, Leila susana Finger (4) REsuMO Objetivo: verificar o resultado da relação s/z e do tipo de voz em pacientes com diagnóstico de disfonias orgânico-funcionais (DOF) e disfonias funcionais por uso incorreto da voz (DFUIV), bem como a ocorrência das diferentes patologias dentro das DOF. Métodos: 70 indivíduos, de ambos os sexos, entre cinco e 65 anos de idade, atendidos numa clínica-escola, cadastrados em Banco de Dados, entre 1998 e 2006, com DOF e DFUIV, ambos classificados em três subgrupos: casos em que a relação s/z indicava hipercontração, normalidade, e falta de coaptação das pregas vocais durante a fonação. Os tipos de voz foram classificados conforme a ocorrência em: sem alteração; ruidosa; ruidosa, comprimida e/ou apresentando alteração de f 0 h; ruidosa e/ou apresentando alteração de f 0 h; e comprimida. Resultados: ocorrência significativa de DOF e de DOF com nódulos vocais; nas DOF e DFUIV, ocorrência significante de relação s/z normal com tempos isolados de /s/ e /z/ abaixo do normal e voz ruidosa. Conclusões: O resultado da relação s/z foi estatisticamente significativo a favor da faixa de normalidade estabelecida, tanto nos pacientes com diagnóstico de DOF, quanto naqueles com DFUIV, sendo que, em ambos os grupos de pacientes, a voz ruidosa foi a mais freqüente. Dentro das DOF, os nódulos vocais foram significativamente mais freqüentes do que as demais patologias. A avaliação perceptivo-auditiva da voz é uma forma subjetiva de identificar alterações na qualidade vocal, baseando-se no conhecimento prévio do examinador e na sua capacidade de discriminar as diferentes características da voz normal e patoló-gica 8 . Um dos aspectos vocais perceptivo-auditivo comumente avaliado da qualidade vocal é o tipo de voz, que é diretamente relacionado à seleção dos ajustes motores empregados, tanto em nível de prega vocal quanto em nível de ressonância, permitindo verificar dados sobre a qualidade vocal, o pitch e a loudness 9,10 . Outro método clássico e habitual utilizado no auxílio do diagnóstico das disfonias é a medida do Tempo Máximo de Fonação (TMF), que utiliza, dentre outros, os fonemas fricativos /s/ e /z/ 9,11 . Tais TMF são obtidos e cronometrados durante a fonação sustentada em uma única expiração e fornecem dados sobre a dinâmica de fonação, sendo bastante fidedignos em avaliação de eficiência glótica 9,12 . Segundo a literatura 12,13 , a relação s/z permite verificar a ocorrência do componente de hipercontração muscular e do componente de falta de coaptação das pregas vocais em pacientes com diagnóstico de disfonia funcional por uso incorreto da DEsCRITOREs InTRODuçãOAs perturbações do fluxo expiratório podem ser conseqüências do uso inadequado da musculatura respiratória, que ocasiona um conflito entre a pressão subglótica e a resistência glótica. RESUMOTema: características vocais acústicas da emissão em registro basal de pacientes adultos masculinos, portadores de fissura palatina pós-forame reparada cirurgicamente. Procedimentos: estudo de casos envolvendo quatro sujeitos do sexo masculino portadores de fissura palatina pós-forame reparada cirurgicamente. Foi realizada análise acústica da emissão sustentada da vogal [a] em registro modal e basal, através dos programas Multi Dimensional Voice Program (análise da fonte glótica) e Real-Time Spectrogram (análise espectrográfica), ambos da Kay Elemetrics Corp. Resultados: por meio da análise espectrográfica do basal, observou-se: diminuição de definição, intensidade e de largura de banda de alguns formantes, diminuição da intensidade de todo o espectro, e discreta diminuição do ruído nas altas frequências; o efeito damping apresentou discreto aumento. Pela análise da fonte glótica no basal através do MDVPA, observou-se: frequência fundamental dentro da faixa do registro modal de fala e aumentada quando comparada à emissão para comparação em registro modal; grande aumento das medidas que evidenciam variação de frequência e de amplitude; maioria das medidas de jitter e de shimmer significativamente aumentadas; medidas de ruído, de quebras de voz, de segmentos não sonorizados e de tremor aumentadas; e índice de fonação suave (SPI) significativamente diminuído. Conclusão: os resultados evidenciam a alta instabilidade e ruído da emissão em registro basal, bem como sua fraca intensidade e diminuição do fluxo aéreo transglótico. Conflito de interesses: inexistente consequentes, contudo, uma das alterações que mais chama a atenção em pacientes fissurados, por afetar a comunicação, é a hipernasalidade. Esta decorre do fechamento inadequado do esfínc-ter velofaríngeo (EVF), o que acaba por provocar a falta de isolamento entre as cavidades oral e nasal 3,4 , contaminando assim os sons orais por um foco de ressonância nasal 5 , além de dificultar que o indivíduo mantenha a pressão de ar na cavidade oral para a produção da fala 6 . A voz hipernasal pode transmitir a impressão de limitações físicas e intelectuais e até mesmo alterações emocionais ao indivíduo que a apresenta 5 . Uma das formas de tratamento da hipernasalidade é através de cirurgia corretiva 7,8 . Porém, muitas vezes, mesmo com a cirurgia reparadora, alguns fatores de ordem funcional conservam-se alterados. Para esses pacientes, a reabilitação fonoaudiológica, por meio da utilização de técnicas vocais é indicada 8 . DESCRITORES: INTRODUÇÃOA fissura lábio-palatina é uma malformação congênita 1,2 caracterizada por alteração na fusão dos processos faciais embrionários responsáveis pela formação da face, podendo acometer o lábio, o rebordo alveolar, o palato duro e o palato mole 3 . OBJETIVO: descrever os tempos máximos de fonação (TMF) e características vocais acústicas de mulheres adultas jovens com nódulos vocais. MÉTODOS: coleta dos TMF das vogais /a/, /i/ e /u/; da relação s/z e medidas acústicas da fonte glótica (Multi Dimensional Voice Program Advanced da Kay PENTAX) de 20 sujeitos do sexo feminino, com faixa etária entre 20 e 40 anos, dez com laudo otorrinolaringológico de nódulos vocais e dez com laudo de laringe normal. RESULTADOS: quanto aos TMF, o grupo sem nódulos (GSN) apresentou 90% dos resultados normais e 10% diminuídos, e o grupo com nódulos (GCN) apresentou 70% dos resultados normais e 30% diminuídos; na relação s/z, todo o GSN apresentou resultados dentro dos padrões da normalidade e o GCN apresentou 70% de resultados normais e 30% aumentados; ambos os grupos apresentaram todas as medidas vocais acústicas dentro da normalidade, com exceção do GSN que apresentou vf0 aumentada. CONCLUSÃO: as mulheres jovens com nódulos vocais apresentaram medidas de TMF e de relação s/z sem diferenças significativas em relação às mesmas medidas de mulheres sem afecção laríngea. As medidas acústicas da fonte glótica foram semelhantes e dentro da normalidade para ambos os grupos, com exceção da medida do cociente de variação de frequência fundamental (vf0) que foi elevada no GSN, sugerindo incoordenação penumofonoarticulatória. As medidas de TMF e da relação s/z, mesmo sem diferenças significativas, pareceram mais sensíveis à presença do predomínio aerodinâmico dos nódulos vocais do que as medidas acústicas.
Patients with palatine fissure have inadequate velopharyngeal closure (VPC), with consequent vocal hypernasality which can be reduced by a basal tone. Aim: to compare VPC during a basal tone with the emission of a modal recording, in patients with repaired post-foramen palatine fissure. Materials and methods: case study with four adult men, all with repaired post-foramen palatine fissure. VPC images through nasal-pharyngoscopy during the emission o f the [a] vowel in a modal and basal recording. The images were studied by four ENTs. Results: in three subjects there was no change in the type of VPC considering the recordings analyzed; the changes which happened to most of the subjects are associated only to the degree of movement of the structures involved, since in the basal recording the movement of the laryngeal lateral walls was kept, the movement of the posterior pharyngeal wall stabilized, the movement of the palatine veil was mildly reduced, and the Passavant fold was evident. Conclusions: The type of VPC was kept in the four subjects analyzed, when we compared the modal and basal recordings, there was a modification in the degree of movements in the structures involved, making it clear the Passavant Fold.
