Introducción: El lupus eritematoso tumidus (LET) es una forma de lupus eritematoso cutáneo infradiagnosticado, que puede ocurrir antes o concomitante con el lupus eritematoso sistémico (LES). Objetivo: Describir, en pacientes con LET, características como edad, sexo, fototipo y formas clínicas predominantes. Identificar la asociación con LES y registrar tratamientos realizados. Material y métodos: Se analizaron historias clínicas de pacientes latinoamericanos adultos que asistieron a consulta entre 2003 y 2017. Resultados: Catorce pacientes cumplieron los criterios de inclusión. La edad media fue de 43 años, 10 pacientes eran mujeres, el fototipo III fue el más frecuente. El 57% de los pacientes presentaron lesiones urticariformes, la mitad mostró fotosensibilidad; 21% de los pacientes exhibió la forma anular, cinco (36%) presentaron lesiones en extremidades inferiores y 14% edema palpebral. Al comienzo dos pacientes tenían diagnóstico de LES, y al completar el estudio 50% de los pacientes cumplió criterios de SLICC para LES. Las manifestaciones inespecíficas fueron artritis, alopecia difusa y livedo reticularis. El tratamiento con antipalúdicos fue el más utilizado y con mejores resultados. Conclusiones: Los estudios epidemiológicos sobre LET son escasos en Latinoamérica. Este trabajo analiza el grupo más grande de pacientes reportados hasta ahora en nuestro país. La mayoría de los resultados concuerdan con otras series. La diferencia destacada fue la frecuente asociación con LES. Las observaciones nos llevan a afirmar la importancia de reconocer al lupus tumidus como una entidad bien caracterizada, mejorando sus criterios de diagnóstico y estableciendo una estrecha relación con el LES, hasta ahora no documentada.
La calcinosis cutis distrófica puede ser una manifestación de múltiples enfermedades autoinmunitarias del tejido conjuntivo y causar desde un trastorno estético hasta una severa morbilidad. El estudio de la fisiopatogenia, donde se destaca el papel de la inflamación crónica, permite un abordaje terapéutico diverso y combinado para lograr mejores resultados y alienta el uso precoz e intensivo de antiinflamatorios.
La vasculitis crioglobulinémica es una vasculitis de vasospequeños que se da en presencia de crioglobulinas séricas.Las crioglobulinemias mixtas son las más frecuentes y se asocian a infecciones crónicas, típicamente,al virus de la hepatitis C, así como a enfermedades linfoproliferativas y autoinmunes, más a menudo,al síndrome de Sjögren. Las manifestaciones clínicasincluyen púrpurade los miembros inferiores, neuropatía periférica, artralgias y glomerulonefritis. La presencia de vasculitis crioglobulinémica en el contexto de un síndrome de Sjögren es marcadora de peor pronóstico. Presentamos el caso de una paciente con vasculitis crioglobulinémica con compromiso cutáneo y renal, que condujo al diagnóstico de un síndrome de Sjögren primario.
Introducción: La hemiatrofia facial progresiva o síndrome de Parry-Romberg y la morfea en golpe de sable forman parte de las morfeas lineales cefálicas. Son enfermedades inflamatorias crónicas de la piel y tejidos subyacentes, que se caracterizan por esclerosis y atrofia cutánea. Objetivos: Describir las características clínicas, manifestaciones asociadas, histología, laboratorio, estudios complementarios y tratamientos instaurados. Diseño: Estudio retrospectivo descriptivo. Materiales y métodos: Se revisaron las historias clínicas de pacientes con morfea evaluados en el Sector Colagenopatías desde julio de 2010 hasta diciembre de 2016. Resultados: De 56 pacientes, 11 cumplieron con los criterios de inclusión, 7 con diagnóstico de hemiatrofia facial progresiva, 2 con morfea en golpe de sable y 2 con ambas patologías. El 64% fueron mujeres. Las manifestaciones extracutáneas se presentaron en el 64% de los casos. El tratamiento utilizado en todos los pacientes fue el metotrexato, asociado o no, al uso de corticosteroides sistémicos. Conclusiones: La mayoría de nuestros resultados concuerdan con la bibliografía consultada, excepto las manifestaciones asociadas. Destacamos el tratamiento asociado de metotrexato y pulsos de corticosteroides intravenosos con resultados satisfactorios y bien tolerado.
