These results provide novel insights into the physiological and pathological significance of dermokine in the epidermis.

The central granular cell odontogenic tumor (CGCOT) is a rare benign odontogenic neoplasm composed of varying amounts of large eosinophilic granular cells and apparently inactive odontogenic epithelium. It tends to occur as a small asymptomatic swelling in the posterior region of the mandible with nonaggressive appearance. We report an unusual case of CGCOT in the maxillary region with clinical features of malignancy. The patient underwent surgical treatment, and the 2-year follow-up revealed no signs of recurrence. Central granular cell odontogenic tumor is a very rare condition with few cases reported, especially in the maxillary region. This case highlights the possibility of aggressive behavior by these lesions.

Purpose: To report a rare case of intraosseous lipoma of the mandible and to discuss the most important features of the lesion, emphasizing the diagnostic pitfall that this entity may represent for general dentists and radiologists.Case description: An 18-year-old male patient presented an asymptomatic radiolucent lesion in the mandible in the region of the teeth 43 and 44 with no clinical alteration. After the incisional biopsy the histopathological exam revealed a capsulated lesion predominantly composed of mature adipose tissue and some areas of dystrophic calcification confirming the diagnosis of intraosseous lipoma. The lesion was surgically removed and no signs of recurrence could be observed after six months of follow-up.Conclusion: Due to the unspecific clinical and radiographic features and its rarity the intraosseous lipoma of the mandible may be a diagnostic challenge for general dentists and radiologists. Therefore, the histopathological examination is required for the correct diagnosis of the lesion.Key words: Bone Tumors; diagnosis; lipoma; mandible ResumoObjetivo: Relatar um caso raro de lipoma intra-ósseo em mandíbula e discutir os aspectos mais importantes desta lesão, enfatizando a desafio diagnóstico que esta entidade pode representar para cirurgiões dentistas e radiologistas.Descrição do caso: Paciente do sexo masculino, 18 anos de idade apresentava uma lesão radiolúcida assintomática na mandíbula na região dos dentes 43 e 44 sem qualquer alteração clínica. Após a biópsia incisional, o exame histopatológico evidenciou uma lesão encapsulada composta majoritariamente por tecido adiposo benigno e algumas áreas de calcificações distróficas confirmando o diagnóstico de lipoma intra-ósseo. A lesão foi cirurgicamente removida e nenhum sinal de recorrência foi observado após seis meses de acompanhamento.Conclusão: Devido às características clínicas e radiográficas inespecíficas e à sua raridade, o lipoma intra-ósseo de mandíbula pode representar um desafio diagnóstico para cirurgiões dentistas e radiologistas. Portanto, o exame histopatológico é fundamental para o correto diagnóstico da lesão.

The bisphosphonate-related osteonecrosis of the jaw (BRONJ) associated with surgery procedures, is an entity with less than 10 years of occurrence in dental practice. The bisphosphonates (BFs) are anti-resorbing drugs highly effective in the treatment of several bone diseases, including metastasis. Although the majority of cases of BRONJ had been reported with the potent third generation of BFs, sodium alendronate (ALN), a second generation bisphosphonate, is widely used for treatment or prevent bone diseases as osteoporosis. The present experimental study aimed to analyze the possible presence of BRONJ in the alveolar process of rats treated with ALN following extraction of the mandibular second molar. For this, we used thirty 7-week-old male Wistar rats, divided into two groups, ALN (treated with alendronate) and CTL (control, treated with saline solution). The administration of ALN received daily subcutaneous injection at a dose of 2.5 mg/kg for 14 days. At that moment, the extraction was performed. After 7, 14 e 21 days of surgery, five animals of each group were euthanized and the mandibular region fixed in 2.5% formaldehyde + 2% glutaraldehyde in cacodylate buffer 0.1M-pH 7.4 and decalcified in 4.13% EDTA for 30 days. The specimens were morphologically analyzed in HE stained sections, and, histomorphometry was used to analyze the resorption of mesial and distal alveolar crests, as well as the bone formed into the alveolus. Immunohistochemistry for the noncollagenous proteins OPN and BSP and for CD105 to revel neoformed blood vessels was also performed. The osteoclasts were revealed through tartrate-resistant acid phosphatase (TRAP) histochemistry. Moreover, the cells and events of alveolar healing were examined by transmission electron microscopy. The results showed light degree of BRONJ in the ALN treated animals at all the studied periods, and the presence of latent osteoclasts near the non-resorbing crest alveolar. Bacteria were observed in contact with the non-resorbed alveolar bone. An evident delay in bone formation when compared to control group was also observed, as well as the decrease of blood vessels in early stages. The distribution of immature bone and in cement lines of OPN and BSP was considered normal. Thus, we concluded that the present protocol of ALN yielded light BRONJ areas, probably by reduced angiogenesis, lock of remodeling of alveolar, presence of bacterial colonies and evident decreased osteoblastic ativitity.

A infecção pelo HPV tem sido reconhecida como fator de risco individual para o desenvolvimento de carcinomas epidermoides de cabeça e pescoço, sendo relacionada ao desenvolvimento de lesões em pacientes que não apresentam os fatores clássicos de risco, como o tabagismo e o etilismo. O contato oral oral e oral genital possivelmente se apresentam como vias de transmissão do HPV entre humanos, justificando ainda, a relação entre o câncer e o comportamento sexual. A literatura atual sugere que os carcinomas epidermoides HPV-positivos apresentam um comportamento biológico diferenciado, e isso tem estimulado a investigação da relação entre a infecção pelo HPV e o desenvolvimento destas neoplasias, além dos possíveis benefícios de uma campanha de imunização contra o HPV na prevenção deste tipo de câncer.
