Introdução. A Doença de Dupuytren é caracterizada por uma fibroproliferação benigna da fáscia palmar e digital, que resulta em deformidades e contraturas em flexão nas mãos e nos dedos. Diversas abordagens terapêuticas são propostas para correção da deformidade e restaurar a função da mão. O objetivo deste estudo é descrever a experiencia de um hospital terciário no manejo da doença de Dupuytren. Métodos. Estudo retrospectivo analisando 59 casos de doença de Dupuytren submetidos ao tratamento cirúrgico com a técnica da palma aberta de McCash modificada. O estudo incluiu 44 homens e 15 mulheres, com idade média de 66 anos. Destes, 16 eram diabéticos, 8 utilizavam neurolépticos, 29 eram etilistas e 20 apresentavam história familiar positiva. O tempo médio de doença no diagnóstico foi de 8,2 anos. 55,9% apresentavam bilateralidade da doença. Resultados: observamos diferença estatística no estágio de Tubiana para o 5º dedo (Teste do Qui-Quadrado p=0,060) e para a variável recidiva (ANOVA p=0,036) e uma maior taxa de recidiva em pacientes que apresentavam bilateralidade (Teste do Qui-Quadrado p=0,047). Conclusão: a técnica de McCash possui resultados satisfatórios considerando as variáveis estudadas e em concordância com a literatura.
Introduction: Eccrine poroma (EP) and poroid hidradenoma (PH) are rare benign neoplasms derived from eccrine sweat glands of the epithelium. Both are variants of poroid neoplasms and rarely become malignant. This report describes the clinical presentation, radiographic and pathological features as well as the treatment of a rare association of EP and PH in a large single lesion over the foot. Case Report: We present a 55-year-old male patient with a large soft tissue mass over the midfoot (approximately 7 cm in length and 4 cm width). It started as a hyperchromic black spot on the skin and has slowly grown over the last eight years. One year prior, pain increased significantly. There was no history of trauma, infection or other tumor in this part of the foot. In the physical exam, the tumor had well-defined edges, fibroelastic consistency and was not adherent to the surrounding soft tissue. Preoperative planning included radiographic exams: X-ray and magnetic resonance imaging (MRI). On the MRI, the tumor showed a low signal intensity on T1-weighted images, with heterogeneous high and low signal intensities on T2-weighted images. Strong contrast enhancement was observed after intravenous gadolinium administration in the base portion. The peripheral portion showed a low signal intensity without enhancement. We decided on total resection, and the procedure was performed in January of 2017. The surgical approach was a longitudinal dorsomedial incision over the first ray starting from the ankle joint to the distal third of the first metatarsal. The entire tumor was excised with clean margins and sent for anatomopathological analysis and histopathological examination. Histopathologic examination showed that the tumor was a poroid neoplasm consisting of both dermis (poroid hidradenoma) and epidermis (eccrine poroma) components. There were no features of malignancy. After 20 months of surgery, patient remains asymptomatic with no signs of recurrence of the tumor. Conclusion: Skin tumors must be born in mind in the differential diagnosis of a chronic mass in the foot. Surgical treatment for this type of tumor is curative and prevents recurrence and malignant change.
ResumoO poroma écrino e o hidradenoma poroide são neoplasias poroides benignas raras, derivadas das glândulas sudoríparas écrinas. Existem quatro tipos de neoplasias poroides, conforme a posição na camada da pele, sendo denominadas hidroacantoma simples, poroma écrino, tumor do ducto dérmico e hidradenoma poroide. As neoplasias poroides geralmente surgem como uma lesão solitária, com crescimento lento, podendo apresentar diferentes apresentações clínicas, como massa sobre o pé, lesão ulcerada, cisto sólido, lesão hemorrágica ou suspeita de melanoma. As extremidades são os sítios mais comuns, especialmente as mãos e os pés. No entanto, a coexistência desses dois tumores em uma única lesão é extremamente rara. A excisão cirúrgica representa o principal tratamento, podendo ser curativa, prevenindo assim as alterações malignas e as recidivas. Descrevemos um raro tumor solitário no pé com características clínicas e histopatológicas de uma associação de poroma écrino e hidradenoma poroide tratado cirurgicamente sem recidiva no acompanhamento de médio prazo.
