At Federal Center of Invalid Expertize and Rehabilitation 175 adolescents with 11-111 degree idiopathic scoliosis were under the treatment. To increase the efficacy of conservative treatment new fixative-reclination corset with springing inserts as well as new functional corrective corset were worked out and those corsets were applied for the treatment of patients. In patients with II degree deformity correction ofprimary arch was from 37% in thoracic to 80% in lumbar scoliosis. In patients with III degree deformity correction made up from 29% in thoracic to 61%o in combined scoliosis. Those data were obtained at radiologic examination in terms under 3 years.

Обследованы 10 здоровых испытуемых и 50 пациентов с II-III степенью ИС С-и S-образного типа. Использована компьютерная электромиографическая методика, позволившая получить ЭМГ-профиль мышц туловища и нижних конечностей и количественные показатели работы мышц при ходьбе. Установлены следующие изменения ЭМГпараметров при ходьбе больных ИС: увеличение электрической активности многих мышц туловища на выпуклой стороне искривления позвоночника и уменьшение на вогнутой; противоположный характер имеют изменения активности мышц нижних конечностей: возрастание активности мышц на ноге, соответствующей вогнутой стороне искривления, и снижение на выпуклой стороне. Наряду с количественными нарушениями ЭМГпрофиля мышц наблюдаются качественные-снижение максимумов активности по амплитуде и пролонгирование их на соседние фазы шага. З За ак кл лю юч че ен ни ие е.. Отмеченные нарушения иннервационной структуры ходьбы резко усиливаются при переходе от II к III степени сколиоза и более выражены при C-образном искривлении позвоночника. Большинство симптомов изменения работы мышц связано с ослаблением функции мышц туловища при увеличении степени деформации позвоночника и компенсаторной реакцией организма на понижение устойчивости при ходьбе. Клинико-биомеханические и физиологические результаты комбинированного консервативного лечения пациентов с идиопатическим сколиозом II-III степени А.А. Скоблин, А.С. Витензон, И.Г. Алексеенко Федеральное бюро медико-социальной экспертизы, МоскваЦель исследования. Анализ эффективности комбини-рованного метода лечения пациентов с идиопатическим сколиозом II-III степени, сочетающего искусственную коррекцию движений посредством функциональной элек-трической стимуляции мышц при ходьбе в корригирую-щем корсете. Материал и методы. Объектами исследования и лечения были 50 больных идиопатическим сколиозом, из них 20 со II степенью искривления позвоночника, 30 -с III. При-менена модификация функционально-корригирующего корсета каркасного типа, конструкция которого дополне-на латеральными пелотами на пружине. Всем больным осу-ществлен 15-дневный курс функциональной электрической стимуляции при ходьбе в корригирующем корсете. Элект-ростимуляции были подвергнуты мышцы туловища с двух сторон: крестцово-остистые, интеркостальные, ромбовид-ные, трапециевидные, а также большая и средняя ягодич-ные мышцы на вогнутой стороне искривления. Результаты. Установлено, что под влиянием комплекса ле-чения происходит значительная коррекция деформации позвоночника при разных типах сколиоза, а также норма-лизация биомеханической и иннервационной структуры ходьбы. Отдаленные результаты лечения отслежены у 36 пациентов в срок до трех лет. При соблюдении предписан-ного ортопедического режима и проведении повторных курсов функциональной электрической стимуляции с пе-риодичностью один раз в 8-10 мес. отдаленные результа-ты расценены нами как хорошие в 29 случаях, как удов-летворительные -в 8. Заключение. Полученный эффект обусловлен уменьше-нием сколиотического искривления, существенным уси-лением работы симметричных мышц туловища и нижних конечностей и более правильным их функционировани-ем при ходьбе.
