Esta pesquisa procurou identificar as adaptações do sistema estomatognático em indivíduos com desproporções maxilomandibulares. A revisão bibliográfica se fez por meio das bases de dados nacionais e internacionais, abordando as adaptações do sistema estomatognático em indivíduos com desproporção maxilo-mandibular submetidos a tratamento ortodôntico-cirúrgico (cirurgia ortognática) associado à intervenção fonoaudiológica. Observou-se que sujeitos com prognatismo e retrognatismo apresentam adaptações em todas as funções realizadas pelo sistema motor oral, no período pré-cirurgia ortognática. No prognatismo mandibular, a função mais adaptada é a mastigação, caracterizando-se em movimentos verticalizados com utilização do dorso da língua para amassamento do alimento e pouca utilização dos músculos mastigatórios. No retrognatismo mandibular, estudos mostram que a deglutição apresenta-se bastante adaptada, com presença de deslize mandibular anterior somado ao movimento póstero-anterior de língua e com participação da musculatura perioral. Após a cirurgia ortognática, alguns casos podem apresentar adequação das funções do sistema estomatognático, porém em outros persistem as alterações, sendo necessário o conhecimento do fonoaudiólogo sobre as adaptações pré-existentes para traçar o melhor plano de reabilitação.
Palatine torus is a benign congenital outgrowth of bone that affects the hard palate and palatine processes, resulting from the "overworking" of osteoblasts and bone deposition along the line of the palatine fusion. Surgical excision is the only treatment for torus, and such patients are susceptible to intraoperative and postoperative complications of a traumatic, functional, or infectious nature. This article describes an atypical case of torus palatinus measuring 20.31 × 27.25 × 59.20 mm, which is the largest size ever described in the literature. This case required the use of a surgical guide in the intraoperative phase, with viable use in the postoperative phase as well. This guide proved versatile in reducing the risk of undercorrection and complications, offering greater patient comfort.
Despite the low frequency of this lesion, additional studies are needed to understand the implications of myeloid metaplasia in canine mammary mixed tumors.
A sialolitíase é uma das doenças mais comuns das glândulas salivares. Os sialólitos geralmente ocorrem nos ductos submandibulares ou na glândula e são classificados como de tamanho incomum ou sialólitos “gigantes” caso qualquer dimensão exceda 15 mm. Esse trabalho tem por objetivo relatar um caso clínico de sialólito localizado em glândula submandibular, bem como discutir a literatura pertinente sobre o tema. Paciente do sexo feminino, 68 anos, compareceu serviço de CTBMF do Hospital de Trauma de Campina Grande, queixando-se de incômodo e episódios dor, vermelhidão em região submandibular esquerda e boca seca. Uma massa calcificada com medidas 16 x 10 mm foi evidenciado através de uma tomografia computadorizada, no trajeto do ducto da glândula submandibular esquerda. Foi realizado a exérese do cálculo salivar, sob anestesia geral. Após acesso e remoção do mesmo, foi feita a instalação de um jelco nº 16 para auxiliar a reepetilização do local a fim de manter a patência do ducto envolvido. Portanto, a correta avaliação tendo em vista a localização e o tamanho do sialólito, é indispensável para o plano de tratamento e a boa execução, neste caso, a excisão cirúrgica do sialólito foi a técnica considerada e resultados positivos foram obtidos.
Introduction: Atrophic mandibular fractures are injuries found in old age due to facial trauma. Its treatment is a challenge, even for experienced surgeons, due to the different approaches that can be taken from clinical decisions. Taking into consideration the limitations that the age presented by most of the affected patients brigs, there are devices that stand out by seeking benefits in terms of predictability and time gain in the intraoperative period. The additive manufacturing is a valuable resource in the surgical planning and rehabilitation of these patients. Objective: To expose a clinical case that demonstrates the success in the use of surgical planning through a prototype printed in 3D technology. Clinical case: Elderly woman, 75 years old, victim of a fall from her own height who had a bilateral atrophic mandible fracture due to facial trauma. The surgical procedure was planned using a prototype printed in 3D technology providing predictability and reduced surgical time. The patient evolved in the postoperative period without complaints, with function immediately restores and without signs of infection or plate exposure. Conclusions: Atrophic mandible fractures are still related to a challenging treatment, therefore, adequate surgical planning and the use of auxiliary strategies such as 3D prototyping provide greater predictability of results, as well as time gain during the intraoperative.
