This study evaluated the resistance to demineralization and fluoride incorporation of enamel irradiated with Er:YAG. A total of 110 bovine teeth were selected and divided into eight groups: unlased, 37% phosphoric acid, and samples irradiated with the Er:YAG laser at several fluences (31.84 J/cm(2), 25.47 J/cm(2), 19.10 J/cm(2), 2.08 J/cm(2), 1.8 J/cm(2), and 0.9 J/cm(2)). The application of acidulated phosphate fluoride was performed after treatments. All samples were immersed in 2 ml of 2.0 M acetic-acetate acid solution at pH 4.5 for 8 h, and fluoride, calcium, and phosphorus ions dissolved were analyzed by atomic absorption spectrometry and spectrophotometry. The phosphoric acid and 31.84 J/cm(2) groups presented the lowest dissolution of calcium and phosphorus ions. Higher fluoride incorporation was observed on 1.8 J/cm(2) and 0.9 J/cm(2) groups. Based on these results, Er:YAG laser was able to decrease acid dissolution and increase fluoride uptake and can be a promissory alternative for preventive dentistry.
Perimolysis is a form of dental erosion, characterized by dental tissue demineralization without bacterial involvement. It RESUMOPerimólise é uma forma de erosão dentária, caracterizada pela desmineralização do tecido dental sem envolvimento bacteriano, de origem intrínseca ou extrínseca. É considerada de origem extrínseca quando causada pelo consumo excessivo de bebidas, alimentos e substâncias ácidas, como refrigerantes, bebidas alcoólicas, frutas ácidas e medicamentos de uso contínuo. Denomina-se intrínseca quando causada por distúrbios gastrintestinais como os que ocorrem como conseqüência da bulimia, da anorexia nervosa e do refluxo gastresofágico. A ação ácida pode causar erosão nas faces palatina/lingual dos incisivos e palatina/lingual e oclusal dos dentes posteriores levando a dor, ou seja, hiperestesia dental. O tratamento geralmente é multidisciplinar, devendo identificar e eliminar ou controlar as causas. A abordagem odontológica deve associar medidas preventivas para diminuir riscos futuros e intervenção restauradora utilizando materiais como cimento ionômero de vidro, resina composta ou até mesmo endodontia e reabilitação protética para casos mais complexos. RESUMOOs distúrbios de desenvolvimento no esmalte apresentam-se como anomalias de estrutura, podendo afetar ambas as dentições. Dentre eles estão a amelogênese imperfeita, a hipoplasia de esmalte e a fluorose dental. A amelogênese imperfeita é uma alteração de caráter hereditário e a hipoplasia de esmalte, um defeito quantitativo do esmalte que ocorre em consequência de problemas sistêmicos, locais e hereditários, ou até da combinação dos mesmos. A fluorose dental é uma hipoplasia gerada pela ingestão crônica de flúor durante o período da odontogênese. Todas essas alterações apresentam características clínicas semelhantes, sendo necessário muito cuidado na sua avaliação. É de extrema importância o conhecimento dessas anomalias para que o diagnóstico diferencial seja instituído e, consequentemente, o plano de tratamento seja estabelecido de forma apropriada para cada situação. Sendo assim, o objetivo do presente trabalho foi realizar uma revisão da literatura a respeito dessas três anomalias do esmalte dental. Por meio desta revisão, concluiu-se que, para estabelecer o diagnóstico diferencial entre essas alterações, assim como um correto plano de tratamento, é necessário o conhecimento das anomalias pelo profissional associado à execução de exame clínico, composto de anamnese e exame físico e, em alguns casos, de exame radiográfico. ABSTRACTThe developmental disorders of enamel are abnormalities of structure which can affect both dentitions. These abnormalities include amelogenesis imperfecta, enamel hypoplasia and dental fluorosis. The amelogenesis imperfecta is a hereditary change and enamel hypoplasia is a quantitative defect of enamel that occurs as a result of systemic problems, local and also inherited factors, or even the combination of them. Dental fluorosis is a hypoplasia caused by the chronic ingestion of fluoride during odontogenesis. All these anomalies have similar clinical characteristics, and it is necessary to be careful in their assessment. It is extremely important to know these abnormalities to establish a differential diagnosis and, consequently, a treatment plan, which can be set for each situation. Therefore, the purpose of this study was to review the literature regarding these three anomalies: amelogenesis imperfecta, enamel hypoplasia and dental fluorosis. It was concluded that to establish the differential diagnosis of these abnormalities as well as a proper treatment plan, it is indispensable the professional knowledge associated with the clinical examination. The examination has to consist of medical history and physical examination, and in some cases, x-ray examination.
ResumoIntrodução: A hipersensibilidade dentinária é uma ocorrência frequente na prática clínica; clinicamente caracterizada por dor aguda, curta e temporária, em resposta ao estímulo mecânico, químico, térmico ou osmótico causada pela exposição dentinária. Objetivo: Comparar in vivo o efeito de um material bioativo cristalino experimental e um dessensibilizante nanoestruturado na avaliação da dor de pacientes com hipersensibilidade dentinária cervical. Material e método: Trinta pacientes foram selecionados para este estudo, que foram randomicamente divididos em dois grupos (n=15) em um desenho experimental de boca dividida. Cada paciente recebeu dois tratamentos: grupo 1 (flúor gel e material bioativo) e grupo 2 (flúor gel e dessensibilizante nanoestruturado). As análises de dor foram realizadas usando uma escala visual analógica, variando de 0-10. A mensuração da dor inicial foi realizada previamente ao tratamento inicial (T 0 ) e novas análises foram realizadas semanalmente durante 3 semanas (T 1 , T 2 e T 3 ) antes da reaplicação dos materiais. A análise final da dor foi realizada 3 meses após o início do tratamento (T 4 ). O grau de redução de dor foi mensurado pela fórmula: T 0 -T período após tratamento . Os dados das mensurações de dor foram analisados pela ANOVA para medidas repetidas 2 critérios e teste de Tukey (α=0,05). Resultado: Independente do período de avaliação, não houve diferença estatística entre todos os tratamentos na redução da dor (p>0,05). O grau de dor reduziu significativamente em cada período avaliado para todos os tratamentos testados (p<0,05), aproximadamente três graus após 3 meses. Conclusão: Os materiais dessensibilizantes testados foram efetivos na redução da hipersensibilidade dentinária.Descritores: Dessensibilizantes dentinários; sensibilidade da dentina; dor. AbstractIntroduction: Dentin hypersensitivity is a frequent occurrence in dental practice. It is clinically characterized by acute, short, and temporary pain in response to mechanical, chemical, thermal, or osmotic stimuli resulting from dentin exposure. Objective: To compare in vivo the effect of an experimental crystalline bioactive material and nanostructured desensitizing on the pain assessment among patients with cervical dentin hypersensitivity. Material and method: Thirty patients were selected for this study, who were randomly assigned to two groups (n=15) in a split-mouth design. Each patient received two treatments: group 1 (fluoride gel and bioactive material) and group 2 (fluoride gel and nanostructured desensitizing). Pain analyses were performed using a visual analogue scale, ranging 0-10. ObjectiveThis study investigated less invasive protocols that do not injure peri-implant tissues during implant surface decontamination and evaluated, in vitro, the efficacyof microbiological decontamination of machined surface titanium discs with photodynamic therapy.MethodsForty eight titanium disc contaminated with 10μL of a Streptococcus sanguinis suspension were randomly divided into groups: 1) titanium disc contaminated with Ss (titanium disc suspension) without treatment; 2) titanium disc suspension rinsed with saline solution; 3) titanium disc suspension rinsed with 0.2% chlorhexidine digluconate; 4) titanium disc suspension treated with Photosensitizer Methilene Blue; 5) titanium disc suspension treated with Photosensitizer Methilene Blue associated with laser diode; 6) titanium disc suspension treated with diode laser. After the treatments, the titanium disc were submersed in 3mL of sterile brain-heart infusion broth under aerobic conditions at 37ºC for 48 hours. Three petri plates were seeded per sample and maintained under aerobic conditions at 37ºC for 48 hours, after which the number of colony forming units per milliliter was counted.ResultsThe Kruskal-Wallis test complemented by Dunn test showed that chlorhexidine digluconate eliminated titanium disc contamination (p<0.05). All the other groups (2, 4, 5, 6) had fewer colony-forming units than group 1 (p<0.05).ConclusionWithin the limitations of this study, Photodynamic Therapy reduced titanium disc contamination but was not better than 0.2 % chlorhexidine digluconate rinsing.Indexing terms: Chlorhexidine.
