Objetivo: Revisar los resultados del tratamiento quirúrgico de los tumores glómicos del miembro superior tratados en nuestro servicio de cirugía de la mano en los últimos 15 años y comparar con la bibliografía especializada los datos de una serie de 14 tumores operados.Material y método: Se trataron 14 tumores glómicos en 14 pacientes: 7 hombres y 7 mujeres. La media de edad fue de 50,7 años (rango 31-70).En todos los pacientes se anotó la zona de presentación del tumor, su aspecto macroscópico con registro fotográfico, la técnica quirúrgica y el resultado final, funcional y estético.Resultados: En todos los pacientes se dieron presentaciones únicas y todos se curaron completamente con el tratamiento quirúrgico, pero en 2 hubo recidivas y uno de ellos debió operarse hasta 5 veces para erradicar el tumor completamente.Conclusión: El tumor glómico, por su infrecuencia, es poco conocido fuera del ámbito de la cirugía de la mano y de la dermatología, de ahí la demora en el diagnóstico que padecen buena parte de los pacientes. Los de presentación extradigital no suelen sospecharse y su diagnóstico lo aporta el estudio anatomopatológico.El tratamiento quirúrgico, consistente en la exéresis de la masa tumoral y el legrado de la cavidad ósea si la hay, es resolutivo en la mayor parte de los casos, si bien la tasa de recidivas es muy elevada, probablemente por haberse realizado una escisión incompleta.La complicación más frecuente es de tipo estético, por deformidades de la uña en los tumores de presentación subungueal.
Antiseptic cord care reduces bacterial colonisation but delays cord detachment
Objetivo: Presentar los resultados del tratamiento quirúrgico de la compresión del nervio cubital en el codo mediante descompresión y reconstrucción de un nuevo retináculo epitrócleo-olecraneano.Material y método: Se estudiaron 51 codos en 49 pacientes: 25 mujeres y 24 hombres, con una edad media de 44,2 años. En todos los pacientes se anotó el aspecto macroscópico del nervio, registrándose la zona en la que éste se presentaba comprimido, así como la existencia de seudoneuroma, imagen en reloj de arena, cambios de coloración o alteraciones morfológicas de otro tipo.En la clasificación de McGowan, 11 casos presentaban un grado I, 28 casos un grado II y 12 casos un grado III. De los 49 pacientes, 43 estaban en edad laboral, la mayor parte en situación de baja laboral en el momento de la cirugía.Resultados: En los 51 casos, se obtuvo un resultado excelente en 37 (72,6%), bueno en 10 (19,6%) y regular en 4 (7,8%). Todos los pacientes que se encontraban de baja laboral se reincorporaron a sus actividades laborales con una media de 41,5 días.Conclusiones: La técnica de descompresión con reconstrucción de un nuevo retináculo epitrócleo-olecraneano es una intervención quirúrgica sencilla que impide la subluxación anterior propia de la descompresión simple y evita las complicaciones de las transposiciones anteriores y de las epitroclectomías.Esta técnica está indicada en todos los grados de afectación siempre que la anatomía del codo sea normal y en pacientes que no hayan sido operados previamente con transposiciones anteriores.
