Los honorarios libres en medicina ambulatoria : una lectura según la teoría de las convenciones
Desde 1980 a 1990 los médicos franceses tuvieron el derecho de elegir libremente su arancel ya que pudieron optar por sobrepasar los honorarios no reembolsables a los pacientes por el Seguro Social y de esta manera elegir el arancel 2 de la medicina libéral (o « arancel de honorarios libres »). El árticulo se propone analizar esta libertad arancelaria con la ayuda de la teoriá de las convenciones. Tratamos de aclarar la noción de convención y mostramos que es un útil adecuado para aprenhender las estrategias arancelarias de los médicos liberales.
Résumé Cet article examine l’incidence des maladies invalidantes sur les contrats d’assurance santé en présence de sélection adverse. Pour ce faire, nous utilisons un modèle d’utilité bivariée de décision dans le risque. Nous montrons que l’introduction des maladies invalidantes modifie les résultats traditionnels de Rothschild et Stiglitz [1976]. Nous caractérisons des conditions pour lesquelles les contrats de premier rang peuvent être offerts en présence de sélection adverse.
Résumé L’assurance dépendance privée reste relativement peu développée dans les pays européens ainsi qu’aux Etats-Unis. Cet article propose un argument théorique afin de rendre compte de la faible maturité du marché de l’assurance dépendance, en privilégiant une explication centrée sur la demande d’assurance. Il montre que la demande peut être faible lorsque l’état de santé actuel et attendu exerce un effet important sur l’utilité marginale de la richesse des individus et par suite sur la demande d’assurance. En particulier, il analyse certaines classes conduisant les prospects à ne pas s’assurer. Le faible développement du marché ne serait donc pas uniquement dû à des inefficiences de l’offre d’assurance ou à l’effet des asymétries d’informations ou à l’impact des arbitrages familiaux ou encore aux effets de l’environnement institutionnel, mais peut être à une conséquence directe des préférences des individus.
