Confluent and reticulated papillomatosis of Gougerot and Carteaud is a dermatosis that despite showing characteristic clinical signs is often poorly recognized and diagnosed. The authors present a case with extensive skin involvement, discuss its association with obesity and describe dermoscopic findings making the histopathological correlation.
A Síndrome de Stevens-Johnson, responsável por lesões muco-cutâneas graves, ocorre geralmente após a exposição a certos medicamentos ou após infecções, podendo estar relacionada a eventos autoimunes. Os autores descrevem o caso de uma paciente de 47 anos que apresentou o quadro após ser submetida à terapêutica com amoxicilina. Após hospitalização e tratamento adequado, a mesma demonstrou recuperação satisfatória, sem apresentar sequelas significativas.
Cada vez mais frequente nos Serviços de Clínica Médica, a doença renal crônica pode ser acompanhada da deterioração de vários outros sistemas orgânicos. Neste contexto, o acometimento da pele pode ser retratado por meio de desordens específicas ou inespecíficas, as quais podem sinalizar o nível de comprometimento pela doença renal crônica. A deposição de sais de cálcio na pele e tecido subcutâneo especialmente, lesão denominada calcinose cutânea, consiste em uma alteração específica da doença renal crônica, podendo indicar estágios mais tardios da patologia. Os autores descrevem o caso de um paciente hipertenso e diabético, que desenvolve doença renal crônica e, no decurso da doença, evolui com calcinose cutânea. Após tratamento de suporte, diante da melhora clínica, o paciente recebeu alta hospitalar, passando a ser acompanhado regularmente pelo Ambulatório de Clínica Médica da unidade.
Fundamentos e Objetivos: Apesar do trauma térmico no grande queimado ser agudo e do pioderma gangrenoso ser uma doença imunomediada, muitos aspectos são comuns nesses dois grupos de pacientes. Defendemos que as regras usadas na determinação da percentagem de pele acometida no grande queimado sejam também usadas nos casos extensos de pioderma gangrenoso. Bem como uma rotina de suporte clínico - claramente seguida nos queimados - mas nem sempre nos pacientes com pioderma gangrenoso.Relato de caso: Paciente masculino com quadro clínico extenso de pioderma gangrenoso, não aderente ao tratamento, apresentava lesões úlcero vegetantes e cicatrizes cribiformes. Após apresentar melhora clínica, não retornou em consulta.Conclusões: Em casos graves e extensos, nos quais a percentagem de acometimento da pele supere 20% da superfície corporal, a abordagem do pioderma gangrenoso deve ser semelhante a de um grande queimado, com exame clínico e avaliação da hidratação corporal periódicos.
Variante do líquen plano, o líquen plano pilar (folicular) acomete a unidade pilossebácea e pode ocasionar alopécia cicatricial. O líquen plano pigmentoso, outra forma de apresentação do líquen plano, caracteriza-se por pápulas ou máculas hipercrômicas que podem ser pruriginosas, sendo mais frequente em pacientes melanodérmicos. Apresentamos o caso de uma paciente que apresentou simultaneamente dois subtipos de líquen plano: pilar no couro cabeludo, região cervical e antebraços e pigmentoso na face, o que é raro. A tricoscopia facilitou a visualização das alterações no couro cabeludo. Na fronte haviam manchas hipercrômicas e na região cervical pápulas foliculares eritematosas. Após a realização de biópsias dos três tipos de lesões, confirmou-se o diagnóstico de líquen plano pilar no couro cabeludo, líquen plano pigmentoso na fronte e líquen plano pilar na região cervical. Inicialmente foi tratada com corticóide tópico com melhora das lesões, mas apresentou novas lesões nos antebraços, sendo iniciado o uso de hidroxicloroquina.
