Alain Teyresite INTRODUÇÃO "Existe um meio de libertar os homens da maldição da guerra?", Einstein surpreende Freud na famosa troca de correspondência entre os dois gênios. A resposta é rápida: "em princípio, os conflitos de interesse entre os homens são solucionados mediante o uso da força". Explica Freud que a evolução tecnológica e intelectual pode e, muitas vezes, está a serviço de estimular o poder pelas armas ou pelo conhecimento. O objetivo seguiria sendo o mesmo, aniquilar o outro. O respeito ao inimigo vem da necessidade de utilizar a vítima para seus propósitos, bastando mantê-la subjugada e atemorizada. Essas são reflexões que seguem atuais, escritas sob a égide da Segunda Guerra Mundial. Partindo do "filicídio institucionalizado" representado pelos conflitos mundiais, conclui-se que as aflições dos grandes mestres sobre as tendências autodestrutivas da humanidade resistem, apesar dos reiterados esforços da sociedade em explicá-las e evitá-las.Partindo da experiência em estudar e acompanhar vítimas e perpetradores de violên-cia, na experiência diária como psiquiatras forenses, membros do judiciário, médicos que atendem a realidade dos ambulatórios e dos centros de triagem, estudiosos de urbanismo e outros determinantes sociais, os autores agruparam suas experiências e conhecimentos para refletir e propor alternativas para lidar com um dos ângulos mais cruéis do crime, aquele que atinge os mais fracos por limitações físicas, emocionais ou sociais. Foi realizada pesquisa bibliográfica recente, ampla e atual, buscando contemplar os diferentes vértices do tema. O enfoque multidisciplinar é, já, uma das propostas. A b s t r a c t Objective: This article describes two complex and doubly preoccupying situations in terms of public health, either, for its high prevalence and/or its consequences. These problems are the psychoactive substance use disorder and crime. The form will be told as diverse researchers associate them, as well as the situation of the exam of users and alcohol dependents and drugs. Method: A revision of publications was become fulfilled on the subject, using Medline and Lilacs as data bases, covering the period of 1986 the 2006. The keywords used had been: "Alcoholism", "drug dependence", "drug abuses" and "crime". Summaries of congresses, articles and excellent books on the subject, published for different authorities in the subject, in diverse phases of research, had been consulted and enclosed. Conclusion: The diverse research coincide in the affirmation of an association between psychoactive substances use disorders and crime. What it is possible to evidence is the high ratio of violent acts when the alcohol or the illicit drugs is used by aggressors, its victims or in both. When it carries through an expert examination in authors who allege some relation of the practiced act with alcohol consumption/drugs, this exam must take in consideration the substance in use, the clinical symptom for caused it, as well as verifying the presence of a diagnosis, the existence of causal nexus and possible alterations in the understanding capacity and/or determination of the agent.Descriptors: Alcoholism; Illicit drugs; Substance-related disorders; Crime; Forensic Psychiatry Resumo Objetivo: Este artigo descreve duas situações complexas e duplamente preocupantes em termos de saúde pública, seja, por sua alta prevalência e/ou por suas conseqüências. Trata-se dos transtornos do uso de substâncias psicoativas e a criminalidade. Será relatada a forma como diversos pesquisadores as associam, bem como a situação das perícias de usuários e dependentes de álcool e drogas. Método: Realizou-se uma revisão das publicações sobre o tema, utilizando-se, como bancos de dados, o Medline e o Lilacs, cobrindo o período de 1986 a 2006. Os descritores usados foram: "alcoholism", "drug dependence", "drug abuse" e "crime". Resumos de congressos, artigos e livros relevantes sobre o tema, publicados por diferentes autoridades no assunto, em diversas fases de pesquisa, foram consultados e incluídos. Conclusão: As diversas pesquisas coincidem na afirmação de uma associação entre transtornos do uso de substâncias psicoativas e criminalidade. O que é possível constatar é a alta proporção de atos violentos quando o álcool ou as drogas ilícitas estão presentes entre agressores, suas vítimas ou em ambos. Objectives To assess the sociodemographic, psychiatric and criminal profile of adolescent offenders complying with temporary custody for homicide/homicide attempt and to compare it to that of the population of adolescents in custody for other crimes. Methods This cross-sectional study was based on the review of the medical records of 74 juvenile offenders in temporary custody at socioeducational agency Fundação de Atendimento Sócio-Educativo do Rio Grande do Sul. For the analysis, variables that presented p < 0.2 were included in multivariate adjustment through logistic regression. Results The sample comprised males only, mostly with white skin color (55.6 vs. 57.9% for homicidal and non-homicidal, respectively) and with a high prevalence of school failure (77.8 vs. 91.2%). There was a high prevalence of family history of delinquency (88 vs. 81%). Only years of study and belonging or not to a criminal organization remained statistically significant in the multivariate model. Conclusion The results show that having fewer years of study and denying belonging to a criminal organization are predictive factors of homicidal behavior in adolescent offenders (both with statistical relevance). The other variables were not statistically significant for this outcome. The present study may serve as a basis for further research, which may improve our understanding of risk factors for juvenile homicide.
Objective: This preliminary study aimed to identify and compare characteristics related to violent behavior in inpatients with schizophrenia at a general psychiatric hospital using the Historical, Clinical, and Risk Management 20 (HCR-20), the Modified Overt Aggression Scale (MOAS), and sociodemographic data. Method: Violent and nonviolent participants were selected based on psychiatric admission reports. Participants with reports of aggressive behavior and HCR-20 total score ≥ 21 upon admission were assigned to the violent patient group. Participants without aggressive behavior and with HCR-20 total score < 21 upon admission were assigned to the nonviolent patient group. The MOAS was applied to characterize the degree of severity of the violent behavior. Results: HCR-20 and its subscales were effective in differentiating between the violent and nonviolent participant groups. Twelve of the 20 HCR-20 items were useful for distinguishing between the groups, although total HCR-20 scores were more reliable when applied to the nonviolent patient group. The MOAS did not show high degrees of severity for the types of aggression observed in the participants. Conclusion: HCR-20 was useful and reliable for distinguishing between violent and nonviolent patients with schizophrenia in this clinical psychiatric setting. Item analysis identified the most relevant characteristics in each group. The use of the HCR-20 in clinical psychiatric settings should be encouraged.
Neonaticide is an infant murder occurring on the day of birth. The case reports found in the literature are often focused on the mother as the agent in the context of pregnancy denial, dissociative symptoms, or psychosis. However, this report describes a rare case of attempted serial neonaticides, in which the acts were committed by a nurse at the nursery of a referral hospital in Brazil. The authors describe a forensic psychiatric evaluation for criminal responsibility and correlate the information from this particular case with relevant forensic themes, namely neonaticide, Munchausen by proxy syndrome, and serial healthcare killers.
A violência é globalmente reconhecida como um problema de saúde pública. Em 2002, a Organização Mundial da Saúde (OMS) deu destaque para o abuso e para a negligência por seu caráter crônico e pelos danos psicológicos, sociais e injúria física que geram, sendo as crianças frequentemente vitimizadas. Este artigo apresenta os diferentes tipos de maus-tratos infantis, com enfoque principal na síndrome de Munchausen por procuração, visto apresentar a característica peculiar de envolver três entidades, mesmo que de forma inadvertida: o médico, o cuidador/perpetrador e a criança/vítima.
