Objetivo: Avaliar o perfil epidemiológico das fraturas do rádio distal em hospitais de referência em Ribeirão Preto(SP), Brasil. Não existem dados suficientes na literatura nacional que corroborem com o perfil epidemiológico das fraturas do rádio distal. Métodos: 245 pacientes apresentaram 254 fraturas do rádio distal, ocorridas entre 2014 a 2017 foram avaliadas retrospectivamente para obtenção do perfil epidemiológico. Os fatores analisados foram idade e sexo, mecanismo do trauma, sazonalidade, tipo de fratura baseada na Classificação AO, presença de exposição óssea, lesões associadas, tipo de tratamento realizado (conservador ou cirúrgico) e o tipo de implante utilizado nos tratamentos cirúrgicos. Resultados: 60,2% dos pacientes participantes eram do sexo masculino e 39,8% do sexo feminino, distribuídos de forma bimodal. A média de idade foi 45,4 anos. Fraturas expostas corresponderam a 92,1% das fraturas e 7,9% representaram as expostas. Pacientes politraumatizados representaram 62,6%. O tempo médio de internação foi 8,09 dias. Introdução: As fraturas da extremidade distal do rádio representam uma das fraturas mais comuns. Diversos fatores descritos na literatura influenciam nos seus resultados após manejo cirúrgico; como a fragmentação articular, a restauração cirúrgica da anatomia do rádio, a reabilitação pós-operatória, entre outros. Objetivo: analisar o resultado funcional dos pacientes operados de fraturas articulares completas da extremidade distal do rádio e correlacionar estes resultados com os parâmetros radiológicos comumente utilizados. Métodos: 18 pacientes entre 18 a 65 anos, submetidos ao tratamento cirúrgico das fraturas do rádio distal do tipo AO 23C, no período de janeiro de 2014 a julho de 2016. Os seguintes parâmetros clínicos e radiográficos foram avaliados e submetidos a análise estatística: ADM (amplitude de movimento) do punho e antebraço, força de pinças e de preensão, PRWE (Patient Rated Wrist Evaluation), classificação AO da fratura e parâmetros radiográficos pós-operatórios. Resultados: A análise de regressão linear mostrou correlação estatisticamente significativa considerando a inclinação radial e o desvio ulnar. Conclusão: Correlação estatisticamente significativa entre os parâmetros radiográficos e resultados funcionais é de difícil determinação. A lesão do ligamento colateral ulnar da articulação metacarpofalangeana do polegar é uma lesão comum no esquiador e outros esportes que envolvem trauma do polegar. Estas lesões, quando negligenciadas acarretam instabilidade articular, trazendo prejuízo funcional e debilidade aos pacientes. O diagnóstico precoce é fundamental, para caracterizar o padrão da lesão. As lesões completas são manejadas preferencialmente de forma cirúrgica, geralmente com excelentes resultados funcionais pós-operatórios. Apresentamos o relato de caso de quatro pacientes do sexo feminino, praticantes de esportes de diferentes modalidades com lesões do ligamento colateral ulnar da articulação metacarpofalangeana do polegar. Uma das pacientes apresentava lesão bilateral. O tratamento foi realizado de forma cirúrgica e todos os pacientes apresentaram bons resultados funcionais pós-operatórios. Nossos resultados são condizentes com os resultados observados na literatura, onde o tratamento cirúrgico acarreta bom resultado funcional, possibilitando, inclusive, o retorno ao esporte.
Introduction Comparison of different surgical techniques to treat patients with rhizarthrosis or carpometacarpal osteoarthritis of the thumb. Materials and Methods A systematic review was conducted using three electronic databases. Randomized, controlled trials in patients who underwent surgery for the treatment of rhizarthrosis were included. The literature review followed the PRISMA protocol. Results A total of 15 articles involving a total population of 958 patients were selected. Seven different surgical techniques were compared. Conclusions We conclude that no procedure is superior to another in terms of pain, physical function, overall patient assessment, range of motion, or strength. Outcome measurements should be standardized to enable better comparison between surgical techniques. Level of evidence II, Systematic Review.
Malunion of metacarpal fractures can result in severe functional and esthetic impairment to the patient, thus requiring surgical correction. Traditionally, the fixation methods most commonly used for these fractures are the Kirschner wires or the plates and screws. However, one of the options for the fixation of metacarpal fractures is intramedullary fixation with a headless screw. This minimally invasive method can also be used for corrective metacarpal osteotomies. It has the advantages of being a minimally invasive technique and allowing the early range of motion. This minimally invasive method can also be used to stabilize the corrective metacarpal osteotomies for malunion. This case report presents a minimally invasive technique for corrective dorsal metacarpal osteotomy and fixation with a headless intramedullary screw. This alternative fixation method has the advantage of low damage to soft tissues and good functional outcomes.
Introdução: A cobertura de falhas cutâneas com retalhos livres e rotacionais é uma proposta estabelecida de tratamento, principalmente com o aumento da capacidade técnica e de instrumentais. Existe uma grande variedade de retalhos descritos, e apesar do esforço para reduzir os erros de técnica, as taxas de complicações ainda são altas. Estas complicações dependem de fatores inerentes ao paciente, à cirurgia e ao manejo pós-operatório. Este estudo propõe elucidar a população submetida a tratamento de falha de cobertura com retalhos livres, rotacionais e de avanço, tal como a taxa de complicação e necessidade de reabordagem. Métodos: Estudo retrospectivo baseado em dados de prontuário de pacientes submetidos a tratamento cirúrgico de falha de cobertura com retalhos livres e rotacionais no período de janeiro de 2015 a dezembro de 2021 em um hospital terciário. Resultados: A taxa de reoperação global foi de 68,8%, sendo 85,7% nos retalhos livres e 57,8% nos retalhos de rotação (p<0,001), com diferença estatística para tempo de internação (p=0,001) e tempo cirúrgico (p<0,001), sem diferença para tamanho da falha (p=0,458). A taxa de falha global dos retalhos foi de 16,9%, sendo 23,8% nos retalhos livres e 12,5% nos retalhos de rotação (p=0,032). A falha do retalho apresentou diferença estatística para o tamanho da falha (p=0,004), tempo cirúrgico (p=0,040) e tempo de internação em dias (p=0,015). Conclusão: a taxa de falha dos retalhos e de reoperação são consistentes com os dados da literatura. As comorbidades podem ser controladas por equipes multidisciplinares de forma a não influenciarem no resultado pós-operatório.
Introdução. A Doença de Dupuytren é caracterizada por uma fibroproliferação benigna da fáscia palmar e digital, que resulta em deformidades e contraturas em flexão nas mãos e nos dedos. Diversas abordagens terapêuticas são propostas para correção da deformidade e restaurar a função da mão. O objetivo deste estudo é descrever a experiencia de um hospital terciário no manejo da doença de Dupuytren. Métodos. Estudo retrospectivo analisando 59 casos de doença de Dupuytren submetidos ao tratamento cirúrgico com a técnica da palma aberta de McCash modificada. O estudo incluiu 44 homens e 15 mulheres, com idade média de 66 anos. Destes, 16 eram diabéticos, 8 utilizavam neurolépticos, 29 eram etilistas e 20 apresentavam história familiar positiva. O tempo médio de doença no diagnóstico foi de 8,2 anos. 55,9% apresentavam bilateralidade da doença. Resultados: observamos diferença estatística no estágio de Tubiana para o 5º dedo (Teste do Qui-Quadrado p=0,060) e para a variável recidiva (ANOVA p=0,036) e uma maior taxa de recidiva em pacientes que apresentavam bilateralidade (Teste do Qui-Quadrado p=0,047). Conclusão: a técnica de McCash possui resultados satisfatórios considerando as variáveis estudadas e em concordância com a literatura.
The trigger finger is a common affection commonly in the daily hand surgeon attending. The management began with non-operative methods. The absence of improvement could appoint to different operative techniques, such as open release, mini-open approaches, and percutaneous needle approaches. With respect to the open surgical approach, there are many alternatives to release the A1 Pulley, such as scalpel, pushing knives, or scissors. This study aims to present a Retinaculotome designed as a Modified Pushing Knife, to release the A1 Pulley for the management of the trigger finger.
