Neste artigo reflete-se sobre os dilemas da integração das políticas sociais no Brasil desde a inscrição da Seguridade Social na Carta Constitucional de 1988 até a tematização mais recente acerca da intersetorialidade. Constrói-se a análise, sobretudo, a partir das trajetórias das políticas de saúde e assistência social, haja vista que os legados institucionais e a história de formação dessas áreas fornecem uma base de argumentação fecunda para o estudo aqui proposto. Demonstra-se que o processo de implementação da Seguridade Social esbarra em obstáculos de ordem política e econômica que dificultam a promessa integradora proposta. Ademais, observa-se que o modelo descentralizado, sob o qual operam as políticas sociais, em especial as de saúde e assistência social, recoloca a necessidade de sinergia intersetorial para enfrentar a complexidade dos problemas sociais. Por fim, busca-se destacar os principais desafios à integração das políticas sociais, sinalizando tratar-se de tarefa nada trivial.

Programa Bolsa Família: nova institucionalidade no campo da política social brasileira?Resumo: O Programa Bolsa Família foi implantado pelo governo federal brasileiro em outubro de 2003 com a perspectiva de combater a pobreza e a fome no país e promover inovações no padrão histórico de intervenção pública na área social. Esse artigo analisa o desenho do Programa, buscando identificar possíveis inflexões nele contidas, a partir dos seguintes eixos: foco e condições de acesso, condicionalidades, descentralização, intersetorialidade e controle social. O estudo tomou por referência a legislação e documentos oficiais, apontando limites e potencialidades à sua operacionalização. Dentre as mudanças pretendidas, destaca-se o complexo esforço de ampliação da arena decisória, de forma a incorporar diferentes níveis e esferas governamentais e a sociedade civil. Por outro lado, observam-se limites dados pela restrição dos mecanismos de focalização da clientela e pela indefinição de aspectos relativos ao controle social e à gestão compartilhada. A questão das condicionalidades configura uma polêmica central, merecendo estudos mais aprofundados. The Family Grant Program: New Institutionalism in Brazilian Social PolicyAbstract: The Family Grant Program was established by the Brazilian federal government in October 2003 to fight poverty and hunger and promote innovations in government social intervention. This article analyzes the program's design and tries to identify possible inflections contained within it, based on the following focus: conditions of access, conditionalities, decentralization, intersectoral factors and social control. The study analyzed official documents and legislation, and indicates the limits and potential for their operationalization. The changes planned are highlighted by the complex effort to expand the decision-making arena, in order to incorporate different government levels and spheres as well as civil society. Limitations were noted that were created by the restrictive mechanisms for the focalization of clientele and by the lack of definition of factors related to social control and shared management. The question of conditionalities is a central problem that deserves greater study.
Este artigo tem por objetivo analisar como os governos locais vêm, no contexto da descentralização do Estado brasileiro, respondendo às novas exigências relativas à implementação e gestão das políticas sociais. Destaca-se o caso da política de saúde, cuja centralidade da esfera local se consolida pari passu ao aprofundamento da reforma setorial em curso a partir dos anos 90. O texto problematiza a capacidade efetiva de os municípios brasileiros exercerem, de fato, a sua autonomia para elaborar e executar políticas, vis-à-vis à tradição política local brasileira e as desigualdades econômicas, sociais e regionais características do país. Tomamos o município de São Gonçalo, na região metropolitana do Rio de Janeiro, como lócus de análise, buscando dimensionar o impacto da descentralização na reorganização da rede de serviços, a partir da composição dos serviços em termos da quantidade e qualidade da oferta de cuidados, bem como do padrão de referência e contra-referência entre os níveis de assistência do sistema local de saúde. (Observa-se que a produção da política de saúde nessa localidade esbarra em constrangimentos caracterizados pela interconexão entre elementos intrínsecos ao próprio ordenamento do sistema local de saúde e a maneira como se conforma o poder político no município).
O artigo discute os aspectos teóricos que envolvem o tema política social e intersetorialidade trazendo à cena, por meio de um estudo empírico realizado no município de Niterói-RJ, os principais desafios práticos relativos à implementação da intersetorialidade no nível local. Nesta pesquisa qualitativa, adota-se a metodologia de estudo de caso, com realização de observação participante e entrevistas com atores locais. Os resultados revelam que o município de Niterói, por meio de profissionais das Secretarias de Educação, Assistência Social e Saúde, vem desenvolvendo ações intersetoriais como forma de enfrentamento da pobreza e dos limites da lógica setorial. Entretanto, a junção de elementos como o fraco apoio do Executivo local, a ausência de articulação das ações intersetoriais com os movimentos sociais e conselhos de políticas públicas e a competitividade política partidária predatória mostram o tamanho da preocupação com a sustentabilidade de experiências de articulação intersetorial.
