Les auteurs rapportent une série de 90 patients, présentant une section tendineuse des fléchisseurs de la main, et suivis avec un recul moyen de 8 mois (min: 2 mois; max: 13 mois). La lésion était localisée dans 12% des cas en zone I, 46% des cas en zone II, 2% en zone III et 25% des cas en zone IV et V. Pour le pouce (13 patients), 10 cas en zone T2 et 3 cas en zone T3. La technique opératoire utilisée était les sutures tendineuses en cadre de Kessler modifié, associée à un surjet épitendineux. Nous avons obtenus 54% de très bons résultats, 34% de résultats moyens et 12% de mauvais résultats. Pour le pouce les résultats semble moins bons avec un taux de résultats médiocre de 48%. Certes les chiffres de cette série sont moins bons que ceux des autres séries publiées dans la littérature. Les facteurs influençant les résultats sont d'abord l'utilisation d'immobilisation postopératoire systématique ainsi que le mécanisme d'agression, et la localisation à la zone II et au pouce. Les complications mécaniques sont représentées par 7% de rupture, toutes au niveau de pouce, et 31% des adhérences tendineuses (soit 30 cas), dont 19 en zone II, l'infections (22%) et 4% des cas d'algodystrophies.
This study report the results in 28 patients affected by closed fractures of the neck of the fifth metacarpal bone (boxer's fracture), treated with percutaneous elastic intramedullary nailing using a single wire, to verify the effectiveness of this surgical treatment. We reviewed the results of 28 patients treated with A single Kirschner wire (K-wire) pre-bent in a lazy-S fashion with a mild bend at approximately 5 millimeters, The K-wire is inserted blunt end first in an antegrade manner and the fracture reduced as the wire is passed across the fracture site The wire is usually removed with pliers post-operatively at four weeks in the fracture clinic. The follow-up period averaged of 20,75 months. The parameters evaluated included angulation, rotational alignment, postoperative metacarpophalangeal (MCP) range of motion, and time to union. We opted for this treatment in all cases, regardless volar angulation of the metacarpal head, malrotation of the fifth finger and associated or/no with a severe swelling of the hand. All the patients were reviewed clinically and radiologically at an average of 20,75 months after surgery. At the final follow-up, no patient reported residual pain and All fractures proceeded to bony union but we have one fracture had to be revised for failed fixation and three superficial wound infections needed antibiotic treatment. We recommend that this minimally invasive: percutaneous intramedullary nailing using a single k-wire in all metacarpal neck fracture( boxers’ fractures), especially when severe swelling of the hand is present, with good functional results and low morbidity.
Femoral fractures in amputation stump are challenging injuries to manage. The authors describe a case of a 51-year-old patient with a right above knee amputation, who had a right hip femoral neck fracture. In this technical note, we describe a technical and surgical procedure with intraoperative tips and tricks, in which we use commonly available materials, for the safe management in such clinical situations.
DeQuervain's stenosing tenovaginitis is a common condition. Nonsurgical treatment by corticosteroid injection has significantly improved the management of this disease. The authors describe a case of subcutaneous rupture of the extensor pollicis longus tendon at the wrist, three months after two corticosteroid injections for DeQuervain's stenosing tenovaginitis. The etiological history has not found any trauma history of the wrist. The aim of our work is to draw attention to this rare complication and discuss its therapeutic management. Our functional results were excellent.
IntroductionTotal subtalar dislocation without fracture of the ankle is a rare clinical entity; it is usually due to a traumatic high-energy mechanism. Standard treatment is successful closed reduction under general anesthesia followed by non-weight bearing and ankle immobilization with a below-knee cast for 6 weeks.Case presentationWe present the case of a 30-year-old Moroccan woman who was involved in a road traffic accident. She subsequently received a radiological assessment that objectified a total subtalar dislocation without fracture of her ankle. She was immediately admitted to the operating theater where an immediate reduction was performed under sedation, and immobilization in a plaster boot was adopted for 8 weeks. The management of this traumatic lesion is discussed in the light of the literature.ConclusionsMedial subtalar dislocation is a rare dislocation and is not commonly seen as a sports injury because it requires transfer of a large amount of kinetic energy. The weaker talocalcaneal and talonavicular ligaments often bear the brunt of the energy and are more commonly disrupted, compared to the relatively stronger calcaneonavicular ligament. Urgent reduction is important, and closed reduction under general anesthesia is usually successful, often facilitated by keeping the knee in flexion to relax the gastrocnemius muscle. Long-term sequelae include talar avascular necrosis and osteochondral fracture, as well as chronic instability and pain.
Aseptic nonunion of the radius and ulna is a major complication of forearm fractures, accounting for 2% to 10% of all forearm fractures. The aim of our study is to evaluate the functional and radiological results of surgical treatment of diaphyseal aseptic nonunion of the radius and ulna, with autologous bone grafting, decortication and internal plate fixation. A series of 21 patients (26 nonunions) was retrospectively reviewed, the average age was 35 years with a mean of 31,58 years (range 12-44 years). The fractures included isolated radius (n=6) and ulna (n=10), and both radius and ulna (n=5). The Grace and Eversmann score was used to evaluate our results. Fifteen had very good results, five good and one average. Consolidation of the two bones was attained in 6.2 months. Therefore, the functional prognosis of the upper limb imposes the need for an adequate treatment. This management strategy has enabled us to have satisfactory results. However, the best treatment of nonunion remains the preventive treatment with an optimal management and care of the forearm fractures.
Omarthrose bilatérale de l’épaule sur ostéonécrose est une pathologie rarement traitée dans la littérature. La présence de cette pathologie chez deux sœurs gémellaires n'est jamais décrite. Sa prise en charge chirurgicale représente un défi et nécessite un chirurgien orthopédique expérimenté en chirurgie du membre supérieur. Nous présentent deux sœurs gémellaires qui avaient une omarthrose bilatérale de l’épaule sur ostéonécrose, qui ont été traités dans notre département par prothèse inversée. La planification préopératoire a été réalisée par une équipe de chirurgiennes entrainées. Globalement nos résultats (cliniques et radiologiques) immédiats, moyen terme et au dernier recul étaient satisfaisants. Le but de notre travail est d'attirer l'attention sur cette association rare et de discuter sa prise en charge thérapeutique.
