Цель. Изучить в выборке российских пациентов ассоциации полиморфного маркера rs4664308 гена PLA2R1 с риском развития идиопатической мембранозной нефропатии (ИМН) и уровнями антител к трансмембранному рецептору фосфолипазы А2 М-типа (PLA2R).Материал и методы. Аллели и генотипы полиморфного маркера rs4664308 гена PLA2R1 идентифицировали у 70 пациентов с ИМН и в двух контрольных группах, в том числе у 100 доноров крови без заболеваний почек и 40 пациентов с другими морфологическими вариантами хронического гломерулонефрита (ХГН). У 33 больных ИМН проанализировали уровни антител к PLA2R в зависимости от носительства генотипов исследуемого полиморфного маркера.Результаты. В группе больных ИМН по сравнению со здоровыми донорами выявлено статистически значимое увеличение частоты аллеля А (81,5% и 59,5%, соответственно) и генотипа А/А (66% и 34%), которое ассоциировалось с повышенным риском развития ИМН у носителей аллеля А (отношение шансов [ОШ] 2,98; 95% доверительный интервал [ДИ] 1,79-4,98) и генотипа А/А (ОШ 3,97; 95% ДИ 2,09-7,52). Среди больных ИМН доля носителей аллеля А и генотипа А/А оказалась статистически значимо выше, чем у пациентов с другими вариантами ХГН, а носительство этих генетических вариантов увеличивало риск развития ИМН примерно в 2,5 раза. В двух контрольных группах частота аллелей и генотипов исследуемого полиморфного маркера значимо не различалась. У больных ИМН с генотипом А/А антитела к PLA2R определялись недостоверно чаще, чем у носителей аллеля G (87,0% и 60,0%, соответственно, р=0,103).Заключение. Статистически значимые различия распределения аллелей и генотипов полиморфного маркера rs4664308 гена PLA2R1 у больных ИМН и лиц без данной патологии свидетельствуют об ассоциации исследуемого полиморфного маркера с развитием ИМН. Носительство аллеля А и генотипа А/А является фактором риска развития ИМН, а носительство аллеля G и генотипа G/G ассоциировано с низким риском развития этого заболевания.Ключевые слова.
