This study aimed to evaluate the prevalence of mesiobuccal 2(MB2) canals not located/treated in maxillary molars and correlated their non-treatment with the presence of periradicular lesion. The study was conducted on 180 cone beam computed tomography (CBCT) scans. The 180 examinations added up to 210 teeth analyzed (140 maxillary first molars and 70 maxillary second molars). The presence of non-located/treated MB2 canals and periapical lesions in the mesiobucal(MB) root was identified by observation of the axial and subsequently of sagittal and coronal slices. Among the 210 teeth evaluated, 91.4% (n=192) had MB2 canal, while 8.6% (n=18) did not have this canal. In the first molars with presence of MB2 (n=133), periapical lesion was observed in 85.0% (n=113). Among the second molars with presence of MB2 (n=59), periapical lesion was observed in 72.9% (n=43). The presence of periapical lesion in the MB root was related to the non-location/treatment of the MB2 canal and was higher when it was an independent canal.
Dens invaginatus (DI) é uma anomalia dentária de desenvolvimento onde há uma invaginação do órgão do esmalte na papila dentária, antes que a calcificação esteja completa. Essas anomalias são clinicamente relevantes, pois bactérias da cavidade oral podem contaminar e se propagar dentro dessas malformações, levando ao desenvolvimento de cárie precoce e, consequentemente, morte pulpar. A causa definitiva destas anomalias ainda é incerta. A frequência relatada é de 0,04-10%. E os incisivos laterais permanentes são os mais acometidos seguidos dos incisivos centrais, pré-molares, caninos e molares, em ordem decrescente. O DI geralmente é diagnosticado em uma radiografia de rotina, entretanto imagens reconstruídas de TCFC são úteis para avaliar a verdadeira natureza da relação da invaginação com o canal radicular. O objetivo deste relato de caso foi apresentar uma técnica endodôntica para tratamento de um DI comprometido. O reconhecimento das anomalias de desenvolvimento e complexidades dos canais radiculares é essencial para o sucesso a longo prazo na terapia endodôntica. A modificação do procedimento de tratamento convencional é frequentemente necessária para uma anatomia de canal incomum. Em razão disto o encaminhamento para um especialista em endodontia é sempre indicado.
Root fractures can involve the dentin, cementum, and pulp and commonly can occur as oblique fractures with varying orientations. The aim of this study was to demonstrate the maintenance of pulp health in a tooth with a fractured root without any endodontic treatment and to discuss the advantage of cone-bean computed tomography (CBCT) compared with traditional radiographs in the diagnosis of oblique root fractures. Intra-oral radiography of tooth 11 revealed a horizontal root fracture at the level of the apical third, while the sagittal CBCT slice reveals a complete fracture line running obliquely from the apical third on the buccal aspect through the cervical third on the palatal aspect. Four years follow-up, the tooth has kept its pulp vitality, no tooth discoloration or discrepancy in arch position, without endodontic treatment. This result illustrates spontaneous healing of root fracture including preservation of pulp health. In addition, it confirms the importance of exams in 3 dimensions to correctly locate the fracture and assist in the treatment decision.
Introdução: A cirurgia parendodôntica é uma alternativa ao tratamento endodôntico convencional, principalmente em casos envolvendo lesões perirradiculares associadas a processos osteolíticos, em que a terapia endodôntica convencional ou seu retratamento não obtiveram sucesso. Novos métodos terapêuticos têm ganhado espaço na clínica odontológica para esse tipo de caso, entre os quais se destaca a fibrina rica em plaquetas e leucócitos, que, ao concentrar e permitir uma liberação mais prolongada dos fatores de crescimento, promove uma modulação do processo reparador. Esse processo é baseado na otimização da regeneração tecidual através da inserção da membrana de fibrina. Objetivo: O objetivo do presente trabalho é apresentar, por meio do relato de um caso clínico, a utilização de fibrina rica em plaquetas e leucócitos em cirurgia parendodôntica, com intuito de reparo ósseo. Resultados: O acompanhamento clínico e tomográfico foi realizado após 190 dias, momento no qual foi constatada ausência de sintomatologia, bem como completo reparo da lesão e restabelecimento do osso cortical vestibular, além de ausência de lesão detectável em tomografia. Conclusão: No caso clínico relatado, houve uma completa regeneração tecidual da área envolvida na lesão.
