O Brasil é referência mundial no combate ao tabagismo e desenvolve ações por meio do Programa Nacional de Controle do Tabagismo e outros Fatores de Risco de Câncer (PNCTOFR). Com objetivo de apresentar as ações do PNCTOFR identificando as evidências de sua eficácia na redução da prevalência do tabagismo no Brasil, foi realizado este estudo bibliográfico. Utilizou-se documentos disponibilizados nos portais eletrônicos do Inca, da Política Nacional de Promoção da Saúde, do Observatório da Política Nacional de Controle do Tabaco e das Organizações Mundial da Saúde e Pan-Americana de Saúde, além das bases de dados Lilacs, PubMed Central,SciELO, ScienceDirect. O referencial para análise das ações foram as áreas/dimensões propostas pela International Union for Health Promotion and Education. A restrição da disponibilidade, controle do marketing e comercialização, atividades educativas nas escolas, atendimento na atenção primária à saúde a profissionais da área e à população em geral, controle do consumo em locais públicos e de trabalho são algumas das ações que contribuíram para redução na prevalência de tabagismo no Brasil, de 32,7% em 1997 para 14,8% em 2011. Os resultados apontam para a eficácia das ações do PNCTOFR, mas o tabagismo ainda é problema de saúde pública e precisa ser desnormatizado.
RESUMOObjetivo: Analisar a influência dos componentes da síndrome metabólica na resistência à insulina, por sexo e fase da adolescência. Sujeitos e métodos: Avaliaram-se dados bioquímicos, clínico, de estilo de vida e composição corporal de 800 adolescentes de 10 a 19 anos, de ambos os sexos, de Viçosa-MG/Brasil, divididos em fases: inicial (10 a 13 anos), intermediária (14 a 16 anos) e final (17 a 19 anos). Resultados: 10,3 e 3,4% apresentavam, respectivamente, resistência à insulina e síndrome metabólica. Na fase inicial, observaram-se maior prevalência de dislipidemia e na intermediária, de hiperuricemia e excesso de gordura corporal. O sexo feminino apresentou maior prevalência de dislipidemia, excesso de gordura corporal e resistência à insulina e o masculino, maior prevalência de HDL baixo, hiperuricemia e pressão arterial alterada. Os da fase inicial apresentaram maiores valores de colesterol total, LDL, HDL, triglicerídeos, glicemia de jejum e relação cintura/quadril, ficaram menos tempo sentados e realizavam maior número de refeições (p < 0,05) em relação às outras fases. O modelo final, ajustado por sexo, foi diferente para cada fase da adolescência. Conclusões: A resistência à insulina está associada à inadequação na composição corporal, nos níveis bioquímicos e no estilo de vida, sendo os fatores associados diferentes em cada fase da adolescência. Subjects and methods: We evaluated biochemical, clinical, lifestyle and body composition data of 800 adolescents from 10 to 19 years old, from both genders, from Viçosa, MG/Brasil, and there was the division by stage: early (10 to 13 years), intermediate (14 to 16 years) and late (17 to 19 years). Objective: To evaluate the relationship between the stages of somatic maturation and body composition in eutrophic female adolescents with or without excessive body fat. Methods:Cross-sectional study of 118 female adolescents, from 14 to 19 years-old, in Viçosa, Minas Gerais, Southeast Brazil. The adolescents were divided in two groups: Group 1 (G1), eutrophic with adequate body fat percentage, and Group 2 (G2), eutrophic with high body fat percentage. The somatic maturation was assessed by the formula for estimating the Peak Height Velocity (PHV). Results: The PHV had higher average score in G1 adolescents compared to G2 (0.26 versus 0.05; p=0.032). There was an association between G1, G2 and the somatic maturation (p=0.049). The female adolescents before and during PHV presented higher values of fat body BMI (p=0.034) and percentage of central fat (p=0.039) compared to the adolescents after PHV. There was a correspondence between before PHV stage and the excess of body fat (α=0.751). Conclusions: There was an association between somatic maturation and body composition in eutrophic female adolescents. Length, BMI and fat percentage were different among the somatic maturation stages. It is relevant to evaluate the somatic maturation and the changes occurring in the body composition during adolescence in order to better evaluate and manage the nutritional status and the body fat excess.
OBJECTIVE: To determine the prevalence of metabolic syndrome (MS) and the hypertriglyceridemic waist phenotype (HW) in a representative adolescent sample; as well as to establish which anthropometric indicator better identifies MS and HW, according to gender and adolescent age. METHODS: This cross sectional study had the participation of 800 adolescents (414 girls) from 10-19 years old. Anthropometric indicators (body mass index, waist perimeter, waist/stature ratio, waist/hip ratio, and central/peripheral skinfolds) were determined by standard protocols. For diagnosis of MS, the criteria proposed by de Ferranti et al. (2004) were used. HW was defined by the simultaneous presence of increased waist perimeter (>75th percentile for age and sex) and high triglycerides (>100 mg/dL). The ability of anthropometric indicators was evaluated by Receiver Operating Characteristic curve. RESULTS: The prevalence of MS was identical to HW (6.4%), without differences between genders and the adolescence phases. The waist perimeter showed higher area under the curve for the diagnosis of MS, except for boys with 17-19 years old, for whom the waist/stature ratio exhibited better performance. For diagnosing HW, waist perimeter also showed higher area under the curve, except for boys in initial and final phases, in which the waist/stature ratio obtained larger area under the curve. The central/peripheral skinfolds had the lowest area under the curve for the presence of both MS and HW phenotype. CONCLUSIONS: The waist perimeter and the waist/stature showed a better performance to identify MS and HW in both genders and in all three phases of adolescence.
OBJETIVO Descrever o escore Nova de consumo de alimentos ultraprocessados e avaliar seu potencial para refletir, no contexto brasileiro, a participação desses alimentos na dieta. MÉTODOS Estudo realizado na cidade de São Paulo com amostra de conveniência de 300 adultos, que responderam, em cerca de três minutos, em um tablet, a um questionário eletrônico de autorrelato sobre o consumo, no dia anterior, de 23 subgrupos de alimentos ultraprocessados comumente consumidos no Brasil. O escore de cada participante correspondeu ao número de subgrupos reportados. A participação de alimentos ultraprocessados no consumo alimentar do mesmo dia, expressa como percentual da ingestão total de energia, foi calculada por meio das respostas dos participantes a recordatório alimentar completo de 24 horas aplicado em cerca de 30 minutos por nutricionistas treinados. A associação entre o escore e a participação de ultraprocessados na dieta foi estudada por modelos de regressão linear. A concordância na classificação dos participantes segundo quintos do escore e quintos da participação de alimentos ultraprocessados na dieta foi avaliada pelo índice Pabak. RESULTADOS O percentual médio de participação de alimentos ultraprocessados na dieta aumentou linear e significativamente com o aumento do escore Nova de consumo de alimentos ultraprocessados. Observou-se concordância substancial na classificação dos participantes segundo quintos da distribuição do escore e quintos da distribuição do percentual de participação de alimentos ultraprocessados na dieta (índice Pabak = 0,67). Relação inversa da idade com a frequência de consumo relativamente elevado de alimentos ultraprocessados (quinto superior da distribuição) foi observada tanto para o escore quanto para a participação de alimentos ultraprocessados na dieta. CONCLUSÃO O escore Nova de consumo de alimentos ultraprocessados, obtido de forma rápida e prática, apresenta bom potencial para refletir, no contexto brasileiro, a participação desses alimentos na dieta.
