The aim of the study was to look back on the course of action involving measures of tuberculosis control in Brazil since the end of the 19th century, covering the history of social struggles and pointing out institutions and people that have dedicated themselves to looking for solutions to these issues. The Brazilian response to tuberculosis started in society with the Ligas Contra a Tuberculose (Leagues Against Tuberculosis), promoting scientific advances, such as the BCG vaccination, which begun in 1927. From the public power, the Inspetoria de Profilaxia da TB (TB Prophylaxis Inspection Service - 1920), the Serviço Nacional de Tuberculose (National Service of Tuberculosis - 1940), and the Campanha Nacional Contra a Tuberculose (National Campaign Against Tuberculosis - 1946), coordinated national policies such as chemotherapy, beginning with the discovery of streptomycin in 1944. The emergence of bacterial resistance led to the development of several therapeutic schemes. The Scheme 1 (rifampycin, hydrazide and pyrazinamid), which was the main one in 1979 and is still used nowadays, had a great epidemiological effect. The WHO declared TB a public health emergency in 1993. In response, Brazil developed some strategies; the first one was the Plano Emergencial para Controle da Tuberculose (Emergency Plan for Tuberculosis Control - 1994), prioritizing 230 municipalities. The current prospects are an effective municipalization of actions and their greater integration with the Programas de Agentes Comunitários e Saúde da Família (Humanitarian Agents and Family Health Programs).

Objetivo: Determinar a efetividade do tratamento com esquemas alternativos para casos confirmados de tuberculose multirresistente (TBMR) primária e adquirida, em pacientes ambulatoriais. Métodos: Casos de TBMR foram definidos como cultura e isolamento de M. tuberculosis e perfil de resistência in vitro a pelo menos à rifampicina, isoniazida e a uma terceira droga dos esquemas padronizados no Brasil, tanto pelo método convencional (LJ) quanto pelo sistema radiométrico BACTEC. Desenho: Ensaio clínico, multicêntrico, não randomizado e controlado. Os pacientes foram arrolados entre abril de 1995 e dezembro de 1997, no total de 197. Por diversas razões 10 casos foram excluídos da análise. Em abril de 1998 permaneciam em tratamento 36 pacientes. Foram analisados 149 casos com duração média de tratamento de 14 meses sem interrupção. Os regimes foram escolhidos conforme o perfil de sensibilidade: 1) estreptomicina, ofloxacina, terizidona, etambutol, clofazimina ou 2) amicacina, ofloxacina, terizidona, etambutol e clofazimina. Demografia: sexo: masculino - 68,4%, feminino - 31,5%; média de idade - 36,9 anos (14-71 anos); prevalência de HIV - 1,9%; taxa de resistência primária - 8%, taxa de resitência secundária - 92%. Resultados parciais: 120 (79,5%) pacientes negativaram a cultura no período de 3 meses; cura - 53%, falência - 31%, óbito - 6%, abandono - 10%. Definições: cura - tratado por 12 meses, com 6 meses de tratamento após 2 culturas consecutivas negativas; abandono - tratamento e consultas descontinuados; óbito - morte causada por TB após 2 meses de tratamento; falência - persistência de positividade na cultura em 12 meses seguidos. Conclusão: O maior preditor da multirresistência no estudo foi tratamento prévio irregular ou incompleto. Outros preditores (p < 0,05) foram: ser homem, ter lesão radiológica cavitária bilateral e ter mais de 2 anos de doença. A taxa de conversão bacteriológica em escarro e cultura foi alta em 6 meses de tratamento.

The tuberculosis incidence rate in prisons in Rio de Janeiro State, Brazil, was 30 times higher in 2004 than in the general population and is probably underestimated, particularly given the difficult access to care in the prison setting. To obtain a better estimate, a survey used systematic X-ray screening and showed a prevalence rate of 4.6% in one such detention facility, A (n = 1,052). Two additional surveys, in facilities B (n = 590) and C (n = 1,372), showed even higher prevalence rates (6.3% and 8.6% respectively). A comparison of socio-demographic characteristics between A, B, and C showed a heterogeneous prison population. As compared to facility A, inmates in B and C come from poorer urban communities and have more frequent histories of incarceration and tuberculosis. These differences, consistent with the prevalence data, imply the necessary adaptation of tuberculosis control programs to each detention facility's epidemiological and socio-demographic profile.

BackgroundTuberculosis (TB) is a major issue in prisons of low and middle income countries where TB incidence rates are much higher in prison populations as compared with the general population. In the Rio de Janeiro (RJ) State prison system, the TB control program is limited to passive case-finding and supervised short duration treatment. The aim of this study was to measure the impact of X-ray screening at entry associated with systematic screening on the prevalence and incidence of active TB.MethodsWe followed up for 2 years a RJ State prison for adult males (1429 inmates at the beginning of the study) and performed, in addition to passive case-finding, 1) two “cross-sectional” X-ray systematic screenings: the first at the beginning of the study period and the second 13 months later; 2) X-ray screening of inmates entering the prison during the 2 year study period. Bacteriological examinations were performed in inmates presenting any pulmonary, pleural or mediastinal X-ray abnormality or spontaneously attending the prison clinic for symptoms suggestive of TB.ResultsOverall, 4326 X-rays were performed and 246 TB cases were identified. Prevalence among entering inmates remained similar during 1st and the 2nd year of the study: 2.8% (21/754) and 2.9% (28/954) respectively, whereas prevalence decreased from 6.0% (83/1374) to 2.8% (35/1244) between 1st and 2nd systematic screenings (p < 0.0001). Incidence rates of cases identified by passive case-finding decreased from 42 to 19 per 1000 person-years between the 1st and the 2nd year (p < 0.0001). Cases identified by screenings were less likely to be bacteriologically confirmed as compared with cases identified by passive-case finding.ConclusionsThe strategy investigated, which seems highly effective, should be considered in highly endemic confined settings such as prisons.
