We analyzed the publications about single-photon emission computed tomography in the diagnostics of takotsubo syndrome; the dynamics of testing data are presented on a clinical example. Single-photon emission computed tomography can be used to evaluate perfusion parameters for exclusion of myocardial infarction in uncertain cases. In segments with impaired contractile function, it is possible to detect normal or reduced perfusion. During the acute phase there is a reduced accumulation of radiopharmaceuticals in myocardial segments with impaired function, which coincides with the regional distribution of neuronal activity. Pathological changes may persist for several months, and use of scintigraphy with a radiopharmaceutical may help in cases when the diagnosis was not well-timed established. The method of myocardial scintigraphy in ECG-synchronized tomography allows to simultaneously assess the perfusion and the function of the left ventricle and differentiate such disorder as takotsubo syndrome.
Реферат. Цель исследования -анализ медицинской литературы, посвященной применению перфузионной томосцинтиграфии в диагностике, и представление данных этого метода исследования у пациентов с безболевой ише мией миокарда. Материал и методы. Проведен анализ медицинской литературы и представление соб ственных данных. В исследование включены пациенты, которым проведена перфузионная сцинтиграфия миокарда в режиме однофотонной эмиссионной компьютерной томографии в период с января 2013 г. по декабрь 2017 г. в условиях ГАУЗ «Межрегиональный клинико-диагностический центр» на гамме-томографе MILLENIUMMPR/GEMedicalSystems/ по двухдневному протоколу. Результаты и их обсуждение. Нарушения перфузии в покое выявлено у всех пациентов. Проведение нагрузочного теста провоцировало увеличение числа сегментов с гипоперфузией от 10,6±3,53 до 11,06±3,13; достоверны различия (р<0,05) для сегментов 11, 12, 16, 17, 19, для сегментов 16 и 18 у пациентов с инфарктом миокарда в анамнезе (р<0,05), для сегмента 12 среди пациентов без инфаркта миокарда. У 88,24% пациентов выявлен высокий риск коронарных осложнений. Сцинтиграммы высокого риска: множественные (более чем в 1 бассейне коронарной артерии) преходящие стресс-индуцированные дефекты перфузии выявлены у 82,5% пациентов. Выводы. Метод перфузионной томосцинтиграфии миокарда позволяет получить трехмерное изображение левого желудочка с информацией о распределении объема перфузии по миокарду, дает возможность более точно дифференцировать такое состояние, как безболевая ишемия миокарда. Полученные данные ставят задачи дальнейшего изучения диагностики безболевой ишемии с помощью перфузионной томосцинтиграфии, сравнительного анализа показателей в зависимости от типа безболевой ишемии миокарда, сопутствующих заболеваний, возраста и пола. Цель -сравнительный анализ результатов оценки регионарной и интегральной сократимости миокарда левого желудочка у пациентов с хронической ишемической болезнью сердца по результатам эхокардиографии и перфузионной сцинтиграфии миокарда. Материал и методы. Анализ результатов плановой эхокардиографии и перфузионной сцинтиграфии миокарда без нагрузки у пациентов с диагнозом «хроническая ишемическая болезнь сердца», вне зависимости от перенесенного инфаркта миокарда, проходивших обследование в кардиологическом отделении. Определялись объемы и интегральная сократимость левого желудочка по методу дисков, регионарная сократимость в баллах в соответствии со схемой из 17 сегментов, ряд показателей центральной гемодинамики. По баллам регионарной сократимости рассчитывался индекс сократимости левого желудочка, который может рассматриваться в качестве косвенной характеристики его интегральной сократимости. Результаты и их обсуждение. Оба метода продемонстрировали высокую степень совпадения значений основных функциональных показателей левого желудочка. В то же время оценка регионарной сократимости имела ряд расхождений: индекс сократимости по результатам сцинтиграфии оказался выше такового для эхокардиографии. Оценка регионарной сократимости по результатам двух методов различалась почти для половины сегментов. В большинстве случаев выявленные расхождения в оценке регионарной сократимости определялись более высокой степенью дисфункции по результатам сцинтиграфии. Выводы. Можно предположить, что выявленные различия связаны с методологией диагностических исследований и c особенностями заболеваний сердца. Окончательное суждение о таком комплексном функциональном показателе, как регионарная сократимость, следует принимать с учетом результатов всех доступных методов. Aim. To perform a comparative analysis of multislice computed tomography (MSCT), echocardiography and single photon emission computed tomography (SPECT) in the evaluation of left ventricular end-diastolic volume (LV EDV) and left ventricular ejection fraction (LVEF). Methods. The study included 44 patients (15 female, 29 male) aged of 21 to 73 years (mean age 55±11 years). LV EDV and LVEF were assessed by noninvasive MSCT coronary angiography. echocardiography and SPECT were also performed. Results. There was a statistically significant difference found between the LV EDV medians for the following pairs: MSCT vs Quantitative Gated SPECT (QGS), MSCT vs SPECT using 4D MSPECT regimen, MSCT vs echocardiography. There was no statistically significant difference determined for the following pairs: echocardiography vs SPECT, QGS SPECT vs 4D MSPECT. Difference between the LV EDV were calculated using the Bland-Altman method as following: MSCT vs echocardiography - 55±33 ml, MSCT vs QGS SPECT - 38±29 ml, MSCT vs 4D MSPECT - 30±33 ml. Differences in the LVEF evaluation methods were: MSCT vs echocardiography - 2,5±7,2%, MSCT vs QGS SPECT - 0,9±8,3%, MSCT vs 4D MSPECT - 1,2±8,1%. The highest LV EDV values were registered by MSCT, the lowest - by echocardiography, with the values registered by SPECT lying in between MSCT and echocardiography volumes. Conclusion. MSCT, echocardiography and SPECT present different left ventricular volume and similar LVEF data.
