Résumé Le rapport social entre médecins et personnel infirmier à l’hôpital public s’est construit historiquement selon le modèle type du rapport de couple conjugal (les médecins sont des hommes, les infirmières sont des femmes) ou modèle de la complémentarité. Son étude actuelle met en évidence la construction de formes sociales comportant des modalités diverses. Ainsi, la féminisation de la médecine et la masculinisation des soins infirmiers ont généré différentes configurations sexuées. Cette évolution du point de vue du genre s’accompagne d’un renforcement de l’antagonisme de classe sociale entre les deux groupes professionnels. D’autres variables comme celle de l’âge ou bien encore l’opposition grande ville/province interviennent dans l’analyse. Deux modèles correspondant à deux configurations sexuées de services de soins parmi les plus répandues sont identifiés. Le premier modèle se caractérise par la mise en place d’un « gouvernement de service » et par des pratiques infirmières qui vont de l’évitement à la résistance. Le second modèle se définit par une organisation du travail flexible et par des pratiques infirmières qui se situent entre une proximité de genre et une contestation du pouvoir médical. En conclusion, ce « nouveau rap port social » apparaît fortement instable. Le phénomène résulte en grande partie du bouleversement de l’ordre sexué professionnel traditionnel.
Résumé Dans un contexte de changements rapides et profonds de l’organisation hospitalière, comment des médecins hospitaliers vivent-ils leur situation au travail ? Comment disent-ils leurs sentiments de satisfaction et d’insatisfaction dans leur pratique professionnelle ? Une enquête en ligne (Santé et satisfaction des médecins au travail) a permis de recueillir en 2007-2008 les libres réponses de 2 555 médecins et pharmaciens salariés sur ce qu’ils aiment dans leur métier et ce qui leur est pénible. Des logiciels informatiques d’analyse lexicométrique ont permis de faire des analyses de la distribution des formes lexicales de ce corpus. On constate que, s’agissant de « ce qu’ils aiment le plus », les médecins et les pharmaciens répondants ont mis spontanément au premier plan les aspects relationnels et de contenu médical de leur profession. Sur « ce qui leur semble le plus pénible », les réponses portent sur les conditions de travail, sur le manque de temps, de personnel, de locaux et, surtout, de reconnaissance, tant de l’administration ou des collègues que des patients, et sur l’agressivité de ces derniers. Les contraintes administratives, enfin, et les difficultés relationnelles avec l’administration sont au cœur de la pénibilité décrite par ces médecins.
