Gereç ve Yöntemler: Metodolojik bir çalışmadır. Araştırmanın örneklemini Nisan-Haziran 2015 tarihleri arasında kadın doğum polikliniğine başvuran 200 gebe oluşturdu. Verilerin toplanmasında tanıtıcı bilgi formu ve Prenatal Emzirme Öz Yeterlilik Ölçeği kullanıldı. Verilerin çözümlenmesinde tanımlayıcı istatistikler, kapsam geçerliği için "content validity index", yapı geçerliği için açımlayıcı faktör analizi ve doğrulayıcı faktör analizi, güvenirlik için Cronbach-alfa kullanıldı. Bulgular: Ölçeğin açımlayıcı faktör analizinde Kaiser Meyer Olkin kat sayısı 0,84 ve Barlett's Sphericity Testi sonuçları χ 2 =1812,608; df=171; p=0,000'idi. Faktörlerin toplam varyansa yaptıkları katkı %59,06 idi. Doğrulayıcı faktör analizine göre, ölçeğin model uyum indeksleri; Ki-Kare (χ 2 =254,23, p=.000, sd=146) değeri başta olmak üzere uyum indeksleri, χ 2 /sd=1,74, Root Mean Square Error of Approximation=0,06, Comparative Fit Index=0,96, Normed Fit Index=0,92, Non-Normed Fit Index=0,96, Goodness Of Fit Index=0,88, Adjusted Goodness of Fit Index=0,85'idi. Prenatal Emzirme Öz Yeterlilik Ölçeğinin iç tutarlılık güvenirlik katsayısı 0,86'idi. Çıkarımlar: Prenatal Emzirme Öz Yeterlilik Ölçeği, Türk kültüründe uygulanması için geçerli ve güvenilir bir ölçektir ve prenatal dönemde emzirmeyi desteklemek için girişimler tasarlamak ve değerlendirmek isteyen tüm sağlık çalışanlarının kullanımına uygun bir araçtır.
Anemi, dünyada ve Türkiye'de sık görülen ve çocukları etkileyen önemli halk sağlığı sorunlarından biridir. Çocukluk çağında görülen en yaygın anemi nedeni ise demir eksikliğidir. Çocukluk döneminde anemi yaygınlığı gelişmiş ülkelerde %4-20 arasında iken gelişmekte olan ülkelerde bu oran %70-80'lere ulaşmaktadır. Anemi, küresel bir sorun olmakla birlikte özellikle gelişmekte olan ülkelerde son derece yaygındır. Tüm yaş grupları için yaşamsal önem taşıyan demir eksikliği anemisi, genel olarak 0-5 yaş grubu çocukların ortalama %50'inde görülür. Ölümlerin yaklaşık yarısının 5 yaş altı çocuk grubunda görülmesinin altında yatan nedenlerden birisinin anemi olması konunun önemini daha da ortaya koymaktadır. Bu önemli halk sağlığı sorunun çözümü için Sağlık Bakanlığı ülke düzeyinde; toplumun demir eksikliği konusunda bilinçlendirilmesi, 4-12 ay arası bebeklere profilaktik amaçlı ücretsiz demir desteğinin sağlanması, anemisi olan 13-24 ay arası çocuklara demir tedavisinin önerilmesi amacıyla 2009 yılında "Demir Gibi Türkiye Projesi"ni başlattı. 0-5 yaş çocuklarda ayrıntılı sağlık öyküsü ve fiziksel değerlendirme yapılarak; aneminin tipi, neden olduğu semptomlar ve bu semptomların yaşamı nasıl etkilediği konusunda önemli veriler elde edilebilir. Anemi değerlendirmesi, çocuk izlemi yapan ebelerin de önemli sorumlulukları arasında yer alır. Sağlıklı nesillerin yetiştirilmesi için anemi konusunda çocukların yeterli ebelik bakımı alması gerekmektedir. Sonuç olarak, aneminin değerlendirilmesi, önlenmesi ve tedavisinde ebelerin sorumluluk sahibi olmaları 0-5 yaş çocukların sağlığının korunması ve geliştirilmesine katkı sağlayacaktır.
Ebelik eğitimi, teorik ve klinik olmak üzere birbirini tamamlayan iki bölümden oluşmaktadır. Klinik deneyimleri boyunca yeni bir ortama giren öğrenciler anksiyete ve stres oluşturan durumlarla baş etmek zorunda kalmaktadır. Klinik uygulamalarda öğrencilerin yaşadığı stres ve anksiyetenin, öğrenilen bilgilerin uygulamaya aktarılmasında zorluğa, hata yapma korkusuna ve mesleki yetersizliğe neden olduğu belirtilmektedir. Bu nedenle eğiticilerin, öğrencilerin öğrenme süreçlerini anlaması ve pratik uygulamaları klinik ortama nasıl aktaracaklarını bilmesi çok önemlidir. Buna bağlı olarak, eğiticiler yenilikçi eğitim tekniklerini takip etmeli ve kliniğe çıkmadan önce öğrencilerin becerilerini güçlendirmelidir. Literatürde yenilikçi eğitim tekniklerinden biri olan simülasyonun, öğrencilerin öğrenme yaşantıları ve mesleki becerilerine olumlu yönde katkı sağladığı belirtilmektedir. Öğrencilere katkı sağlaması nedeniyle simülasyon destekli ebelik eğitimi modelinin kullanılması, yaygınlaştırılması ve ebelik bölümlerinin müfredatlarına dahil edilmesi önerilmektedir. Bu derlemenin amacı; ebe eğitiminde simülasyon kullanımı konusunda farkındalık oluşturmaktır.
It is desired to increase breastfeeding rates at national and international level. However, maternal smoking during the lactation period, which has negative effects on breastfeeding, is common both in our country and in Europe. Smoking during the breastfeeding period of the mother is directly related to the inhibition of the milk let-down reflex and the decrease in milk production. Maternal smoking during lactation causes changes in infants' sleep/wake patterns, deterioration of the pro-oxidant-antioxidant balance of breast milk, and low iodine-based breast milk transfer. In addition, in the babies of smoker mothers; bad taste from breast milk, restlessness, possibility of getting respiratory system infections such as pneumonia, asthma, bronchitis, risk of sudden infant death syndrome tendency to colic and obesity are more higher. Some mothers who smoke believe that it is safer to give formula than to breastfeed. In addition, there are mothers who ask questions such as whether smoking spoils the quality and taste of my milk, what can I do to make smoking less harmful to my baby, I smoke, can I breastfeed my baby in clinical practice. Medical staff Ali should be aware of the effects of nicotine exposure on infants of breastfeeding and smoking mothers during breastfeeding education and counseling, and should pay enough attention to this issue. Healthcare professionals should seize every opportunity to increase interest in breastfeeding, to raise awareness of the harm caused by cigarette exposure in the newborn, and to encourage smoking cessation.For this reason, the aim of this review is to review the literature on breastfeeding and smoking.
Although the non-invasive prenatal test has very high sensitivity and specificity as a screening test, few false positive and false negative results have been reported. Since it is performed in the early weeks of pregnancy, this test reduces the need for invasive testing, and its prevalence and scope are gradually increasing. However, the fact that this test can be performed outside of medical reasons causes important ethical problems and controversy. Consultancy and informed consent are very important for the optimum non-invasive prenatal test offered to pregnant women. Ethical issues such as avoiding harm, equality, inclusiveness, autonomy and free choice of the pregnant woman and her partner should be addressed.
Objectives This study compared effects of vitamin D, fish oil and exercise on cardiovascular parameters in ovariectomized rats. Materials and methods This is an experimental study conducted with 45 female Wistar Albino rats; consisted of one sham (n=8) and four intervention (n=37) groups. Rats (n=37) were oophorectomized and, randomly divided into four groups at the end of the first week following wound healing. Interventions were applied to the groups of oophorectomize+Dvit, oophorectomize+fish oil and oophorectomize+exercise for 12 weeks. In analyzing the data, ANOVA and Tamhane’s T2 tests were used (p<0.05). Results The levels of total-cholesterol (66.7 ± 7.6 mg/dL), HDL-cholesterol (33.8 ± 2.1 mg/dL), TOS (7.3 ± 1.2 μmol H2O2 Eq/L), TAS (4.0 ± 0.5 mmol Trolox Eq/L), OSI (0.2 ± 0.1) and total-cholesterol/HDL-cholesterol (2.0 ± 0.3) were obtained significant in exercising oophorectomized rats according to the oophorectomized rats (p<0.05). The estrogen levels of rats using vitamin D and fish oil and exercising were found to be higher than those in menopause. Conclusions It was concluded that exercising oophorectomized rats had a lower risk of cardiovascular disease. In this context, exercise/physical activity should be recommended and must be supported for practicing in order to protect the cardiovascular health of postmenopausal women.
