RESUMO -A meningoencefalocele transesfenoidal é rara apresentação dos disrafismos do tubo neural. Caracteriza-se pela herniação do tecido neural através de uma falha óssea no osso esfenóide. A apre s e n t a ç ã o clínica é variável, podendo ser desde assintomática até obstrução das vias aéreas, rinorréia, meningite, disfunção hipotalâmica e anomalias ópticas. Seu tratamento cirúrgico é controverso. Descrevemos o caso de um menino de 7 anos que apresentava uma estrutura pulsátil preenchendo o teto da cavidade oral, disgenesia palatina e hipertelorismo. O diagnóstico de meningoencefalocele transesfenoidal transpalatina foi confirmado pela tomografia computadorizada e ressonância magnética. It is characterized by the herniation of the neural tissue through a bony defect in the sphenoid bone. The clinical presentation is variable. It may be assymptomatic or it may include an upper airway obstru c t i o n , rh i n o rrhea, meningitis, hypothalamic dysfunction and optic anomalies. The surgical treatment is contro v e r s i a l . We describe the case of a 7-year-old boy who presented a pulsate stru c t u re filling the palate, palate digenesis and hypertelorism. The diagnosis of transsphenoidal transpalatal meningoencephalocele was confirm e d by a computerized tomography and a magnetic resonance imaging. Constitui 1 a 10% de todas as encefaloceles 3 . Caracteriza-se pela h e rniação do tecido neural através de um defeito nas e s t ruturas ósseas da base do crânio 2 , 7 , 8 . A encefalocele basal é classificada em transetmoidal, esfenoetmoidal, esfenoorbital, esfenomaxilar e transesfenoidal 8 , 9 . O tipo transesfenoidal é o menos freqüente, re p re s e ntando 5% das encefaloceles da base do crânio 2,7,10 . R e l a t a remos um caso de meningoencefalocele transesfenoidal transpalatina diagnosticado e tratado em nosso serviço. CASOMenino, leucoderma de 7 anos de idade, atendido na Santa Casa de Montes Claros-MG com relato de déficit de a p rendizagem, dificuldade para alimentar, obstrução nasal, respiração bucal e voz anserina. Chance fracture, suggestive of a flexion-distraction mechanism, is an uncommon type of injury in the pediatric population. Its occurrence, with associated intra-abdomimal lesions, has increased with the widespread use of seat belts. Since this type of fracture presents subtle variations on common image examinations, its diagnosis may be delayed increasing the probability of subsequent neurologic deficits. In this paper we report the case of three siblings, victims of a motor vehicle head-on collision,either presenting seat belt fracture or Chance fracture. They were treated in the Hospital João XXIII, Belo Horizonte. The case reported reinforces the hypothesis describing the fracture mechanism and associated intra-abdominal injuries.
Successful treatment of chronic pain for patients with failed back surgery syndrome can be extremely complicated. These patients require careful and individualized clinical assessment, as they often present with mixed pain syndromes that involve both neuropathic and nociceptive components. The distinct types of pain involved in such cases may require combined treatments from individual interventions that are analgesically independent and specific for each type of pain involved. Neuromodulation by electric stimulation at appropriately chosen targets and combinations may be an important option to consider for such patients. We present a case of combined debilitating axial nociceptive spinal pain and bilateral neuropathic leg pain in a patient after 14 failed back operations. A combination of spinal cord stimulation (SCS) and deep brain stimulation in the periventricular gray (PVG) have successfully provided the patient with complete relief of both components of his chronic pain condition, after all other pain management options had been exhausted. By alternating activation of each implanted stimulator separately and in conjunction, we were able to demonstrate a clinically independent analgesic character for each stimulation system, each specific to a particular type of pain. The SCS provided complete relief of the neuropathic pain component, without affecting the nociceptive component at all. The PVG stimulation provided complete relief of the nociceptive component, without affecting the neuropathic component at all. In combination, there was complete relief of the total chronic pain condition. There appeared to be no overlapping or synergistic effect between the two neuromodulation systems in the patient. The patient has had prolonged complete relief from his chronic pain condition with the combined neuromodulation intervention over 22 years of follow-up.
RESUMO -Infarto encefálico isquêmico extenso (IEIE) é a perda do suprimento sangüíneo de uma grande área cerebral, principalmente do território da artéria cerebral média. Pode evoluir com edema importante, hipertensão intracraniana e óbito em até 80% dos casos. Objetivo: Avaliar os resultados da craniectomia descompressiva no tratamento da hipertensão intracraniana secundária ao IEIE, comparando com os resultados de outros estudos publicados na literatura. Método: Foram analisados 34 pacientes com IEIE tratados clinicamente sem sucesso e que necessitaram de craniectomia descompressiva para controle da hipertensão intracraniana. Resultados: 8 pacientes (23,52%) faleceram, 26 (76,47%) sobreviveram, sendo que 2 (7,70%) permaneceram em estado vegetativo. Conclusão: Os fatores idade acima de 50 anos e sexo masculino se associaram a maior risco de evolução para óbito. O nível de consciência à admissão e a área do retalho ósseo apresentaram valores próximos de significância estatística. Is possible that evolutes a major edema, intracranial hypertension and death in about 80% of the cases. Objective: To avaliate the results of a descompressive craniectomy on the treatment of the secundary intracranial hypertension to LICI, comparing to other results of medical literature already published. Method: Were analysed 34 pacients diagnosed with LICI clinically treated unsuccesfully that needed forther on the decompressive craniectomy treatment, for the control of intracranial hypertension. Results: 8 pacients (23.52%) died, 26 (76.47%) survived, and 2 (7.70%) developed a vegetative state condition. Conclusion: The factors age over 50 years and male gender were associated with a high death risk. Os IEIEs podem evoluir com edema grave, hipertensão intracraniana (HIC) e óbito em 80% dos casos, apesar do tratamento conservador, justificando a denominação de infarto maligno [1][2][3][4] . Sinais clínicos e radiológicos como hipodensidade precoce em mais de 50% do território irrigado pela ACM ou volume do infarto à tomografia computadorizada (TC) igual ou superior a 240 cm 3 são sugestivos de evolução para infarto maligno 1,3,5 .
