarda ve yoğun bakımda yüksek prevalansına rağmen klinisyenler tarafından sıklıkla tanınmamakta ya da depresyon, yoğun bakım sendromu, demans gibi kritik hastalıkların beklenen önemsiz komplikasyonları olarak görülmektedir. Çalışmamızın amacı, açık kalp cerrahisi sonrasında yoğun bakım ünitesinde, 65 yaş ve üzeri hastalarda, prevalans ve deliryum ile ilişkili risk faktörlerini belirlemektir. Çalışmaya 65 yaş ve üzeri 217 hasta dahil edildi. Veriler yoğun bakım ünitesinde konfüzyonu değerlendirme ölçeği (YBÜ-KDÖ) [The Confusion Assesment Method for the In-tensive Care Unit (CAM-ICU)] ve deliryumun tanımında altın standart olan tüm diğer psikiyatrik tanıların da sınıflandırıldığı, uluslararası kılavuz olan, ''Diagnostic and Statistical Manual for Mental Disorders (DSM-IV)'' ile ölçülerek toplandı. Çalışmada deliryumun prevalansı % 29,5 olarak bulundu. İleri yaş, erkek cinsiyet, yüksek acute physiology and chronic health evaluation II (APACHE II) değeri, New York heart association functional classification (NYHA)>2, serebrovasküler hastalık, kötü fiziksel durum, aortik cerrahi, ameliyat süresi, iskemi süresi, hematokrit düşüklüğü (<30), kan laktat düzeyi artışı (>1,2mmol/L), hipoalbuminemi (<3g/dL), BUN/Kreatinin oranında artış (>18), immobilizasyon, postoperatif 48. saat medikasyonlar, uyku bozukluğu ve yoğun bakımda kalma süresi yaşlılarda deliryum gelişiminde risk faktörleri olarak tespit edildi. Açık kalp cerrahisi sonrası yaşlı hastalarda görülen deliryum multifaktöryel bir hastalıktır. Med., 2012; 29:101-107 ABSTRACT Delirium is a neuropsychiatric syndrome and commonly seen after cardiac surgery. In intensive care units, it has a high prevalence in the elderly but is not easily recognized by clinicians or in critically ill patient. Delirium is thought as an expected minor complication of other diseases such as depression, intensive care syndrome, dementia. The goal of this study was to determine the risk factors associated with delirium after open heart surgery in the 65 years and older patients in the intensive care unit. The sample consisted of 217 patients admitted to an intensive care unit. Data were collected using the Confusion Assessment Method (CAM) and the DSM IV. The prevalence of delirium was 29,5% . The risk factors determining the development of delirium in the eldery were detected as, advanced age, male gender, high APACHE II value, NYHA>2, cerebrovascular disease, poor physical condition, aortic surgery, operative time, ischemia time, low hematocrit (<30), increase in blood lactate level (>1.2 mmol/L), hypoalbuminemia (<3g/ dL), increased BUN / creatinine ratio (>18), immobilization, postoperative 48 hours of medications, sleep disturbance and intensive care unit stay. Kardiyovaskuler cerrahi sonrası uzamış yoğun bakım ihtiyacı olan kalp yetmezliği hastalarında nutrisyonel desteğin etkileri Giriş ve Amaç: Bu çalışmada kardiyovaskuler cerrahi sonrası uzamış yoğun bakım ihtiyacı olan kalp yetmezlikli hastalarda, enteral ve kombine (enteral+parenteral) beslenmenin biyokimyasal parametreler, morbitide ve mortalite üzerine etkilerinin ortaya konulması amaçlandı. Gereç ve Yöntem: Hastalar enteral (Grup E, n=30) ve kombine (enteral+parenteral) beslenme (Grup EP, n=30) olmak üzere rastgele iki eşit gruba ayrıldı. Biyokimyasal parametreler; glukoz, kolesterol, trigliserid, albumin, alanin aminotransferaz (ALT), aspartat aminotransferaz (AST), laktat dehidrojenaz (LDH), alkalen fosfataz (ALP), direkt bilirübin, gamma glutamil transferaz (GGT) ve elektrolit düzeyleri 1. 7. ve 14. günlerde incelendi. Yan etkiler ve mortalite kaydedildi. Bulgular: İki grup karşılaştırıldığında demografik özellikler ve incelenen biyokimyasal parametrelerde istatiksel olarak anlamlı fark saptanmadı (p>0.05). Grup içi karşılaştırmalarda, Grup E'de glukoz seviyelerinde istatiksel olarak anlamlı fark saptanmaz iken (p>0.05), Grup EP'de yükselme görüldü (p=0.01). Kolesterol ve trigliserid seviyeleri, her iki grupta yükselirken, albümin değerlerinde azalma saptandı (p<0.001). Karaciğer enzim değerlerinde ise; Grup E'de ALP ve AST'de düşme, Grup EP'de ise ALP'de düşme gözlendi (p<0.05), LDH ve total bilirübin düzeyleri benzerdi. Grup E'de sodyum ve klor değerleri yükselirken, kalsiyum değerlerinde düşme ve Grup EP'de ise; sodyum ve potasyum değerlerinde yükselme saptandı (p<0.05). Komplikasyonlar ve mortalite açısından anlamlı fark saptanmadı (p>0.05). Sonuç: Erken enteral beslenme postoperatif dönemde ilk başlanan beslenme tedavisi olmalıdır. Hedeflenen gerekli enerji miktarına ulaşılamıyor ise, enteral ve parenteral beslenme birlikte başlanabilir.
