Hypertension is the number one cause of death worldwide, however, this occurrence can be prevented if proper follow-up is performed. One way to improve the accuracy of follow-ups is through tools that enable constant update and monitoring of hypertensive patients. Therefore, the objective of this study reflects on the application of a standard operating procedure that allows such actions to be performed ensuring that providing care needs to be continuous and the data has to be updated in order to have a better treatment plan for each patient enrolled in the Basic Health Unit. Thus, this article is based on an integrative review on tools for continuity of care for hypertensive patients. In this way, 95 articles were selected within the theme, and after applying the exclusion and inclusion criteria, only 20 were analyzed. The studies were published in journals with high to medium impact factors, recognizing the degree of relevance of the theme studied. One of the major issues currently existing for the effective monitoring of hypertensive patients is the lack of accurate tools that have practical application and that allow the primary care team to effectively monitor patients registered in their territory. Thus, the literature review supported this study to direct the understanding about problems inherent to hypertension in primary health care, supporting the argument and allowing new ways to monitor hypertensive patients so that future problems due to this chronic disease can be prevented.
O mapa vivo configura-se como importante ferramenta de planejamento e gestão da atenção primária. Por meio dele é possível compreender a dinâmica situacional da saúde de determinada região a fim de criar estratégias que visem à promoção, proteção e recuperação da saúde, além de fomentar subsídios para a tomada de decisão pelos gestores, profissionais e usuários do SUS. O estudo justifica-se pelo impacto desta ferramenta na gestão do trabalho assistencial em saúde, podendo ser a razão pela qual a aplicação prática dessa ferramenta possa ser descontínua. Assim, se faz necessário o levantamento de informações para investigar as causas que levam ao surgimento de lacunas na continuidade da manutenção do mapa. Foi realizada uma revisão integrativa que buscou elencar informações capazes de evidenciar a influência do mapa vivo na gestão estratégica e na educação em saúde, os motivos que levam à obsolescência da aplicação contínua do mapa vivo, bem como a dificuldade de sua manutenção. A busca realizou-se em bancos de dados nas áreas da saúde como a LILACS, SciELO, Cochrane Library e Biblioteca Virtual de Saúde. Foi possível integrar conceitos relacionados à aplicação prática do mapa vivo no contexto do planejamento de ações estratégicas nas Unidades Básicas de Saúde. Nesse aspecto notou-se significativo debate contemplado no que tange a ausência de padronização e o questionamento da viabilidade do mapa vivo no cotidiano dos trabalhadores da saúde brasileira.
A hipertensão arterial sistêmica (HAS) é uma doença crônica e silenciosa de elevada prevalência comunitária em que a continuidade do monitoramento é essencial para melhor qualidade de vida e prevenção de agravos associados ao risco cardiovascular. Um dos problemas da monitorização é a incerteza quanto ao diagnóstico bem como o não cadastramento dos portadores dessa patologia na Estratégia de Saúde da Família (ESF). Diante dessa problemática, foi proposta a implementação de um Procedimento Operacional Padrão (POP) para garantir a confirmação do diagnóstico e o tratamento longitudinal do paciente hipertenso, visando a preservação da saúde coletiva, além disso, o Plano Municipal de Saúde do município de São Caetano do Sul (2022-2025) estabelece prioridade na salvaguarda e no manejo da HAS. Assim, foi realizado um estudo observacional transversal no qual foi elaborado um POP baseado em fluxograma organizacional para ser aplicado pelos Agentes Comunitários de Saúde nos usuários da UBS Maria Corbeta Segatto visando padronizar a conduta de acolhimento e acompanhamento em conjunto com a ESF, a fim de contribuir com a meta estabelecida pelos indicadores publicados no plano municipal. Com isso, a sugestão conceptiva de POP foi proposta com foco na organização atuante, sedimentada e direcionada da atividade profissional da equipe de forma a manifestar melhor grau de expertise na ação de atendimento e melhor expressividade da qualidade de assistência recebida pelo munícipe.
