Cutaneous angiosarcoma (CA) is an uncommon malignant mesenchymal neoplasm with aggressive behavior and a high mortality rate. A rare histological variant of CA is the epithelioid variant angiosarcoma (EA), which is associated with a worse clinical outcome. We report a 74-year-old female with an 8-month history of an ulcerated, poorly defined, painless tumoral mass on her scalp, face, and neck. Her past medical history included rheumatoid arthritis. A panel of immunohistochemical markers positive for vascular markers CD31 and CD34, confirmed the diagnosis. She had an overall 9-month survival since the diagnosis was made. Comorbid diseases and tumoral size make most of CA patients ineligible surgical candidates. Limited treatment options are available and due to its accelerated progression, achieving local control and prevention of metastasis in EA is challenging. We emphasize the need for early diagnosis and a multidisciplinary approach in order to improve survival in these patients.
The first case of penile leiomyosarcoma was described by Levi in 1930. In 1957 Ashley and Edwards reported the first case in the British literature and in 1963 Pack reported the first in the American literature. MacKenzie et al. were the first to recognize two types of leiomyosarcoma: superficial and deep. The first develop from smooth muscle cells from the superficial dermal layers of the glans penis or distal third of the penis, they are generally asymptomatic and less malignant, rarely invading deeper structures and without involvement of the urethra on physical exam. The best prognostic predictors are type of tumor at presentation (superficial vs. deep) and treatment choice, being total penectomy more effective for failures of local resections than for primary treatment of deep tumors.
En este trabajo pretendemos comprobar que las preconcepciones que los alumnos poseen sobre los circuitos eléctricos de corriente continua son comunes a alumnos de diferentes edades y están tan fuertemente arraigadas que ni la instrucción durante muchos años permite modificarlas.
Este trabajo presenta una herramienta didáctica, basada en la V de Gowin, cuya finalidad es facilitar un aprendizaje significativo que permita a los alumnos enfrentarse a la resolución de problemas de física de una forma creativa y constructiva. Para ello se ha elaborado una plantilla que guía la resolución de problemas, relacionando contenidos conceptuales con contenidos de tipo procedimental y ha sido aplicada a los temas de dinámica que aparecen en el curriculum oficial de la asignatura física y química de 1° de bachillerato. En esta experiencia han participado 43 alumnos divididos en dos grupos, uno formado por los alumnos que han seguido el método propuesto en este trabajo y el otro formado por aquellos que siguieron el método tradicional. Se ha realizado un pretest y un postest sobre conocimientos en dinámica así como un estudio de la evolución de la resolución de los problemas sobre este tema, antes, durante y después de la instrucción sobre la V de Gowin. El análisis de los resultados obtenidos muestra que el método de resolución de problemas según la V de Gowin mejora la calidad de aprendizaje de los alumnos, incrementándose tanto sus capacidades en la resolución de problemas de física como en la comprensión de conocimientos sobre temas de dinámica.
El hepatoblastoma del adulto es una patología in-usual, de patogenia no bien conocida y de muy mal pro-nóstico. Presentamos un caso de hepatoblastoma del adulto tratado en nuestro centro. Varón de 65 años, sin hepatopatía previa, que con-sulta por dolor en hipocondrio derecho de evolución subaguda. El diagnóstico anatomopatológico fue hepa-toblastoma epitelial del adulto, con bordes quirúrgicos libres. Fue reintervenido 5 meses después por recidiva precoz y falleció 10 meses después del diagnóstico por nueva recidiva masiva. Su diagnóstico definitivo es his-tológico. La cirugía radical ofrece el único tratamiento que aumente la supervivencia, pero frecuentemente recidiva. No existen pautas bien definidas de quimiote-rapia adyuvante, ni experiencia en trasplante. Palabras clave. Hepatoblastoma. Tumores hepáticos primarios. Adulto. abstRact Adult hepatoblastoma is a rare pathology. Its pathogeny is not well understood and prognosis is very bad. We present a case of adult hepatoblastoma treated in our centre. A 65 year-old male, without previous hepatopathy, who consulted due to right hypochondrial pain with a subacute evolution. The pathological diagnosis was adult epithelial hepatoblastoma, with free surgical margins. The patient received a second surgical intervention 5 months later due to early recurrence and died 10 months after the diagnosis due to a new massive recurrence. His definitive diagnosis is histological. Radical surgery is the only treatment that increases survival, but recurrence is frequent. There are no well-defined patterns of adjuvant chemotherapy nor is there any transplant experience.
