Our results demonstrate that the 1,064-nm long-pulsed Nd:YAG laser could be a safe and effective treatment modality in the management of patients with onychomycosis.

A high-fluence 1064-nm Q-switched Nd:YAG laser treatment of CALMs in Asian patients is a safe and effective method without side effects and recurrence.

Café-au-lait macules (CALMs) are light to dark brown macules or patches of increased melanin concentration found along the dermoepidermal junction. Although many attempts to treat CALMs using various kinds of laser/light-based devices have been reported, CALMs remain refractory thereto with high recurrence rates. In this case series, we describe four patients with idiopathic CALMs that were effectively and safely treated with a non-ablative, high-fluenced, Q-switched (QS), 1064-nm neodymium:yttrium aluminum garnet (Nd:YAG) laser. The typical laser parameters for treating CALMs, including a spot size of 7-7.5 mm, a fluence of 2.4-2.5 J/cm, and one to two passes until the appearance of mild erythema, but not petechiae, were utilized in this study over 12-24 treatment sessions at 2-week intervals. We suggest that high-fluenced QS 1064-nm Nd:YAG laser treatment can be used as an effective and alternative treatment modality for CALMs with minimal risk of side effects.

Various lasers have been used for the treatment of erythematotelangiectatic rosacea (ETR) that does not respond to systemic or topical therapy. The pulsed dye lasers (PDLs) are an effective option for ETR, and the purpuragenic fluence proved to be superior until now. Given that purpura and subsequent possible postinflammatory hyperpigmentation (PIH) are occasionally unbearable in some patients, and several studies using the low nonpurpuragenic fluence were reported. To deliver the sufficient high fluence of a PDL without generating purpura, we designed the fractionation of high fluence using five passes and longer pulse duration (6 milliseconds) of a PDL in succession. A total of eight patients with ETR were enrolled in this study; all patients were treated with PDL 10 times at 2-week intervals. Erythema and telangiectasia scores, as well as improvement, were assessed by two physicians using the digital photographs. Moderate-to-marked improvement was achieved in most of the patients, and erythema and telangiectasia scores were significantly decreased. Purpura and PIH were not reported in all patients. The fractionation of high-fluence, long-pulsed 595 nm PDL is a very safe and effective treatment for ETR.

Hori's nevus is an acquired benign dermal melanocytosis as a small macular hyperpigmentation on the face, especially the malar areas and the both sides of the forehead. The color of lesion is mostly blotchy gray to blue. Treatment is not necessary for Hori's nevus except cosmetic concerns. However treating the Hori's nevus without side effects such as purpura, crust, postinflammatory hyperpigmentation and scarring is extremely difficult. Therefore, the authors introduce a new combination treatment using Dr. Hoon Hur's Golden Parameter and Dr. Hoon Hur's Optimal Suicide-2 Parameter with a high fluence 1064nm Q-switched Nd:YAG laser that can effectively treat Hori's nevus without side effects and recurrences.

Becker's nevus, a cutaneous hamartoma, usually occurs as a large, unilateral, hyperpigmented and hypertrichotic patch on the shoulder or upper trunk, which can be present in childhood or adolescence. Becker's nevus can be treated with traditional laser therapy using various types of lasers including a 532nm potassium titanyl phosphate (KTP) laser, 694nm ruby laser and 755nm alexandrite laser. However, this may cause harmful side effects such as scarring, mottled hypopigmentation, post-inflammatory hyperpigmentation (PIH), purpurae, and crusts. As a result, no standard for the treatment of Becker's nevus using lasers has been established. Therefore, this study was implemented to investigate the safety and efficacy of treating Becker's nevus using Dr. Hoon Hur's Golden Parameter Therapy (GPT) followed by Dr. Hoon Hur's Optimal Melanocytic Suicide-1 Parameter Therapy (OMS-1 PT) which uses a high fluence 1064nm Q-switched Nd: YAG laser (QSNL). Forty-two Koreans suffering from Becker's nevus participated in the study and received treatment on a weekly basis for 75-100 sessions using the QSNL according to Dr. Hoon Hur's GPT then followed by Dr. Hoon Hur's OMS-1 PT. The parameters for this study were a spot size of 7 mm, a fluence of 2.2 J/cm 2 and a pulse rate of 10 Hz using a sliding-stacking technique for a single pass over the Becker's nevus, followed by Dr. Hoon Hur's OMS-1 PT using a sliding technique for 5 passes with the QSNL over the Becker's nevus. Upon completion of the last treatment, all 42 patients with Becker's nevus were cured, having no side effects, and entirely removing the pigmented lesions. None of the 42 patients reported any recurrences after their follow-ups 6-15 months after the final treatment. As a result of this study, we propose therapy to safely and effectively treat Becker's nevus using a fusion of Dr. Hoon Hur's GPT and Dr. Hoon Hur's OMS-1 PT with a high fluence 1064nm QSNL to avoid any recurrences or harmful side effects.
