Measurements of carbon ion ranges in various phantom materials and real bones are presented. Together with measured Hounsfield values, an empirical relation between ranges and Hounsfield units is derived, which is an important prerequisite for treatment planning in carbon ion therapy.

The introduction of dynamic intensity modulation into radiotherapy using conventional photon beams or scanning particle beams requires additional and efficient methods of dose verification. Dose measurements in dynamically generated dose distributions with a single ionization chamber require a complete application of the treatment field for each single measurement. Therefore measurements are performed by simultaneous use of multiple ionization chambers. The measurement is performed by a computer controlled system and is comprised of the following steps: (a) automated positioning of the ionization chambers, (b) measurement at these points, (c) a comparison with the calculated dose from the treatment planning system, and (d) documentation of the measurement. The ionization chambers are read out by a multichannel electrometer and are densely packed into a mounting of polymethylmetacrylate, which is attached to the arm of a three-dimensional motor-driven water phantom. The measured and planned dose values are displayed numerically as well as graphically. The mean deviation between measured and planned doses as well as their standard deviation are calculated and displayed. Through printouts complete documentation of the measurement is obtained and a quick decision can be made whether the dose distribution is acceptable for the patient. The system is now routinely used for dose verification at the heavy ion therapy project at the Gesellschaft für Schwerionenforschung in Darmstadt. Up to now 242 measurements have been performed for heavy ion treatment of 30 patients. The system allows efficient verification and documentation of carbon ion fields and is in principle also applicable to intensity-modulated photon beams.

Intensity modulated radiation therapy (IMRT) has evolved toward the use of many small radiation fields, or "beamlets," to increase the resolution of the intensity map. The size of smaller beamlets can be typically about 1-5 cm2. Therefore small ionization chambers (IC) with sensitive volumes < or = 0.1 cm3 are generally used for dose verification of IMRT treatment. The dosimetry of these narrow photon beams pertains to the so-called nonreference conditions for beam calibration. The use of ion chambers for such narrow beams remains questionable due to the lack of electron equilibrium in most of the field. The present contribution aims to estimate, by the Monte Carlo (MC) method, the total correction needed to convert the IBA-Wellhöfer NAC007 micro IC measured charge in such radiation field to the absolute dose to water. Detailed geometrical simulation of the microionization chamber was performed. The ion chamber was always positioned at a 10 cm depth in water, parallel to the beam axis. The delivered doses to air and water cavity were calculated using the CAVRZ EGSnrc user code. The 6 MV phase-spaces for Primus Clinac (Siemens) used as an input to the CAVRZnrc code were derived by BEAM/EGS4 modeling of the treatment head of the machine along with the multileaf collimator [Sánchez-Doblado et al., Phys. Med. Biol. 48, 2081-2099 (2003)] and contrasted with experimental measurements. Dose calculations were carried out for two irradiation geometries, namely, the reference 10x10 cm2 field and an irregular (approximately 2x2 cm2) IMRT beamlet. The dose measured by the ion chamber is estimated by MC simulation as a dose averaged over the air cavity inside the ion-chamber (Dair). The absorbed dose to water is derived as the dose deposited inside the same volume, in the same geometrical position, filled and surrounded by water (Dwater) in the absence of the ionization chamber. Therefore, the Dwater/Dair dose ratio is a MC direct estimation of the total correction factor needed to convert the absorbed dose in air to absorbed dose to water. The dose ratio was calculated for several chamber positions, starting from the penumbra region around the beamlet along the two diagonals crossing the radiation field. For this quantity from 0 up to a 3% difference is observed between the dose ratio values obtained within the small irregular IMRT beamlet in comparison with the dose ratio derived for the reference 10x10 cm2 field. Greater differences from the reference value up to 9% were obtained in the penumbra region of the small IMRT beamlet.

Small, circular, x-ray beams are commonly used for radiosurgery applications. Dosimetric characteristics of 4, 6, 10, 15 and 24 MV circular x-ray beams ranging in size from 10 to 40 mm are reported. These characteristics include the measurement of TMR, beam profiles and relative output factors. Measurements of these parameters were performed in a solid water phantom using film, a small diode, small parallel-plate and cylindrical ionization chambers and TLD. Comparison of relative dose measurements of small, circular beams performed using these detectors showed that the small diode, film and TLD results consistently agreed for circular beams as small as 10 mm diameter. Beam profiles were measured using film dosimetry. Comparison of TMR values of a 10 mm diameter beam measured using film and a small parallel-plate ionization chamber showed no significant differences. Tertiary collimators designed with tapered, divergence-matching holes, and straight-drilled holes have been used for radiosurgery applications. Measurement of beam penumbra produced with either of these types of tertiary collimators showed minimal differences between them.
