The literature review is devoted to the important problem – orthotics in children and adolescents with idiopathic scoliosis (AIS). The main types of braces used in complex treatment of these patients are described. The indications for orthotic treatment and its results were analyzed. The orthotics potentialities in conservative correction of spinal deformities were shown.
At Federal Center of Invalid Expertize and Rehabilitation 175 adolescents with 11-111 degree idiopathic scoliosis were under the treatment. To increase the efficacy of conservative treatment new fixative-reclination corset with springing inserts as well as new functional corrective corset were worked out and those corsets were applied for the treatment of patients. In patients with II degree deformity correction ofprimary arch was from 37% in thoracic to 80% in lumbar scoliosis. In patients with III degree deformity correction made up from 29% in thoracic to 61%o in combined scoliosis. Those data were obtained at radiologic examination in terms under 3 years.
Examination and treatment results of 925 patients with lumbar-sacral ostechondrosis are analyzed. Two groups of patients were defined: patients with mainly chronic recurrent radiculitis and patients with radiculoneuritis. Complex approach to the treatment included pain relief, orthotics and artificial movement correction using muscular electrostimulation at walking. Studies using biomechanic and electrophysiologic methods showed high efficacy of that medical rehabilitation complex. Orthotics provided fixation and unloading of the spine, promoted elimination of pain syndrome. Use of corset and artificial movement correction resulted in pain relief, normalization of biomechanic and innervative gait structure, achievement of firm long remission.

Обследованы 10 здоровых испытуемых и 50 пациентов с II-III степенью ИС С-и S-образного типа. Использована компьютерная электромиографическая методика, позволившая получить ЭМГ-профиль мышц туловища и нижних конечностей и количественные показатели работы мышц при ходьбе. Установлены следующие изменения ЭМГпараметров при ходьбе больных ИС: увеличение электрической активности многих мышц туловища на выпуклой стороне искривления позвоночника и уменьшение на вогнутой; противоположный характер имеют изменения активности мышц нижних конечностей: возрастание активности мышц на ноге, соответствующей вогнутой стороне искривления, и снижение на выпуклой стороне. Наряду с количественными нарушениями ЭМГпрофиля мышц наблюдаются качественные-снижение максимумов активности по амплитуде и пролонгирование их на соседние фазы шага. З За ак кл лю юч че ен ни ие е.. Отмеченные нарушения иннервационной структуры ходьбы резко усиливаются при переходе от II к III степени сколиоза и более выражены при C-образном искривлении позвоночника. Большинство симптомов изменения работы мышц связано с ослаблением функции мышц туловища при увеличении степени деформации позвоночника и компенсаторной реакцией организма на понижение устойчивости при ходьбе.
