Introdução: O câncer de mama é o segundo tipo de câncer mais frequente no mundo e o mais comum entre as mulheres. Atualmente, surgiu diversos estudos que mostram a metformina como uma representante de bons resultados na apoptose de células cancerígenas e na prevenção do tumor. Objetivo: Buscar enumerar os efeitos celulares da metformina e como ela pode auxiliar no tratamento do câncer de mama. Metodologia: Uma revisão sistemática da literatura do período de 01 de janeiro de 2016 até 10 de janeiro de 2018 na base de dados MEDLINE. Os descritores (MeSH) utilizados foram metformin e breastcancer. Foram encontrados 122 artigos dos quais 26 foram inclusos por estarem diretamente relacionado com o tema. Resultados: Umas das primeiras linhas de estudo mostra que a metformina causa a ativação intracelular da AMPK(proteína quinase ativada com monofosfato de adenosina). Esta ativação gera dois efeitos em cascata: a inativação de mTOR (alvo de mamífero da rapamicina) que irá culminar na supressão de RNA mensageiro e a fosforilação da p53 de células mamárias neoplásicas gerando uma apoptose ou autofagia da célula. Outra ação observada da metformina é que ela interfere no ciclo celular por meio da diminuição da ciclina D1, a qual leva a célula da fase G1 para a fase S do ciclo celular. Observou-se também uma inibição da telomerase, impedindo uma reprodução sem controle do tumor. A metformina mostrou-se pró-apoptótica para células tumorais da mama, pois aumenta os níveis celulares de Bax e diminui os níveis de Bcl-2, além de aumentar a concentração intracelular de cálcio em alguns tipos celulares, que irá promover uma turgência da mitocôndria e ativação da apoptose. Outro ponto é que a ação observada no tratamento da diabetes mellitus tipo 2 de reduzir a resistência do organismo à insulina contribui para a prevenção da carcinogênese, dado que ao diminuir a concentração de glicose no sangue, reduz a nutrição das células tumorais. Por fim, estudos clínicos mostraram uma melhora no efeito de quimioterápicos quando associado com metformina, abrindo espaço para sua redução com a preservação dos resultados obtidos. Conclusão: Diversos artigos mostraram os efeitos antineoplásicos da metformina de forma concreta, no entanto ainda faltam estudos clínicos para determinar a real eficácia e a dose necessária para se ter o efeito desejado. Além disso, o sinergismo com alguns quimioterápicos mostra uma boa vertente a ser explorada.Palavras-chave: metformina e câncer de mama. __________________ ¹ Acadêmico do curso
Introdução: Osteossarcoma é um tumor ósseo maligno corriqueiro no estirão puberal, pois há intensa divisão dos osteoblastos. É pouco prevalente, agressivo, com grande mortalidade em pacientes metastáticos e resistentes a ação dos quimioterápicos. Segundo estudos, sua malignidade se deve às CSC (células tronco de câncer), que são refratárias aos tratamentos e responsável pela reincidia agressiva da doença. Consequentemente, cerca de 35-45% dos pacientes desenvolve resistência ao tratamento. Atualmente, estudos mostram que a Metformina, fármaco usado no tratamento de diabete mellitus tipo 2, pode prevenir e tratar o osteossarcoma apresentando ações deletérias nas CSC. Objetivo: Enumerar as ações moleculares e celulares da metformina na inibição das CSC e sua ação preventiva no osteossarcoma. Metodologia: Realizou-se revisão sistemática da literatura do período de 1 de janeiro de 2008 até 7 de janeiro de 2018 na base de dado MEDLINE. Os descritores (MeSH) utilizados foram metformin e osteosarcoma. Escontrou-se 13 artigos dos quais 7 foram inclusos por correlacionar-se com o tema. Resultados: A metformina mostrou ser promissora combinada a outros fármacos para uma ação antineoplásica. Metformina associada com dexorrubicina, droga que inibe a topoisomerase aumentando radicais livres, intensifica a ação citotóxica deste fármaco e apoptose celular. Combinada com cisplatina, fármaco que se liga ao DNA e promovendo sua deformação pela formação de ligações intracadeias, a metformina desregula o balanço energético da CSC, causando apoptose por ativação de caspase 3 e 9. Já com tricostatina A, antifúngico, bloqueia o ciclo celular das CSC na fase G2, induzindo a apoptose. Além desses efeitos combinados, a metformina ativa AMPK(proteína quinase),pois bloqueia o complexo I da fosforilação oxidativa mitocondrial, desregulando a concentração intracelular de AMP/ATP que ativa a enzima. Também promove inativação do mTOR (alvo de mamífero da rapamicina), que inibirá o RNAm, a síntese proteica e a proliferação celular. Por fim, também foi possível observar que a metformina causa apoptose das células porque modula concentrações de bcl-2 e bax, liberação de citocromo C e ativação de caspase-3. Conclusão: Artigos demonstram efeitos antineoplásicos da metformina, porém é necessário comprovar sua eficácia e dose terapêutica relevante. Além disso, é preciso observar os efeitos da metformina com outros quimioterápicos, pois o tratamento do câncer é feito com fármacos conjuntamente.
