RESUMOO envelhecimento é um processo natural e gradativo que gera limitações morfológicas, funcionais e bioquímicas no organismo, tornando-o vulnerável ao desenvolvimento de doenças. O idoso apresenta diversas necessidades cirúrgicas, contudo, a presença de alterações sistêmicas, deficiências nutricionais e idade torna o caso especial, necessitando abordagem integral, personalizada e multiprofissional. Diante da variabilidade de doenças e individualidade que o idoso apresenta, o presente estudo tem por objetivo relatar um caso clínico que se fez necessário integrações terapêuticas na cirurgia oral de uma paciente idosa com doenças sistêmicas. Paciente, gênero feminino, 74 anos, hipertensa e diabética, foi encaminhada pelo cardiologista ao ambulatório de Cirurgia Oral da Faculdade Adventista da Bahia, queixando-se de dor em região posterior de mandíbula. Ao exame físico intraoral, notou-se: prótese total superior e inferior, dente 48 semi incluso, cariado e trauma entre mucosa pericoronária e prótese. Na radiografia panorâmica foi observado sinais sugestivos de proximidade do dente com o canal mandibular e base da mandíbula, indicando risco de parestesia e fratura em tentativa de exodontia. Solicitou-se tomografia computadorizada, que confirmou a intima relação. Foi adotado como conduta a coronectomia, sob anestesia local, aplicação de laser vermelho 2J/cm2, em quatro pontos, por 30 segundos no pós operatório imediato seguido de orientação dietética e indicação de fisioterapia. O acompanhamento clínico radiográfico, constatou-se boa cicatrização tecidual e reparo ósseo adequado, evidenciando eficácia da terapêutica aplicada.Palavras-chave: Coronectomia, nervo alveolar inferior, terceiro molar.

A atresia maxilar é caracterizada pelas irregularidades nas dimensões verticais do paciente, sendo essa a principal causa das mordidas cruzadas posteriores unilaterais e bilaterais. O tratamento de crianças e adolescentes consiste na Expansão Rápida de Maxila (ERM), enquanto os pacientes que atingiram a maturidade óssea exigem um tratamento ortocirúrgico, por meio da Expansão Rápida de Maxila Cirurgicamente Assistida (ERMCA). A ERMCA fundamenta-se na combinação de tratamentos cirúrgicos e ortodônticos, no qual a etapa cirúrgica compreende osteotomias que visam anular a resistência das articulações craniofaciais, as mesmas podem ser realizadas com brocas, serras e por um dispositivo elétrico denominado Piezo Surgery. O objetivo deste trabalho é relatar um caso clínico de expansão rápida de maxila cirurgicamente assistida, realizada por meio de acesso minimamente invasivo e auxílio da Piezo Surgery. Paciente do sexo masculino, 31 anos, encaminhado pelo ortodontista, compareceu ao serviço de Cirurgia e Traumatologia Buco-Maxilo-Facial do Hospital Otorrinos, apresentando palato ogival, atresia maxilar e mordida anterior topo a topo. Sendo assim, com base nas características clínicas e etária do paciente, o tratamento proposto foi a ERMCA associada a ativação diária do dispositivo Hyrax, seguida de acompanhamento ortodontico até a obtenção do diastema interincisal desejado. Esse procedimento demonstrou resultados satisfatórios, com maior conforto pós-operatório, descruzamento maxilar posterior e melhora na respiração, resultando no restabelecimento da oclusão, função e estética, evidenciando assim o sucesso do tratamento e da execução cirúrgica.
O trauma dentoalveolar é uma adversidade frequente para o sistema de saúde e necessita de diagnóstico e intervenção precoce. A gravidade dessas lesões pode variar de acordo com a sua classificação, de simples trincas em esmalte a avulsões do órgão dental e fraturas ósseas. Sendo assim, o objetivo deste trabalho é relatar um caso clínico de trauma dentoalveolar associado a fratura do bloco ósseo em região anterior de maxila, tratado tardiamente por meio de redução bidigital e esplintagem. Paciente do sexo masculino, vítima de acidente automobilístico, compareceu ao serviço de emergência 04 dias após o trauma. Durante o exame físico maxilofacial foi possível observar presença de mobilidade em bloco anterior de maxila e luxação lateral dos dentes anteriores. Na tomografia de face foi possível analisar a presença de linha de fratura em tábua vestibular e palatina da região anterior da maxila, assim como luxação lateral envolvendo as unidades 12, 11, 21 e 22. Apesar do longo período pós-trauma, o tratamento proposto foi a redução fechada, associada a instalação de contenção semirrígida, proporcionando estabilidade ao segmento fraturado e ambiente propício para correta cicatrização óssea. O manejo inicial do trauma dental tem grande influência no prognostico, no entanto, foi possível observar por meio desse caso, que o tratamento tardio apresenta resultados satisfatórios, auxiliando no reestabelecimento da função e estética do paciente de forma menos invasiva.
RESUMOO Granuloma central de células gigantes (GCCG) é uma lesão intraóssea, rara e de etiologia incerta, podendo estar associado ao trauma ou diferentes patologias. O GCCG é subdividido em lesões agressivas e não agressivas, onde a evolução clínica, padrão de crescimento e taxa de recidiva são alguns dos indicadores da classificação da patologia em questão. Sendo assim, ao analisar essas características, é feita a escolha do tratamento que abrange desde as técnicas cirúrgicas até aos métodos conservadores. O objetivo desse trabalho é relatar um caso clínico de GCCG em corpo de mandíbula, onde foi realizada ressecção cirúrgica com fixação rígida com placa 2.4 em base de mandíbula e enxerto ósseo sintético injetável para corrigir defeito ósseo. Paciente de gênero masculino, 43 anos, compareceu ao Hospital Otorrino em Feira de Santana-BA, queixando-se de aumento de volume facial em região mandibular esquerda, sem sintomatologia dolorosa.
