Objective: Mental disorders and early trauma are highly prevalent in female inmates. Brain-derived neurotrophic factor (BDNF) plays an important role in learning, memory processes, and mood regulation. The aim of this study was to evaluate the relationship between serum BDNF levels and mental disorders among imprisoned women as compared with age-and education-matched controls. Methods: A consecutively recruited sample of 18 female prisoners with mental disorders was assessed for sociodemographic, criminal, and clinical variables using standardized instruments, the Mini International Neuropsychiatric Interview Plus (MINI Plus), and serum BDNF levels. Results: High rates of childhood sexual abuse and posttraumatic stress disorder (PTSD) were found in the group of forensic patients. Serum BDNF levels in the forensic group did not differ from those of healthy controls, and were significantly higher when compared with those of women with mental disorders hospitalized in a general hospital. Conclusion: Elevated serum BDNF levels were found in imprisoned women. The results of this study may suggest neurobiological mechanisms similar to those seen in previous clinical and preclinical studies showing the involvement of BDNF in the pathophysiology of PTSD.

ResumoObjetivo: Examinar o fenômeno clínico de homicídio seguido de suicídio (HS) a partir de um caso no qual o desfecho de suicídio não ocorreu por motivos alheios à vontade e às ações do sujeito. Descrição do caso: Trata-se do caso de um homem que matou sua namorada por ciúmes, por ocasião do término do relacionamento entre ambos, tentando, a seguir, o suicídio. No presente artigo, é dada especial ênfase aos aspectos forenses do caso, avaliado para fi ns de determinação de responsabilidade penal e acompanhado no Instituto Psiquiátrico Forense Dr. Maurício Cardoso, Porto Alegre (RS). Comentários: Ressalta-se que, ao se modifi car o desfecho de um caso típico de HS, por não ter se consumado o suicídio do homicida, dois aspectos assumem relevância: as consequências legais a serem enfrentadas pelo sujeito e o manejo do impacto psíquico no sobrevivente. Do ponto de vista da prevenção, o conhecimento do fenômeno HS e dos fatores descritos como associados a ele justifi caria uma constante atenção ao tema por parte dos profi ssionais de saúde mental. Case description: A man killed his girlfriend at the end of the relationship because of jealousy. After that, he attempted suicide. Special emphasis is given to the forensic aspects of the case, which has been assessed to determinate the individual's criminal responsibility and has been followed up at the Forensic Psychiatric Institute Dr. Maurício Cardoso, in Porto Alegre, state of Rio Grande do Sul, Brazil. A causalidade da psicopatia reside em uma interação ainda mal compreendida entre fatores genéticos, biológicos, ambientais, sociais e psicodinâmicos. O clássico modelo biopsicossocial hipotetiza que a psicopatia se desenvolve quando há componentes genéticos e neurobiológicos associados a traços de personalidade como impulsividade, com aumento de risco quando esses indivíduos são expostos a uma família disfuncional e agravamento quando o ambiente social do entorno falha na proteção básica. No entanto, outros modelos vêm sendo estudados. Costuma-se categorizar estrutura e funcionamento cerebral, neurotransmissores e hormônios como fatores de risco biológicos; no entanto, a maneira como se manifestam e alterações nessas estruturas podem ter origem tanto genética quanto ambiental, ou refletir uma interação entre ambos, o que é denominado de interação biossocial. Atuando na expressão gênica, fatores ambientais podem modificar o cérebro, alterando os traços psicopáticos e aumentando o risco para a patologia ou, ao contrário (de maneira positiva), protegendo o indivíduo. Com relação às influências ambientais, disfunção familiar é o fator psicológico mais importante, abrangendo comportamento antissocial ou alcoolismo paternos, falta de limites e supervisão enquanto criança e separação ou perda de um dos pais. Trauma infantil pode ter um impacto dramático na saúde mental da criança, estando também relacionado à psicopatia. Por fim, questões éticas são discutidas: A presença da psicopatia altera a responsabilidade penal desses indivíduos? Se sim, de que forma? O lugar do psicopata, após um crime cometido, é a prisão ou o hospital forense? É correto submeter psicopatas a pesquisas e investigações científicas? Essas e outras questões são abordadas à luz dos conhecimentos e controvérsias atuais.
Brazil is a federation that possesses national laws, which are hierarchically superior, and state laws. Only legally sane people can be found guilty. The concept of criminal insanity requires the accused to present with some mental disorder at the time of the crime such that he or she would not be able to understand that the action was wrong or would be incapable of behaving according to his or her understanding. In Brazil, incarcerated people present high levels of psychopathology. The number of mentally ill prisoners has been growing due to the stressors and poor prison conditions and to changes in psychiatric service models resulting in a consequent reduction of psychiatric beds in the community. Attention given to the mental health of prisoners is very inconsistent and presents huge regional differences. The penitentiary conditions in Brazil are precarious and need to be improved.
