OBJECTIVES:Non-alcoholic fatty liver disease is one of the most prevalent liver diseases and increases the risk of fibrosis and cirrhosis. Current standard treatment focuses on lifestyle interventions. The primary aim of this study was to assess the effects of a short-term low-calorie diet on hepatic steatosis, using the controlled attenuation parameter (CAP) as quantitative tool.METHODS:In this prospective observational study, 60 patients with hepatic steatosis were monitored during a hypocaloric high-fiber, high-protein diet containing 1,000 kcal/day. At baseline and after 14 days, we measured hepatic fat contents using CAP during transient elastography, body composition with bioelectrical impedance analysis, and serum liver function tests and lipid profiles using standard clinical–chemical assays.RESULTS:The median age was 56 years (25–78 years); 51.7% were women and median body mass index was 31.9 kg/m2 (22.4–44.8 kg/m2). After 14 days, a significant CAP reduction (14.0% P<0.001) was observed from 295 dB/m (216–400 dB/m) to 266 dB/m (100–353 dB/m). In parallel, body weight decreased by 4.6% (P<0.001), of which 61.9% was body fat. In addition, liver stiffness (P=0.002), γ-GT activities, and serum lipid concentrations decreased (all P<0.001).CONCLUSIONS:This study shows for the first time that non-invasive elastography can be used to monitor rapid effects of dietary treatment for hepatic steatosis. CAP improvements occur after only 14 days on short-term low-calorie diet, together with reductions of body composition parameters, serum lipids, and liver enzymes, pointing to the dynamics of hepatic lipid turnover.
Zusammenfassung Hintergrund Übergewicht und Adipositas bestimmen zunehmend unseren Alltag. In Deutschland ist mehr als jeder Zweite übergewichtig, fast jeder Vierte adipös. Normalgewicht weist weniger als die Hälfte der Bevölkerung auf. Adipositas nimmt insbesondere in den jungen Altersklassen zu, sodass immer mehr eine adipöse Gesellschaft heranwächst. Dies stellt unsere Gesellschaft, den Arbeitsmarkt sowie das Gesundheits- und Sozialsystem vor neue Herausforderungen. Um diesen Entwicklungen nachhaltig begegnen zu können, sind alltagstaugliche Maßnahmen notwendig. Material und Methodik 5-Jahres-Ergebnisse einer multizentrischen Erhebung von 135 adipösen Teilnehmern (46 Männer [34,1 %], 89 Frauen [65,9 %]) an einem 12-wöchigen, ambulanten, ärztlich betreuten Adipositastherapieprogramm. Anschließend besteht die Möglichkeit zur Teilnahme an einer Langzeitbetreuung. Die Kombination von Ernährungsumstellung, Verhaltensänderung und Steigerung der Bewegungsaktivität bilden die Programmschwerpunkte. Vorgestellt werden die Veränderungen von Körpergewicht, BMI und Körperzusammensetzung. Ergebnisse Alle Gruppen (Gesamtgruppe, Männer, Frauen) erzielten eine kontinuierliche Gewichtsabnahme über mindestens 6 Monate – und damit über den 12-wöchigen Kursverlauf hinaus. Trotz Wiederzunahmen befand sich das mittlere Körpergewicht zu jedem Messzeitpunkt in allen Gruppen unter den Ausgangswerten. Nach 5 Jahren bestand in der Gesamtgruppe noch ein mittlerer Gewichtsverlust von 3,8 kg/3,6 % (P < 0,001). 41,5 % (N = 56) der Teilnehmer hatten ihr Körpergewicht zu diesem Zeitpunkt um mind. 5 %, 22,2 % (N = 30) um mind. 10 % vermindert. Die Gewichtsabnahme erfolgte überwiegend über Körperfett, bei Steigerung der relativen Anteile der stoffwechselaktiven Masse sowie des Körperwassers. Schlussfolgerung Das vorgestellte, ambulante, ärztlich betreute Adipositastherapieprogramm bietet auch über 5 Jahre – und damit deutlich über den regulären Kurszeitraum hinaus – die Möglichkeit für eine deutliche Gewichtsabnahme, die überwiegend über Körperfett erfolgt.
Zusammenfassung ! Fragestellung: Übergewicht und Adipositas haben in Wohlstandsländern wie Deutschland seit mehr als 20 Jahren eine zunehmend hohe Prä-senz. Laut "Studie zur Gesundheit Erwachsener in Deutschland" (DEGS) sind derzeit 67,1 % der Männer und 53,0 % der Frauen übergewichtig und 23,3 bzw. 23,9 % adipös. Mit Übergewicht, insbesondere Adipositas, sind schwerwiegende Erkrankungen (wie Typ-2-Diabetes mellitus, Hypertonie) assoziiert, was die Einbindung eines Arztes in die Gewichtsreduktion empfehlenswert macht. In der vorgestellten Arbeit wird geprüft, ob mit einem ambulanten, ärztlich betreuten Ernährungskonzept neben einer effektiven Gewichtsabnahme eine Verbesserung der bestehenden Stoffwechselsituation erzielt werden kann. Methodik: 1-Jahres-Ergebnisse von 392 überge-wichtigen und adipösen Teilnehmern eines ärzt-lich betreuten, ambulanten Ernährungskonzepts, das temporär mit einem Mahlzeitenersatz arbeitet. Schwerpunkte des Programms bilden Ernäh-rungsumstellung, Verhaltensänderung sowie Steigerung der Bewegungsaktivität. Betrachtet wurden Veränderungen von Körpergewicht, BMI, Körperzusammensetzung (Körperfett, -wasser, stoffwechselaktive Masse) sowie Taillenumfang, Blutdruck (systolisch, diastolisch), Blutfette (Gesamt-, LDL-, HDL-Cholesterin, Triglyzeride), Harnstoff, Kreatinin und Harnsäure. Ergebnisse: Innerhalb eines Jahres wurden die Erfolgskriterien der nationalen Fachgesellschaften für ambulante Adipositastherapieprogramme erfüllt. Dabei wurde eine signifikante Gewichtsreduktion von im Mittel 10,2 kg erzielt. Knapp 70 % des Gewichtsverlusts war auf den Abbau von Kör-perfett zurückzuführen, während die prozentualen Anteile von Körperwasser und stoffwechselaktiver Masse gesteigert werden konnten. Die Gewichtsabnahme ging mit einer Verbesserung der Mehrzahl der erfassten Stoffwechselparameter Abstract ! ZusammenfassungDie nicht-alkoholische Fettlebererkrankung (NAFLD) wird primär mittels Kalorienrestriktion mit konsekutiver Reduktion des Körpergewichts behandelt. Ein Austausch von Stärke und Zucker gegen Protein, einfach ungesättigte und langkettige Omega-3-Fettsäuren wie auch präbiotisches β-Glucan sollen dabei unterstützend wirken.In einer randomisierten, kontrollierten Interventionsstudie wurde der Effekt zweier Diätkonzepte auf den hepatischen Lipidgehalt und biochemische Parameter an Patienten mit Body-Mass-Index (BMI) > 30 kg/m2 und Zeichen einer Fettlebererkrankung untersucht. Die Interventionsgruppe (IG, n = 17) erhielt eine kommerziell erhältliche Formula-Diät mit Haferballaststoffen, die Kontrollgruppe (KG, n = 19) wurde auf eine kalorisch vergleichbar beschränkte Kost (ca. 1000 kcal/Tag) nach der Methodik der „low glycemic and insulinemic diet“ eingestellt.Beide Diätkonzepte führten nach 12 Wochen zu einer Reduktion des BMI (IG: von 33,8 ± 2,9 nach 29,3 ± 2,5 kg/m2, KG: von 33,7 ± 2,8 nach 30,1 ± 3,2 kg/m2, beide p < 0,001) sowie zu einer Verbesserung der Leberfunktion und der metabolischen Situation. Der hepatorenale Index verringerte sich in beiden Gruppen, allerdings in der IG deutlicher als in der KG (Studienende: 1,1 ± 0,2 vs. 1,9 ± 0,3, p < 0,05). Eine Blutdruckreduktion trat nur in der IG auf (systolisch von 136 ± 15 auf 122 ± 11 mmHg, diastolisch von 89 ± 9 auf 79 ± 11 mmHg, beide p < 0,01).Zusammenfassend sind beide diätetischen Interventionen ähnlich effektiv hinsichtlich Gewichtsreduktion, aber die Intervention mit der Formula-Diät mit Haferballaststoffen war hinsichtlich der Reduktion des Leberfettgehalts und der Blutdrucksenkung effektiver als die Kontroll-Diät.
