Although immediate workup for microbiology, including Gram stain and culture, is indispensable and well established, crystal analysis may at times not be immediately performed. Our study suggests that when crystal identification cannot be done immediately, it can be safely performed up to 3 days after arthrocentesis when SF is stored at 4°C or even at stable room temperature (20°C).
Peripheral arterial disease is more aggressive in concomitant diabetes posing an increased risk for critical limb ischemia and subsequent limb loss. The majority of therapies available are not effective to prevent amputation in patients with severe disease. The current observational study reports the effect of the heparin-induced extracorporal LDL-precipitation (H.E.L.P.) as a novel therapeutic approach in patients with severe diabetic foot syndrome. Seventeen diabetic patients with septic foot lesions recruited from the diabetic outpatient clinic underwent H.E.L.P. apheresis regularly until fibrinogen levels were stabilized at 3 g/l or infection was controllable as evidenced by alleviation of necrosis. Patients were subsequently followed up for 2 to 73 months. Fibrinogen levels were reduced by 68% after H.E.L.P. treatment. No severe complications were noted. Necrosis could be confined in sixteen patients. Minor amputations were indicated in twelve patients. Three patients underwent major amputations of the lower limb and two patients received surgical reconstruction. In conclusion, H.E.L.P. apheresis may offer an alternative therapeutic option to diabetic patients with critically ischemic feet and appears to have a beneficial major/minor amputation ratio.
ZUSAMMENFASSUNGBei Patienten mit einem Diabetes mellitus Typ 2 finden sich in der Regel weitere metabolische Erkrankungen, so sind neben der Adipositas eine Hyperurikämie und Gicht häufig assoziiert. Vor allem beim Bestehen von Langzeitfolgen, wie der diabetischen Nephropathie ist das Auftreten einer Hyperurikämie zu beobachten. Besteht zudem eine periphere Neuropathie kann es sein, dass der für die Diagnose einer Gicht so entscheidende heftige Gichtanfallsschmerz vom Patienten gar nicht mehr als Signal wahrgenommen wird. So kann sich stumm eine tophöse Gicht mit destruierenden Gelenkveränderungen entwickeln. Systematische Untersuchungen darüber, wie häufig bei einem diabetischen Fußsyndrom auch eine tophöse Gicht eine Rolle spielt und wie sich dies auf Komplikationen wie z. B. Amputationen auswirken, existieren nicht. Die folgende Arbeit stellt anhand von Patientenbeispielen mögliche Szenarien vor, und diskutiert unter Berücksichtigung der vorhandenen Evidenz praktische Überlegungen zur Diagnostik sowie Therapie von Patienten mit Diabetes und der komplizierenden Komorbidität Gicht.
ZUSAMMENFASSUNGDer Charcot-Fuß ist in Deutschland hauptsächlich mit dem Diabetes mellitus und seiner häufigsten Folgeerkrankung, der Polyneuropathie assoziiert. Diese diabetische neuropathische Osteoarthropathie (DNOAP) weist immer eine Hyposensibilität auf und kann mono- sowie bilateral mit Rötung, Schwellung, Hyperthermie einhergehen. Oft können ein unbemerktes Trauma, eine Operation oder eine Weichteilläsion Auslöser sein. Die Behandlung richtet sich nach der Ausdehnung des Befundes und dem klinischen Stadium – im komplizierten Stadium mit Wunde steht dabei die lokale sowie systemische Ausbreitung der Infektion im Fokus, aber auch eine Entlastung, die Behandlung der Grunderkrankung, die Prüfung des Gefäßstatus und chirurgische Behandlungsmaßnahmen sind essenziell. Die Indikation zu korrigierenden chirurgischen Interventionen wird stringent und zuückhaltend gesehen. Eine optimale orthopädische Schuh- oder TCC (Total Contact Cast)-Versorgung mit entsprechender Anbindung beim Diabetologen und Chirurgen ist neben der optimalen Behandlung der Grunderkrankung entscheidend für die Vermeidung von Komplikationen, die mit einer wesentlich höheren Morbidität und Mortalität assoziiert sind. Kernziel der Behandlung ist die Prävention von Rezidiven. Die DNOAP muss vor diesem Hintergrund als interdisziplinäres Krankheitsbild verstanden werden
Compared to other chronic inflammatory diseases, gout appears to based on a rather "simple" pathophysiology and therefore the amount of teaching time in medical school and during internship is rather limited. On the other hand, several problems in short- and long-term management still need to be solved - combined with the problem of an increased incidence in elderly people. However, there is significant advance in the knowledge of its pathophysiology including the fact that gout is more than a pure "crystal arthopathy" but rather within the spectrum of chronic inflammatory immunologic diseases. This includes cytokines such as interleukin-1 and intracellular signaling via the inflammasome. For treatment, the novel and effective xanthine oxidase inhibitor febuxostat has been added to the therapeutic armamentarium. Guidelines of EULAR and BSR support the physician in the long-term management of the numerous gout patients.
