Purpose The incidence, patient features, risk factors and outcomes of surgery-associated postoperative acute kidney injury (PO-AKI) across different countries and health care systems is unclear. Methods We conducted an international prospective, observational, multi-center study in 30 countries in patients undergoing major surgery (> 2-h duration and postoperative intensive care unit (ICU) or high dependency unit admission). The primary endpoint was the occurrence of PO-AKI within 72 h of surgery defined by the Kidney Disease: Improving Global Outcomes (KDIGO) criteria. Secondary endpoints included PO-AKI severity and duration, use of renal replacement therapy (RRT), mortality, and ICU and hospital length of stay. Results We studied 10,568 patients and 1945 (18.4%) developed PO-AKI (1236 (63.5%) KDIGO stage 1500 (25.7%) KDIGO stage 2209 (10.7%) KDIGO stage 3). In 33.8% PO-AKI was persistent, and 170/1945 (8.7%) of patients with PO-AKI received RRT in the ICU. Patients with PO-AKI had greater ICU (6.3% vs. 0.7%) and hospital (8.6% vs. 1.4%) mortality, and longer ICU (median 2 (Q1-Q3, 1–3) days vs. 3 (Q1-Q3, 1–6) days) and hospital length of stay (median 14 (Q1-Q3, 9–24) days vs. 10 (Q1-Q3, 7–17) days). Risk factors for PO-AKI included older age, comorbidities (hypertension, diabetes, chronic kidney disease), type, duration and urgency of surgery as well as intraoperative vasopressors, and aminoglycosides administration. Conclusion In a comprehensive multinational study, approximately one in five patients develop PO-AKI after major surgery. Increasing severity of PO-AKI is associated with a progressive increase in adverse outcomes. Our findings indicate that PO-AKI represents a significant burden for health care worldwide. Background This prospective, randomized, double-blind trial aimed to compare the postoperative analgesic efficacy of One-Level pre-incisional erector spinae plane block (ESPB) and Bi-Level pre-incisional ESPB in patients undergoing video-assisted thoracic surgery (VATS). Methods This pilot trial was conducted between April 2022 and February 2023 with sixty patients. The patients were randomly divided into two groups. In One-Level ESPB Group (n = 30) block was performed at the thoracal(T)5 level with the 30 ml 0.25% bupivacaine. In the Bi-Level ESPB Group (n = 30) block was performed at T4 and T6 levels by using 15 ml of 0.25% bupivacaine for each level. In the postoperative period, 50 mg dexketoprofen every 12 h and 1 g paracetamol every 8 h were given intravenously (IV). Patient-controlled analgesia (PCA) prepared with morphine was applied to the patients. 0.5 mg/kg of tramadol was administered via IV for rescue analgesia. Visual analog scale (VAS) scores were recorded in the postoperative 1st, 2nd, 4th, 12th, 24th, and 48th -hours. The need for additional analgesics and side effects were recorded. In two groups, patients’ demographics and postoperative hemodynamic data were recorded. Results VAS scores at resting were statistically significantly higher at the 1st (p: 0.002) and 4th -hour (p: 0.001) in the One-Level ESPB. When the groups were evaluated in terms of VAS coughing scores, the 4th -hour (p: 0.001) VAS coughing scores results were found to be statistically significantly higher in the One-Level ESPB group. In terms of VAS values evaluated during follow-up, the rates of VAS coughing score > 3 values were found to be statistically significantly lower in the Bi-Level ESPB group (p: 0.011). There was no statistically significant difference between the groups in terms of side effects, morphine consumption, and additional analgesic use (p > 0.05). Conclusions Adequate analgesia was achieved in the early postoperative period in the group treated with Bi-Level ESPB with similar morphine consumption and side effects. This may be an advantage, especially in the early postoperative period when the pain is quite intense.
INTRODUCTION: The effects of midazolam-based intravenous anesthesia (TIVA) and sevoflurane-based inhalation anesthesia (SEVO) maintenance on postoperative hematological parameters were compared in patients undergoing coronary artery surgery. METHODS: The 100 patients included in the study were divided into two groups according to the anesthesia management: All patients' anesthesia inductions were performed with 10 μgkg-1 fentanyl, 0.15 mgkg-1 midazolam, and 0.8 mgkg-1 rocuronium. Anesthesia was maintained with total intravenous anesthesia in the TIVA group patients, while sevoflurane was used in the SEVO group patients. Demographic, clinical, operative data, red cell distribution width (RDW), and mean platelet volume (MPV) values were recorded. RESULTS: Except that the extubation time was shorter in the SEVO group, no difference was observed between the groups in terms of all these variables. There was a significant increase in postoperative RDW and MPV measurements in both groups. Postoperative RDW value was lower in the inhalation anesthesia group compared to the TIVA group (p=0.013). DISCUSSION AND CONCLUSION: In our study, RDW was found to be lower in the inhalation anesthesia group, indicating less inflammation. There was no difference in terms of postoperative complications and mortality.
Amaç: Dünyada yaşlı nüfusun %50-60'ında vitamin D eksikliği vardır. Kritik hastalarda ise vitamin D eksikliğinin (25 (OH) D<20ng/ ml) prevalansı %70'i aşmaktadır. Bu çalışmada Yoğun Bakım Ünitesi (YBÜ)'nde takip ve tedavi edilen 65 yaş üstü; vitamin D verilen ve verilmeyen hastaların prognoz açısından karşılaştırılması amaçlanmıştır. Gereç ve Yöntem: Kasım 2014-Nisan 2015 tarihleri arası hastanemiz YBÜ 'de takip ve tedavi edilen hastaların kayıtları geriye dönük olarak incelendi. Enteral beslenen ve vitamin D desteği alan 20 hasta (Grup D), enteral beslenen ancak vitamin D desteği almayan 20 hasta (Grup K) arasında yoğun bakıma kabul sırasında (T1), 7. (T2) ve 15. (T3) günlerde ölçülen vitamin D, albümin, prealbumin, total lenfosit, CRP düzeyleri ve triceps kas kalınlığı açısından fark olup olmadığı değerlendirildi. Bulgular: İki grup demografik veriler açısından benzerdi. Grup D ile Grup K arasında T1,T2 ve T3 zamanlarında vitamin D düzeyleri, albüminprealbümin düzeyleri, enfeksiyon oranları, mekanik ventilasyon süreleri, yoğun bakım yatış günü ve mortalite açısından istatistiksel olarak anlamlı bir fark saptanmazken T1 ve T2 zamanında total lenfosit (TLS) düzeyleri (p<0,05) vitamin D grubunda istatistiksel olarak anlamlı yüksek idi. Hastaların sadece T3 zamanında vitamin D ve diğer değişkenler ele alındığında ise APACHE II değerleri, MV gün, T3 albümin, T3 prealbümin ve T3 total proteinle istatistiki olarak anlamlı ilişki gözlendi. Sonuç: Çalışmamızda vitamin D verilen grupla verilmeyen grup arasında bazı değişkenlerde anlamlı fark gözlenmesi, prospektif ve daha fazla sayıda hasta ile yapılacak çalışmalar açısından yol gösterici olacağını düşünmekteyiz.
Burns that occur in the first 2 decades of life are usually caused by preventable accidents. Scald burns are the most common in the first decade. Flame burns have high morbidity and mortality rates in infants. This case report is presented to demonstrate appropriate anesthesia management and the multidisciplinary approach required in an intensive care unit (ICU). A 35 -day-old infant was admitted to ICU with second and third degree flash burns on the face, neck, and body covering 18% total body surface area according to Lund and Browder chart. Patient also had facial and oral edema, which resulted in difficult airway (grade IV). Burns were secondary to an accident at home involving matches lit by her brother.
Çalışmanın amacı Yoğun Bakım Ünitesi (YBÜ)'sinde tekrarlayan yatışların nedenleri ve sonuçlarını analiz etmektir. Ayrıca bu hasta grubunda mortaliteyi etkileyen faktörler incelenmiştir. Gereç ve Yöntem: Temmuz 2007-Haziran 2010 tarihleri arası hastanemiz YBÜ'de takip ve tedavi edilen hastaların kayıtları geriye dönük olarak incelendi. Yeniden yatış endikasyonları, iki yatış arası zaman ve tekrar yatışların sonuçları belirlendi. Bulgular: Üç yıllık sürede 2011 hasta tedavi edilmişti. Bu hastaların 68'ine (%3,38) tekrar yatış verilmişti. Bu hastaların 33'üne (%49) aynı, 35'ine (%51) ise farklı endikasyon ile YBÜ'ne yatış verilmişti. Tekrar yatışların 10'u (%14,7) ilk 48 saatte YBÜ'ne tekrar kabul edilmiş olup bu hastaların 7'si (%70) eksitus olurken 48 saatten geç yatışlarda bu oran %30 idi. Veriler gösterdi ki; tekrar yatışta mekanik ventilasyon uygulanan 53 hastanın 12'si (%23) ilk yatışlarında ''weaning'' tamamlandıktan sonraki ilk 24 saatte taburcu edilmişlerdi. Erken tekrar yatışlarda mortalite oranı geç tekrar yatışlara göre anlamlı olarak yüksek bulundu. Sonuç: Hastanın 'weaning' tamamlandıktan sonra 24 saatten daha uzun süre YBÜ'de takip edilmesiyle tekrar yatış oranının azaltılabileceği kanaatindeyiz. Bu hastaların YBÜ çıkış kriterleri stabil oluncaya kadar 'yakın bakım üniteleri'ne çıkarılması daha iyi olacaktır. Anah tar Ke li me ler: Tekrarlayan yatış, yoğun bakım ünitesi SUMMARY Objective: The purpose of the study to analyze the reasons and outcomes of re-admission in the intensive care unit (ICU). And also, the factors, which affect mortality in this group of patients, were studied. Material and Method: The registry of patients, who were re-admitted in ICU between July 2007 and June 2010 in our hospital, were analysed retrospectively. The data about indications of re-admission, the time between two admissions and the outcomes of the second admission, have been gathered. Results: 2011 patients were treated in three years' period. Sixty-eight of them (3.38%) were re-admitted to ICU. Thirty-three of these patients (49%) were re-admitted to ICU with the former indications, 35 (51%) with a different indications. Ten (14.7%) of the patients who were discharged from ICU, readmitted in the first 48 hours. While seven of the patients (70%) who were re-admitted in the first 48 hours after discharge were died. Seventeen of patients (30%) who were readmitted after 48 hours were lost. The data revealed that, 12 (23%) of the 53 patients who were mechanically ventilated among the readmitted cases were discharged in the first 24 hours after weaning completed in their first admission. The mortality rate in the early readmitted patiens were significantly higher than in the late re-admitted ones. Conclusion: We believe that the rate of readmission can be reduced if patients keep in the ICU more than 24 hours after weaning. The better, to monitor the patients in the "close care units" before discharge, until the discharging criteria become steadier.
Bu çalışmada, yanık yoğun bakım ünitelerinde takip edilen hastaların prognozunu etkileyen faktörlerin tespit edilmesi amaçlandı. Gereç ve Yöntem: Aralık 2008 ile Kasım 2010 tarihleri arasında hastanemizin yanık yoğun bakım ünitesinde tedavi edilmiş hastalar geriye dönük olarak incelendi. Yaş, cinsiyet, yanık yüzdesi, yatış endikasyonu, yanık yoğun bakım ünitesi ve hastanede kalış süreleri, yanık yoğun bakım ünitesinden taburculuğu, yanık yoğun bakım ünitesinden çıkarılıp hastanede takip edilen hastaların taburculuğu, trakeotomi uygulaması, inotrop ilaç desteği, kabulde Glasgow koma skalası puanı, kabulde ve 24. saatte APACHE II skorları, mekanik ventilasyon süreleri, tekrar yoğun bakım ünitesinde takip ihtiyacı, kan, doku/yara, trakea ve idrar kültür sonuçları parametreleri ile mortalite oranları karşılaştırıldı. Bulgular: Yanık yoğun bakım ünitesinde ölen hastaların yaş ortalaması, taburcu olanlardan yüksek saptanmıştır (p<0,01). Cinsiyetler arasında farklılık bulunmamaktadır (p>0,05). Yanık yoğun bakım ünitesine yatış nedenleri alev yanığı (%50), elektrik yanıkları (%22), suyla haşlanmaya bağlı yanıklar (%17) ve diğerleri olarak saptandı. Yanık yoğun bakım ünitesinde mortalite oranı %44,8 olarak bulundu. Yanık yüzdesi arttıkça mortalitenin arttığı gözlendi. Yanık yüzdesi %70 ve üzeri olan hastalarda ölüm oranı %100 olarak saptandı. Yoğun bakımda ölen hastaların yanık yüzdeleri (p<0,01), yatış APACHE II skorları ve 24 saatlik APACHE II skorları (p<0,01), GKS skorları (p<0,01), taburcu olanlardan anlamlı şekilde yüksek saptandı. Ölen hastaların mekanik ventilasyon süreleri, taburcu olanlardan yüksek bulundu (p<0,01). Yanık yoğun bakım ünitesinden taburcu olan hastaların yanık yoğun bakım ünitesine tekrar yatışları da incelendi, oran %2,8 olarak bulundu. En sık ölüm nedeni sepsis ve çoklu organ yetmezliğiydi. Hastaların kan, doku/yara, trakea ve idrar kültür sonuçları ile mortalite ilişkisi incelendi. Kan, idrar ve doku/yara kültürlerinde üreme ile mortalite arasında anlamlı fark saptanmadı. Fakat ölen hastalarda, taburcu olanlara göre trakeada üreme daha fazlayken (p<0,05) kateterde üreme daha az bulundu (p<0,01). Sonuç: Çalışmamızda, yüksek yanık yüzdesinin, mekanik ventilasyon desteği süresinin, inotropik ilaç desteği ve 24 saatlik APACHE II skorunun yüksekliğinin mortaliteyi arttırdığı saptanmıştır.
