Zusammenfassung Hintergrund Seit 2015 definiert das Hospiz- und Palliativgesetz einen Anspruch auf Vorsorgeplanung in der letzten Lebensphase für Pflegeheimbewohner. Dennoch ist das Konzept Advance Care Planning (ACP) in Deutschland bislang unzureichend beforscht. Die defizitäre Evidenzlage erscheint zu diesem Thema in Verbindung mit dem Krankheitsbild Demenz noch gravierender. Die Bedeutung eines solchen Gesprächsprozesses steigt mit dem Umstand der Diagnosestellung Demenz für den Betroffenen, aber auch für seine An- und Zugehörigen und medizinisch-pflegerische Begleitung erheblich. Ziel der Arbeit Im Hinblick auf Menschen mit Demenz wurden Dokumente eines prominenten deutschen ACP-Anbieters von den Autoren als unzureichend eingeschätzt. Ziel der hier dargelegten Pilotstudie war es, einen im nationalen Kontext vielfach verwendeten Fragebogen (Werteanamnese) zu modifizieren, um ihn bei dieser vulnerablen Gruppe anwenden zu können. Material und Methoden Die Werteanamnese wurde in 11 Schritten modifiziert und im Pretest mit früherkrankten Menschen mit Demenz erprobt. Der Fokus des Dokuments liegt auf Einstellungen zu Leben, Sterben und lebensverlängernden Maßnahmen. Der Modifikationsprozess erfolgte in einem iterativen Prozess, angelehnt an die Grounded Theory nach Strauss und Corbin. Ergebnisse und Diskussion Die schrittweise Anpassung des Dokuments hat einen praktikablen und niedrigschwelligen Zugang geschaffen, der es auch der vulnerablen Gruppe der Menschen mit Demenz ermöglicht, sich mit existenziellen Fragen umfassend auseinanderzusetzen und sich darüber auszutauschen. Dieser Fragenkatalog dient als Basisdokument für die anschließende Hauptstudie – hier wird nach der Lebensbindung von Menschen mit Demenz gefragt und somit eine neue Perspektive auf ACP gelegt.
Aim Although caring relatives of people with dementia are a mainstay of many care systems, the availability of support services for them within the municipal community shows deficiencies. Adopting the Theoretical Domains Framework (TDF) this study aims to investigate 1) which of the TDF domains adapted to gerontology show up in public dialogue, and 2) the results that public dialogues produce in terms of support services for caring relatives. Subject and methods The data consists of town hall meetings and focus groups from 14 municipal communities in Germany. Participants were caring relatives and stakeholders of the communities. A qualitative content analysis was conducted, focusing on the assessment of three TDF domains, namely knowledge, goals, and sociopolitical context as well as outcomes of care optimisation. Results With regard to domain knowledge, it was evident that in every community there were actors aware of the situation and relevance of carers and their relatives. Only some actors mentioned goals for optimising the care of the target group. The sociopolitical context is often addressed through statements about incomplete requirements. Conclusion Overall, a relation between the discussion about the domains in public dialogues and changes in supporting carers of people with dementia can be assumed. The results indicate that an increased discussion about the domains within town hall meetings influences the actors and their statements with regard to the improvement of support services for caring relatives of people with dementia. Since the domains were not developed exclusively for the outlined context, this approach can also be applied to other areas of care.
Zusammenfassung. Hintergrund: Die Verfügbarkeit von Unterstützungsangeboten für pflegende Angehörige von Menschen mit Demenz in der Kommune weist durch die Corona-Pandemie Defizite auf und birgt somit eine veränderte Versorgung der Pflegenden. Fragestellung: Es stellen sich die Fragen, welche Veränderungen der Versorgung pflegender Angehöriger von Menschen mit Demenz Pflegende wahrnehmen, wie Akteur_innen aus der Kommune diese Versorgungssituation während der Pandemie erleben und inwiefern die Wahrnehmung der Gruppen übereinstimmt bzw. sich unterscheidet. Methode: Die Datengrundlage bilden Rathausgespräche und Fokusgruppen aus 13 Kommunen Deutschlands. Hieran nahmen pflegende Angehörige von Menschen mit Demenz sowie Akteur_innen aus der Kommune teil. Die Datenanalyse erfolgte mittels qualitativer Inhaltsanalyse. Ergebnis: Nachteilige Veränderungen der Versorgung Pflegender zeigen sich z.B. durch den Wegfall von Betreuungsangeboten oder Angehörigengruppen. V.a. Akteur_innen nehmen vorteilhafte Veränderungen wahr, wie eine voranschreitende Digitalisierung oder verstärkte Nachbarschaftshilfe. Die Wahrnehmung der Veränderungen der Versorgungssituation unterscheidet sich in mehreren Subkategorien zwischen Pflegenden und Akteur_innen. Schlussfolgerung: Obwohl durch die Pandemie große Versorgungslücken entstanden sind, ergeben sich durch aufkommende Herausforderungen neue Möglichkeiten der Unterstützung wie digitale Angehörigengruppen. Zukünftig müssen Möglichkeiten geschaffen werden, um bereits entwickelte Interventionen so zu implementieren, dass die Zielgruppe diese in Anspruch nehmen kann.
