Pilocarpine is a cholinergic agonist that increases salivary flow and has been used to treat xerostomia. Oral intake is the most frequent route of administration. Adverse effects are dose-dependent and include sudoresis, facial blushing and increased urinary frequency. The objective of the present study was to evaluate the effects of topical pilocarpine solutions as mouthwashes on salivary flow and their adverse effects on healthy subjects. Forty volunteers received 10 ml 0.5, 1 and 2% pilocarpine solutions or 0.9% saline in a randomized, double-blind, placebo-controlled manner. Salivation was measured before and 45, 60 and 75 min after mouth rinsing for 1 min with 10 ml of saline or pilocarpine solutions. Vital signs were measured and ocular, gastrointestinal and cardiovascular symptoms, anxiety and flushing were estimated using visual analog scales. There was a dosedependent increase in salivation. Salivation measured after 1 and 2% pilocarpine (1.4 ± 0.36 and 2.22 ± 0.42 g, respectively) was significantly (P<0.001) higher than before (0.70 ± 0.15 and 0.64 ± 0.1 g), with a plateau between 45 and 75 min. Cardiovascular, visual, gastrointestinal and behavioral symptoms and signs were not changed by topical pilocarpine. Mouth rinsing with pilocarpine solutions at concentrations of 1 to 2% induced a significant objective and subjective dose-dependent increase in salivary flow, similar to the results reported by others studying the effect of oral 5 mg pilocarpine. The present study revealed the efficacy of pilocarpine mouthwash solutions in increasing salivary flow in healthy volunteers, with no adverse effects. Additional studies on patients with xerostomia are needed.

O artigo aborda epidemiologicamente o perfil dos coletores de lixo seletivo e descreve as lesões musculares e acidentes de trabalho mais freqüentes. Este estudo transversal foi desenvolvido com duas empresas (denominadas como empresa A e empresa B), em Porto Alegre, selecionando-se 50 funcionários aleatoriamente (presentes e aptos nos dias das entrevistas para a pesquisa) e aos quais foi aplicado um questionário. A Média de idade da amostra foi de 34 e 22 anos para a empresa A e B, respectivamente. Cerca de 66% dos funcionários da empresa B trabalham a menos de 5 anos na companhia, ao passo que 95% dos funcionários da empresa A trabalham há mais de 5 anos. O índice de lesões entre as duas empresas é alta 90% e 46,6% para empresas A e B, respectivamente. Lesões são mais comuns nos membros inferiores, seguidas pelas lesões dos membros superiores e da coluna. Ambas as empresas apresentavam deficiência do uso de equipamentos de proteção, sendo estes praticamente ignorados pelos funcionários, embora de uso obrigatório. As avaliações médicas são realizadas esporadicamente, sem acompanhamento regular. Ambas as empresas apresentam altos índices de leões em geral, sendo as lesões de membros inferiores mais comuns. Também são altos os índices de cortes e outros pequenos ferimentos, provavelmente pelo não uso de equipamentos de proteção obrigatórios.
Objective: Evaluate the ability of serum concentration of prostate specific antigen (PSA) between 2 cutting points to predict the existence of bone metastasis confirmed by bone scintigraphy in man with prostate cancer.Materials and Methods: Two hundred and fourteen consecutive patients with prostate cancer were evaluated during the present study in the period from 1998 to 2001. From all patients, PSA serum concentrations and bone scintigraphy were obtained. For the study, 2 cutting points of PSA (10 and 20 ng/mL) were adopted to predict the existence of bone metastasis.Results: From the 214 patients, 35 (16.3%) presented positive scintigraphic examinations for the presence of bone metastasis. No patient presented bone metastasis in scintigraphy if having PSA < 10 ng/mL, and in only 1 patient (0.46%) with bone metastasis PSA concentration was < 20 ng/mL. Therefore, when the cutting point adopted for PSA serum concentration was 10 ng/mL, a negative predictive value for bone metastasis was 100% with sensitivity rates of 100%. Nevertheless, the positive predictive value and the specificity of the method were, respectively, 24.5% and 39.7%. When the cutting point of PSA serum concentration was 20 ng/mL, an increment was observed in rates of positive predictive value and specificity (41.5% and 73.2%), respectively, without substantial changes in negative predictive value (99.2%) and sensitivity (97.1%) of the method.Conclusions: Data of present study allow for the conclusion that PSA serum concentration over 20 ng/mL was a more accurate cutting point than PSA serum concentration over 10 ng/mL to predict the presence of bone metastasis in scintigraphy.
The prevalence of ocular metastasis in patients with malignancy and metastatic disease seemed significant in the studied group. The neoplasms more frequently causing ocular metastasis were: breast and lung carcinoma, and the choroid was involved more frequently.
RESUMOA ectopia lentis et pupillae é uma síndrome genética rara, congênita, autossômica recessiva de expressividade variável, caracterizada por ectopia do cristalino e da pupila, geralmente bilateral e simétrica, sem associação com anormalidades sistêmicas. Sua patogênese ainda é desconhecida, porém se especula que haja alteração mesodérmica, neuroectodérmica, combinada ou mecânica. O artigo relata o caso clínico de um paciente portador de ectopia lentis et pupillae, descrevendo seus aspectos clínicos e genéticos, além de suas complicações secundárias e diagnósticos diferenciais.
RESUMOObjetivo:Nos últimos anos , o transplante de membrana amniótica (MA) tem sido utilizado com freqüência na reconstrução da superfície ocular em diversas doenças oculares. Este estudo objetiva identificar as indicações mais freqüentes do transplante de MA, assim como avaliar os resultados deste procedimento. Métodos: Foram analisados retrospectivamente os prontuários de 107 pacientes submetidos ao transplante de MA, avaliando a indicação do transplante de MA e a evolução do quadro no período pós-operatório. Resultados: Dos 107 pacientes estudados, as indicações de MA mais freqüentes foram exérese de pterígio 33.64%(n=36), defeito epitelial persistente 29.90%(n=32), liberação de simbléfaro 18.69%(n=20), queimadura química 6.54%(n=7), cérato-conjuntivite primaveril 4.67%(n=5), síndrome de Stevens-Johnson 2.80%(n=3), penfigóide ocular cicatricial 1.86%(n=2), ceratopatia bolhosa 1.86%(n=2). No seguimento dos pacientes houve recidiva do pterígio em (6.25%) nos pterígios primários e em (15%) nos pterígios recidivados. Não houve recidiva nos pacientes com simbléfaro, tendo como causa pterígio recidivado. Houve fechamento do epitélio com o uso de MA nos pacientes do estudo variando de 50 a 80%, dependendo da doença de base. Na fase aguda da queimadura química houve fechamento epitelial em 50% dos casos. Já nos que utilizaram MA na fase crônica da queimadura a restauração da superfície ocular ocorreu em 33% dos casos. Uso da MA na cérato-conjuntitivite primaveril com papilas gigantes, 80% não apresentaram recidiva das papilas.Todos os pacientes que tiveram indicação de MA, devido à ceratopatia bolhosa apresentaram diminuição da dor . Os pacientes que tiveram como causa da utilização de membrana a Síndrome de Stevens-Johnson (SSJ) na fase aguda apresentaram diminuição importante do processo inflamatório . Dos pacientes que receberam enxerto de MA na fase crônica da SSJ, 50% apresentaram evolução favorável. Conclusão: O enxerto de membrana amniótica nas diferentes doenças avaliadas apresentou resultados favoráveis na maioria delas. Esta técnica pode ser considerada como boa alternativa terapêutica a ser empregada nestas situações, além de ser um método barato e de fácil execução cirúrgica.
