A persistência e a progressão desta virose estão condicionadas à sobrevivência e reprodução do seu vetor, a fêmea do mosquito Aedes aegypti, no ambiente. A dengue é uma arbovirose típica de ambientes urbanos degradados, saneamento deficiente e provimento irregular de água potável. Ela hoje ocorre periodicamente nos meses quentes do nosso país 1 , preferencialmente nos períodos de menor pluviosidade 2 .Os valores de Ro (número reprodutivo básico) para a dengue variam de 1,3 a 11,6 segundo diferentes autores em diferentes anos e lugares 3 , de acordo com os métodos utilizados para calcular Ro e as condições em que isto ocorreu [4][5][6][7] . A magnitude de Ro nos dá uma idéia da magnitude de propagação da epidemia e até que ponto podemos controlá-la. Epidemias com Ro grandes são impossíveis de serem controladas.O Ro é uma grandeza escalar que estima o número médio de infecções secundárias geradas a partir de um caso primário durante o período de transmissão da doença 8 , estando a epidemia em seu início e a população inteiramente susceptível ao patógeno infeccioso. Quando Ro > 1, ou seja, a infecção tende a se espalhar na população; e se Ro < 1, a infecção tende a declinar e pode se extinguir.As epidemias locais terminam quando a imunidade da população atinge um valor máximo (imunidade de grupo), que pode levar à eliminação do patógeno. Contudo, se a população se enquadra acima de um limiar crítico, os susceptíveis adicionados regularmente (por nascimentos, migração ou perda de imunidade) serão suficientes para manter o patógeno circulando endemicamente. Em outras palavras, o patógeno encontra-se agora em equilíbrio estacionário (ou endêmico), condição em que Ro = 1. A população torna-se um reservatório e epidemias se sucederão de tempos em tempos sempre que o número de susceptíveis atinja o limiar epidêmico.Muitas cidades brasileiras têm populações de tamanho suficiente para manter a endemicidade da dengue.Endemicidade da dengue no Brasil Como Ro é definido para uma população inteiramente susceptível, e esta condição não é mais observada no Brasil, estimamos o potencial epidêmico a partir do R (número reprodutivo efetivo), que é o valor Ro afetado pela fração de susceptíveis (x), da população, tal que R = Ro.x 9 , sendo 0 ≤ x ≤ 1. Quando uma epidemia termina, a fração de susceptíveis atinge o seu valor mínimo, x*, devido à formação de imunidade de grupo (I), tal que I = 1 -x*, e R = Ro (1 -I). Como neste caso se atingiu o equilíbrio endêmico, R = 1, então Ro (1 -I) = 1, e a imunidade de grupo será dada por 1 : I = 1 -1/Ro (1). Podemos então conhecer Ro a partir de estimativas da imunidade de grupo por sorologia ou outro método em amostras da população. Assim, se a epidemia deixa 23% da população imune (I = 0,23), obtemos Ro = 1,3; se 91% da população estiver imune (I = 0,91), Ro = 11,6. Em ambos os casos, se obtém R ≈ 1, ou seja, atinge-se o estado estacionário mas não se elimina o patógeno da população. A imunidade de grupo dura alguns anos até que a proporção de susceptíveis aumente o suficiente para uma nova epidemia.
