Эмануэля» РАН, Москва, Россия резюме Цель исследования. изучить ассоциацию полиморфных маркеров (ПМ) G(-238)A гена TNF, G(-174)c гена IL6 и G(-1082) A гена IL10 с клиническими особенностями хронического гломерулонефрита (хГН) и ответом на иммуносупрессивную терапию (иСТ). Материалы и методы. у 102 больных хГН проанализировали клинические синдромы на момент установления диагноза, морфологические варианты нефрита и ответ на иСТ в зависимости от носительства исследуемых ПМ генов TNF, IL6 и IL10. результаты. Ассоциации ПМ G(-238)A гена TNF с особенностями клинической картины хГН не обнаружено. у носителей аллеля С ПМ G(-174)c гена IL6 по сравнению с гомозиготами GG чаще отмечалось нарушение функции почек на момент установления диагноза (р=0,014). у носителей генотипа АА ПМ G(-1082)A гена IL10 чаще обнаруживали АГ (р=0,023); кроме того у них наблюдалась тенденция к более частому сочетанию нефротического синдрома и АГ (р=0,082). При ана-лизе распределения морфологических вариантов хГН обнаружено, что пролиферативные варианты чаще выявляли у боль-ных с генотипом GG (ген TNF) по сравнению с носителями аллеля А (р=0,067), а непролиферативные формы -у гомо-зигот GG (ген IL6) по сравнению с носителями аллеля С (р=0,067). При изучении ответа на иСТ обнаружено, что полный ответ к 12-му месяцу лечения чаще наступал у носителей аллеля С гена IL6 (р=0,045) и генотипа GG гена IL10 (р=0,030). Заключение. Обнаружена ассоциация ПМ G(-174)c гена IL6 и G(-1082)A гена IL10 с клиническими особенностями хГН и ответом на иСТ.Ключевые слова: хронический гломерулонефрит, полиморфный маркер, ген TNF, ген IL6, ген IL10.
Pandemia of diabetes mellitus (DM) remains one of the biggest worldwide medical problems associated with dangerous complications, particularly diabetic nephropathy (DN), which is the leading cause of the end-stage renal disease. It was confirmed convincingly today that podocytes injury is implicated primarily or secondarily in various proteinuric glomerular diseases, including DN. The characteristic signs of podocytopathy are podocytes hypertrophy, foot process effacement, detachment from the glomerular basal membrane and apoptosis. Structural and functional alterations of podocytes evolve early, outpacing clinically significant albuminuria. The increasing loss of podocytes is associated with morphological and clinical signs of DN progression. This review details the molecular and cellular events, mediators and signaling pathways that contribute to podocytes injury in DM, discusses the most important achievements, clinical and experimental data in these issues, describes the urinary biomarkers of podocytes injury.
